Abstract
본 연구는 신경계 손상 내담자의 연하기능의 재활을 위해 악센트 기반의 노래부르기를 적용한 가창중재 프로토콜(Accent-Based Singing Protocol; ABSP)을 개발하기 위해 수행되었다. 기존 연하재활을 위한 음악중재 프로토콜은 말 기능과 연하기능에 공통적으로 관여하는 생리 해부학적 기제를 근거로 하여 말 기능 재활에 대한 음악중재의 치료적 원리를 적용하는 방법으로 시행되어 왔다. 즉, 가창 시 호흡, 발성, 공명, 조음 등의 말 산출 단계가 유사하게 요구되는 사실을 기반으로, 말 기능 향상을 위한 중재 기법을 연하기능의 재활에도 적용하는 경우가 많았다. 실제로는 연하의 기제가 말 기능과 유사하나 실제로 동일한 기제를 공유하는 것으로 보기는 어렵다. 즉, 말 산출의 기제가 앞서 언급된 네 단계에 걸쳐 부분적으로 독립된 생리적 작용에 의해 일어나는 반면, 연하 과정은 말 산출 과정보다 매우 짧은 시간 안에 훨씬 더 복잡한 협응 기제에 의해 발생한다. 따라서 말 산출 기능을 목표로 한 음악중재의 논거 및 활동을 연하기능의 재활을 목표로 한 중재 활동으로 적용하기에는 기존의 중재 논거만으로는 설명되기 어려운 부분이 있다. 이와 같은 이유로 본 연구에서는 신경학적 손상 내담자의 연하기능 개선을 위한 악센트 중심의 가창중재 프로토콜을 개발하였다. 연구를 위한 이론적 근거로는 연하 기제를 연하 시 호흡 작용과 삼킴 작용 등의 두 경로로 구분하는 이론과, 음성 및 말 산출 과정에 작용하는 가창의 치료적 논거와 함께 연하에 관여하는 호흡 및 발성 과정에 가장 효율적으로 적용되는 악센트의 적용 원리를 기술하였다. 중재 프로토콜(ABSP)의 개발은 두 단계로 나누어 시행되었다. 개발 1단계에서는 총 8편의 기존 연하재활 음악중재 연구 고찰을 통해 중재 대상자, 중재 내용 및 논거, 연구의 주요한 논의점 및 한계점을 분석하였다. 2단계에서는 분석된 연구의 결과를 바탕으로 프로토콜을 구성하여 초안을 구안하고 이를 전문가에게 의뢰하여 타당도 검증을 시행함으로써 연하 재활에 가장 효율적으로 작용할 수 있는 음악요소로 액센트를 활용한 연하재활 가창중재 프로토콜을 개발하였다. 프로토콜을 구성하는 주요 활동은 호흡, 발성, 조음 등의 과정에 악센트를 활용하여 가창하는 형식으로, 호흡패턴 조절 및 호흡근 강화와 함께 성대 근육의 강화를 목표로 하여 구성하였다. 이를 위해 악센트의 발생 원리를 반영한 세부 진행 절차와, 내담자가 악센트를 발성하도록 하기 위한 음악의 적용 원리를 구체적으로 제시하였다. 또한 음악 요소의 효과적 적용을 위해 치료사의 음악적 전문성을 강조하여 음악중재만의 고유성과 치료적 효율성이 부각되도록 구성하였다. 본 연구에서 개발된 프로토콜은 기본적으로 비침습적인 접근이라는 점에서 기존 연하재활 접근과 구별될 뿐 아니라 적용의 방향성에 있어서도 연하재활을 위한 기존의 음악중재 접근과도 차별화된다는 특징이 있다. 즉, 연하장애 내담자들을 위한 기존의 음악중재가 가창과 말 산출 단계의 유사성을 기준으로 포괄적인 중재방법을 적용했던 것과 달리 연하 기제에 특화된 음악요소를 보다 집중적으로 활용하여 연하장애 내담자의 손상된 기능의 회복 뿐 아니라 전반적 연하기능의 향상을 도모하고자 하였고, 이 과정에서 가창의 세부적 단계에 대한 음악요소 적용 가능성의 이론적 근거를 제시했다는 점에 의의가 있다. 또한 상세하고 구체적인 시행 방법의 기술을 통해 자격을 갖춘 음악치료사라면 유사한 수준의 중재를 제공할 수 있도록 하는 지침을 마련하기 위한 중재 프로토콜 개발을 시도했다는 데 또 다른 의의가 있다. 이러한 점은 향후 의료 환경 내 음악치료 중재 분야에서 다른 신체 기능의 재활을 목표로 한 프로토콜을 개발하기 위해 시행되는 연구들에 대해 지침의 수준 및 기술 방법을 안내하는 역할을 할 수 있을 것으로 기대되는 부분이다. 이러한 연구의 의의에도 불구하고, 프로토콜의 개발 이후 효과성 검증 및 실행화 단계가 이루어지지 못한 것이 연구의 제한점이라 할 수 있다. 개발된 프로토콜의 실제 실행을 통해 프로토콜 적용의 가능성 및 효과성을 확인하고 중재 과정을 직접 경험한 치료사와 내담자의 의견을 수렴하는 과정을 시행하고자 하였으나, 연구의 진행 시점에서 중재가 시행되기 어려운 사회적 상황이 장기간 지속되어 이와 같은 과정으로 연결되지 못한 점이 매우 아쉽다. 따라서 본 연구에서 개발된 중재 프로토콜을 신경학적 손상 내담자의 연하재활에 적용하는 후속 중재 연구를 통해 프로토콜의 적용 가능성 및 효과성이 검증되어, 많은 연하장애 내담자의 기능 개선을 위해 효율적으로 적용됨과 동시에 음악중재의 치료적 전문성이 다시 한 번 입증되기를 기대하는 바이다.;This study was to develop an accent-based singing protocol for the rehabilitation of swallowing function in neurologically impaired patients. The existing music intervention program for dysphagia rehabilitation has been implemented on the premise of the anatomical and physiological similarities between singing and speech production, based on the anatomical mechanism shared by speech and swallowing functions. In other words, although the step-by-step mechanism of speech production such as breathing, vocalization, and articulation divided according to the musical elements of songs, the mechanism is similar, so the singing technique has been applied to the function of speech production. Furthermore, in many cases, treatment measures to improve speech function have been used for the rehabilitation of swallowing function. Although the mechanism of swallowing is similar to that of speech, it is hard to consider that they share the same mechanism. That is, while the mechanism of speech production is caused by partially independent physiological actions through the above mentioned four stages, the process of swallowing is caused by a much more complex coordination mechanism within a much shorter time than the process of speech production. Therefore, it is difficult to explain the application of the arguments and activities of music intervention for the function of speech production to intervention activities with the goal of treatment the swallowing function only with the existing intervention arguments. For this reason, this study aimed to present a draft music intervention protocol with specific musical elements to directly mediate the swallowing function in patients with dysphagia. For the rationale for the study, the results of previous studies that divided the process of swallowing into two partial mechanisms, including actions related to breathing during swallowing and those involved in the act of swallowing, were presented. This study also described the neurological mechanism through physiological responses caused by musical elements that are related to the mechanism of speech production. To derive the components of the protocol, this study was conducted with two sub-studies. Study 1 aimed to review the literature to analyze the relationship between each element and trends were analyzed based on the research subjects, the content, and arguments of intervention and the results of the functional evaluation from the existing studies on music intervention for dysphagia rehabilitation. In Study 2, to propose and develop an accent-based singing intervention protocol, literature and reference materials were collected and analyzed for swallowing techniques, other than music intervention, being performed for dysphagia rehabilitation. For the research of music intervention for dysphagia rehabilitation, a total of eight studies were analyzed. In study 2, domestic and international research on dysphagia rehabilitation as well as intervention studies on dysphagia rehabilitation conducted in the field of nursing and occupational therapy in Korea were analyzed centering on the techniques and treatment goals. In study 1, the characteristic factors of intervention such as the functional level of the study subjects for the intervention protocol, activity components, intervention period, and duration were derived. The range of subjects applicable to the intervention included neurologically impaired patients who were able to make vocal sounds, and it was considered unnecessary to put limitations on cognitive and other physical functions. In study 2, rehabilitation techniques were classified based on the breathing and vocalization activities derived from study 1, based on which detailed treatment goals were developed according to the application method of the breathing and swallowing functions in patients with dysphagia. As a result of the study, it was analyzed that the most effective musical element for swallowing rehabilitation was an accent, and a music intervention protocol for dysphagia rehabilitation was developed using the element. This study has significance in attempting to improve the function of swallowing in patients with dysphagia through an approach that emphasizes the unique advantages of music intervention by focusing on the application of musical elements specialized in the swallowing mechanism and emphasizing therapists' musical expertise, while previous music intervention for patients with dysphagia introduced a comprehensive intervention method based on the function of speech production. However, this study also has limitations in that it failed to carry out an implementation stage with the verification of its effectiveness after developing the protocol. In this regard, it is expected that the intervention protocol proposed in this study will lead to follow-up research to help patients with dysphagia effectively promote their physical and psychological functions.
