파킨슨병 환자를 위한 운동·정서 상호연결적 가창 중재모델 개발

Abstract

This study developed a motor and emotional inter-connectivity singing therapy model for Parkinson's patients. As a study of evidence-based music therapy, theoretical considerations were taken in terms of music related neuroplasticity and music therapy for Parkinson's disease, and analysis of singing interventions of existing clinical researches was analyzed according to the model development process to provide the theoretical framework for the development of a intervention model. Study 1 selected 20 pieces published in international journals from the 1990s to 2020 of Parkinson's disease patients, conducted singing intervention analysis through literature review, and developed a therapeutic model through a descriptive integration process. The clinical and theoretical directions were derived. In Study 2, through the structure of the intervention model development process and the structuring of the intervention model, a singing therapy model was developed from the perspective of the inter-connectivity of music and how the musical elements and singing are mediated for Parkinson's disease patients. As a result of the study of study 1, the musical elements of singing intervention, the therapeutic function of music activity and the proposal for therapy of singing, the suggestion of the song(music) stimulation process, the singing and music inter-connectivity function. We derived the interconnected singing activity and several treatment strategies. In the study 2, based on the results of the study 1, the motor and emotional inter-connected process analysis of the music elements of singing intervention for Parkinson's disease clients and the intrinsic and others based on the singing principle and the motor and emotional neural processes A singing intervention model has been developed based on the theoretical framework of a therapeutic model that not only provides a stimulus but also induces an emotional response to activate motor function. Inter-connectivity of motor and emotional domains by music therapy functions of rhythmic elements(meter, rhythm, tempo), tonal elements(melody, chord, and mode), and structural elements(form-phrasing, pitch, dynamic, lyrics) were main therapeutic factors as the theoretical framework. The motor and emotional inter-connectivity singing therapy was developed based on the process in which the motor areas (synchronization, vocal control, and vocal coordination) are promoted by motor and inter-connected with emotion through motor optimization. In other words, arousal by song(music) is stimulated, and static emotion and intrinsic motivation in the emotional domain such as emotional induction and emotional regulation are improved. The mechanism of interconnected singing related to respiratory, vocalization, and articulation of Parkinson's disease patients through the inter-connectivity of motor and emotional domain was derived by singing. This study is meaningful in that it systematically and therapeutically examines how therapeutic musical elements and singing activities are mediated through the development of a motor-emotional inter-connectivity singing therapy model for Parkinson's patients. The motor and emotional inter-connectivity singing therapy model for Parkinson's patients developed in this study needs to be continuously supplemented by verifying specific effectiveness according to the symptom level and treatment goals of Parkinson's patients in a clinical study in the future.;본 연구는 파킨슨병 환자를 위한 운동·정서 상호연결적 가창 중재모델을 개발하는 것이다. 파킨슨병은 다른 퇴행성 신경계 질환과 구별되는 신경학적 기전에 의한 움직임의 자동성 및 음악적 리듬성의 신경학적 손실로 인한 운동성 장애와 정서의 경험 및 표현 부족이라는 임상적 특성을 가지고 있다. 본 연구에서는 이 부분을 음악 중재로 보완할 수 있도록 파킨슨병 환자를 위한 운동·정서 상호연결적 가창 중재모델을 개발하였다. 먼저, 파킨슨병 환자의 음성 및 정서적 특성과 치료적 가창의 원리와 운동·정서 신경 프로세스, 음악에 대한 신경가소성, 음악치료 메커니즘 측면에서 이론적 고찰하고, 선행연구들의 가창 중재를 음악 요소와 연계된 음악 활동으로 분석하여 치료모델 개발의 적용점과 이론적 구성틀을 개발하였다. 선행연구들의 가창 중재를 분석한 연구[1]은 1990년부터 2020년까지 파킨슨병 환자를 대상으로 진행한 가창 중재 연구 중 국제학술지에 게재된 20편을 선정하여 문헌 고찰을 통해 가창 중재를 분석하고, 서술적 통합과정 거쳐 치료모델 개발의 방향성과 임상적, 이론적 적용점을 도출하였다. 연구[2]에서는 치료모델 개발 구조화를 통해 파킨슨병 환자를 위한 가창의 치료적 중재화를 운동·정서 상호연결적 관점에서 가창 중재모델을 개발하였다. 연구[1]의 가창 중재 분석 결과로는, 파킨슨병 환자를 위한 가창 중재의 음악 요소와 연계된 음악 활동의 치료적 기능과 가창의 중재화에 대한 제안, 노래(음악) 자극의 반응 프로세스 분석, 가창의 운동·정서 상호연결 기능 등이 도출되었다. 파킨슨병 환자는 보컬라이징과 가창을 통해 호흡, 발성, 얼굴 근육의 움직임 촉진, 음성-운동 상호연결 기능 활성화, 선율의 리듬(시간적)과 음정(공간적) 특성 활용, 선율 재산출에 필요한 운동·정서 기능 활성화, 가사와 연관된 움직임의 조직화, 제시된 선행 악절의 모방 및 재창조, 목소리 즉흥연주의 자발적 참여, 정서와 상호교류의 소통, 음악 안에서 정서적 연결 및 공감 형성 등으로 운동·정서 상호연결 기능이 도출되었다. 연구[2]에서는 연구[1]의 연구 결과를 바탕으로 파킨슨병 환자를 위한 구조화된 보컬라이징과 가창 중재의 음악 요소의 운동·정서 상호연결적 프로세스 분석과 가창의 원리와 운동·정서 신경 프로세스에 기반하여 내재적, 외재적 자극을 제공할 뿐만 아니라 정서적 반응을 유도하여 운동 기능을 활성화시킬 수 있는 치료모델의 이론적 구성틀을 개발하였다. 가창 중재모델은 리듬 요소군(박자, 리듬, 템포), 조성 요소군(선율, 화음, 선법), 구조적 요소군(형식-악절, 음고, 음량, 가사) 등 음악 요소의 치료적 기능에 의한 운동과 정서 영역의 상호연결을 바탕으로 개발되었다. 운동·정서 상호연결적 가창 중재는 운동 영역(동조화, 발성 조절, 발성 협응)의 운동 촉진과 운동 최적화를 통해 운동과 정서가 상호연결된다. 노래(음악)와 가창에 의한 각성자극, 정서유발, 정서조율로 정서 영역에서의 정적 정서와 내재적 동기가 강화되면서 운동과 정서 영역이 상호연결된다. 이러한 가창의 운동과 정서 영역의 상호연결 과정을 통해 파킨슨병 환자의 호흡, 발성, 조음과 관련된 상호연결적 가창의 메커니즘이 도출되었다. 호흡과 관련해서는 마디별 구조에 따른 호흡 길이 및 지점 조절(쉼표), 긍정적 정서에 기반한 안정화된 호흡 패턴, 발성과 관련해서는 리듬과 음고 조절을 통한 음성 질 향상, 악절 전개에 따른 음량 조절이 해당되며, 조음과 관련해서는 가사 반복을 통한 조음 촉진, 단계별 난이도(모음-자음)에 따른 조음 강화를 의미한다. 운동·정서 상호연결적 가창 중재모델에서 가사는 관련적 의미 또는 연상, 장·단기 기억을 통한 정서 재경험, 구조화된 언어리듬-선율리듬으로 활용되어 상호연결적 가창 메커니즘에 해당되었다. 이러한 음악 요소들과 운동 및 정서 영역의 상호연결적 가창 메커니즘을 중심으로 파킨슨병 환자를 위한 운동·정서 상호연결적 가창 중재모델이 개발되었다. 본 연구는 선행연구의 가창 중재들과 구별되게 파킨슨병의 운동성 장애, 정서의 경험 및 표현 부족, 점진적 음성 장애의 특성을 고려하여 음악 요소와 구조화된 보컬라이징과 가창이 어떻게 중재화 되는지를 체계적으로 살펴보고, 파킨슨병 환자에 특화된 운동·정서 상호연결적 가창 중재모델을 개발하였다. 가창 중재는 감각운동활동으로서 감각운동 시스템과 감각적 정서 반응을 함께 유도하는데, 가창에 의한 신경가소성과 음악치료 측면에서 파킨슨병 환자의 운동·정서적 측면을 상호보완 또는 통합적으로 지속적인 도파민 자극의 치료적 프로세스 측면에서 치료모델 개발을 시도한 것에 의의가 있다. 따라서, 본 연구를 기반으로 퇴행성 신경계 질환 전문음악치료사와 의료진이 함께 파킨슨병과 음악에 대한 기초연구와 치료목표에 따른 구조화된 프로토콜 개발 및 효과성 검증 등 지속적으로 보완될 필요가 있는 것으로 사료된다.
