- 최근 전세계적으로 서구화된 식습관과 운동부족으로 인한 섭취-소비 에너지 불균형으로 비만은 계속적으로 증가하고 있는 추세이다. 이렇게 비만인구가 증가함에 따라 여러 대사질환과 함께 비 알코올성 지방간의 발병률이 증가하고 있다. 비 알코올성 지방간이란 유의한 알코올 섭취 없이 간 내에 중성지방이 축적되는 질환으로, 단순히 간에 중성지방이 축적되는 지방간에서 염증으로 발전한 지방간염과 간경화로 발전한 이후 간암까지의 발전가능성이 높다. 초기에 회복 가능한 상태에서의 치료가 매우 중요하지만, 현재 치료제로 인정된 약물은 없다.
PAI-1은 tissue-type plasminogen activator (t-PA)와 urokinase-type plasminogen activator (u-PA)와 결합하여 plasmin의 생성을 막아 세포기질단백의 분해와 섬유소 용해 과정을 억제하게 된다. 또한, PAI-1은 제 2형 당뇨병, 심혈관계 질환, 비 알코올성 지방간과 같은 병적인 상태에서 상향 조절된다고 잘 알려져 있다. 최근 연구들에서 PAI-1 결핍이 담관결찰로 유도된 간 섬유화를 억제하고, 고지방식이로 유도된 비만과 인슐린 저항성을 완화하는 것을 관찰하였다. PAI-1억제제의 보호효과는 신장, 폐, 지방세포, 혈관 등 이미 많은 연구에서 실험된 바가 있지만, 간 질환에서는 아직 연구되지 않았다.
본 연구에서는 출혈이 거의 없는 새로운 PAI-1 억제제인 TM5441의 1) 보호 및 2) 치료효과와 3) 간세포에 PAI-1이 직접적으로 미치는 영향과 PAI-1을 억제하였을 때의 효과를 확인해보고자 실험을 진행하였다. TM5441은 10주 고지방식이로 유도된 인슐린저항성과 간 내에 축적된 중성지방이 유의하게 감소시켰다. 간 내의 염증과 섬유화도 TM5441에 의해 유의하게 개선되는 것을 확인하였다. 또한 지방간을 악화시키는 미토콘드리아 기능장애도 TM5441처리 시 개선되는 것을 확인하였다.
인간간암세포주인 HepG2 세포에서 tumor necrosis factor-α (TNF-α) 로 유도된 미토콘드리아 기능장애에서 PAI-1 억제가 어떠한 효과를 가지는지 알아보기 위해서 TM5441과 PAI-1작은 간섭 리보핵산처리를 하였다. TNF-α 처리로 손상된 미토콘드리아의 기능이 PAI-1을 억제하였을 때 개선되는 것을 PGC1α, mtDNA, TFAM, NRF1, NRF2 의 mRNA발현으로 확인할 수 있었다.
이와 같은 결과는 TM5441이 고지방식이에 의해 유도되는 대사성 스트레스 상태로 유도된 간 손상을 완화하고, 미토콘드리아 생체 내 합성 관련유전자들의 발현을 높임으로서 비 알코올성 지방간을 효과적으로 개선할 수 있음을 시사한다. TM5441의 미토콘드리아 기능 향상에 관련된 기전은 추가적인 실험이 필요하겠지만, 본 연구는 TM5441이 비만으로 유도된 비 알코올성 지방간과 관련질병의 효과적인 치료 타겟이 될 수 있음을 제시하였다.;Plasminogen activator inhibitor 1 (PAI-1), a 50 kDa glycoprotein, is the major physiological inhibitor of tissue-type and urokinase-type plasminogen activator. Plasma and hepatic PAI-1 levels are increased in various liver diseases including non-alcoholic fatty liver disease (NAFLD), and PAI-1 serves as a potential predictor for liver steatosis and fibrosis. In addition, PAI-1 null mice show improved insulin sensitivity under high-fat and high-sucrose diet (HF/HSD)-induced metabolic stress. The present study was conducted to examine whether a novel, orally active PAI-1 inhibitor devoid of bleeding episode, TM5441, (5-chloro-2{[(2-{[3-(furan-3-yl)phenyl]amino}-2-oxoethoxy)acethyl] amino}benzoic acid, has therapeutic as well as preventive effect on high-fat diet (HFD)-induced NAFLD progression.
10-week-old C57BL/6J male mice were fed an HFD for 10 weeks, and TM5441 was daily administered with the initiation of HFD at a dose of 20 mg/kg by oral gavage. Also, 5-week-old mice were used and TM5441 was administered from 5 weeks after the HFD when glucose intolerance was accompanied. TM5441 attenuated HFD-induced hepatic accumulation of triglyceride and insulin resistance (phosphorylation of Akt, GSK3β, and JNK) in mice. HFD-induced hepatic inflammation including upregulation of proinflammatory cytokine expression and macrophage infiltration (F4/80 staining) were prevented by TM5441. Delayed treatment with TM5441 also attenuated HFD-induced liver injury. Mechanistically, HFD-induced suppressed hepatic mitochondrial biogenesis (PGC-1α and COX4-i1) was effectively blocked by TM5441. In addition, TM5441 inhibited TNF-α-induced mitochondrial dysfunction, and PAI-1 induced mitochondrial dysfunction in a concentration-dependent manner, suggesting that PAI-1 mediates TNF-α-induced mitochondrial dysfunction.
Taken together, the present data demonstrated that PAI-1 may directly affect hepatic mitochondrial dysfunction leading to NAFLD and suggest that PAI-1 inhibitor, TM5441, may become a novel therapeutic agent targeting HFD-induced NAFLD.
