Abstract
흑색종 환자에서 성별과 나이에 따른 유전자 발현의 차이와 예후 및 예측적 의미를 조사하였다. 첫째, 큰 코호트 자료를 이용하여 연령과 성별에 따른 흑색 종 환자의 임상적 예후를 평가하여, 젊은 여성 환자는 유리한 예후가 있음을 확인하였다. 둘째, 흑색종 환자의 전사체 데이터를 이용하여 생존에 영향을 미치는 경로 분석을 수행하고 성별 및 연령 그룹 사이에서 경로의 발현 수준을 비교하였다. 정상 피부 조직과 비교하여 흑색종에서 상당히 상향 조절된 유전자를 추출하였다. 이들 유전자 중에서, 발현 수준에 따라 생존에 크게 영향을 미치는 유전자를 선택하고 관련 경로를 분석하였다. 종양 조직에서 상향 조절되고 생존에 영향을 미치는 가장 중요한 경로는 프로그램 된 세포 사멸 단백질 [PD]-1, 인터페론-γ 및 인터페론-α/β 경로였다. 여성의 경우, 환자의 면역 신호 발현이 남성보다 높았으며 연령에 따라 그 정도는 감소했다 (63.5 %, 53.8 % 및 47.6 %). 남성의 경우 감소 경향이 미미했다 (47.6 %, 50.0 % 및 41.6 %). 60 세 이하의 환자에서, 여자는 남자보다 유리한 생존율을 보였다 (p = 0.055). 면역 신호가 높은 환자를 제외할 경우 성별 간에 생존 차이가 관찰되지 않았다 (p = 0.6). 셋째, 흑색종 환자의 전사체 데이터를 이용하여 방사선 반응에 영향을 미치는 경로 주석을 수행하고 성별 및 연령 그룹 사이에서 경로의 발현 수준을 비교하였다. 여성 및 젊은 환자는 각각 남성 및 노인 환자보다 방사선 치료에 대한 반응률이 높은 경향이 있었다. 방사선 요법에 대한 완전한 반응을 보이는 환자는 방사선 치료 후 진행성 질환을 가진 환자보다 호중구 탈과립이 유의하게 상향 조절되었는데, 이는 상향 조절된 면역 신호가 또한 방사선 요법에 대한 반응에 영향을 미친다는 것을 시사한다. 결론적으로, 젊은 여성 흑색종 환자는 나이 많은 여성 및 남성 환자보다 높은 면역 신호 발현을 보였다. 이 상향 조절된 면역 신호는 젊은 여성 환자의 생존 및 방사선 치료에 대한 반응을 향상시키는 것으로 생각된다.;This study aims to investigate the difference of gene expression and its prognostic and predictive significance according to age and gender in melanoma. First, we evaluate the clinical prognosis of the melanoma patients according to age and gender using a big cohort data and confirmed that younger and female patients had a favorable prognosis compared to older female and male patients. Second, we performed pathway annotations which impact on survival using transcriptome data of melanoma patients and compared the expression level of the pathways among the gender and age group. Significantly upregulated genes in melanoma compared to normal skin tissues were extracted. Among these genes, genes that significantly affected survival according to expression level were selected, and pathway annotation was performed. The patient proportion with high/low expression of the most significant pathways was analyzed in each age (<50, 50–59, 60≤) and gender group. Survival was analyzed according to age, gender, and pathways. The most significant pathways that were upregulated in tumor tissues and also had impacts on survival were programmed cell death protein [PD]-1, interferon-γ, and interferon-α/β pathways. In women, the immune signaling rate in patients was higher than men and decreased with age (63.5%, 53.8%, and 47.6%). In men, the decreasing tendency was minimal (47.6%, 50.0%, and 41.6%). In patients aged <60 years, women had a favorable survival rate than men (p =0.055). Except for patients with high immune signaling, no survival difference was observed between genders (p=0.6). Third, we performed pathway annotations which impact on radiation response using transcriptome data of melanoma patients and compared the expression level of the pathways among the gender and age group. Female and younger patients had a tendency of higher response rates to radiotherapy than male and older patients, respectively. Patients with complete response to radiotherapy showed significantly upregulated neutrophil degranulation than those with progressive disease after radiotherapy, suggesting that upregulated immune signals also impact on response to radiotherapy. In conclusion, younger female melanoma patients had high immune signaling than older women and men. This immune signaling may improve survival and response to radiotherapy of the younger female patients.
