노래심리치료를 통한 말기 암 환자의 심리적 안녕감 증진 사례

Abstract

본 연구는 노래심리치료를 통한 말기 암 환자의 심리적 안녕감 증진에 대한 사례연구이다. 연구의 대상은 의료적 처치에도 불구하고 전이와 재발을 경험하는 가운데 병이 진행 및 악화되고 있는 재가의 노인 말기 암 환자로 총 두 명이 본 연구에 참여하였다. 말기 암 환자의 특성상 3회기의 단회기적 접근을 두 주에 걸쳐 시행하였으며, 환자의 상황과 참여 수준에 따라 노래감상, 노래회고, 노래대화, 가창 등의 노래심리치료를 적용하였다. 60세 이상의 노인 말기 암 환자의 필요를 기반으로 심리적 안녕감 증진을 위해 긍정적 정서경험, 삶의 의미와 목적 상기, 긍정적 변화 확인하기를 회기별 목표로 설정하였으며, 노래심리치료 과정에 따른 대상자의 심리적 안녕에 대한 반응들을 언어적·비언어(음악)적으로 분석하였다. 말기 암 환자를 대상으로 한 노래심리치료는 대상자의 심리적 안녕감을 향상시킬 수 있었으며 따라서 본 연구는 임상 현장에서 말기 암 환자의 심리적 돌봄을 위한 중재 시 노래심리치료 적용을 위한 근거 자료로 활용될 수 있을 것이다. 특별히 대부분 노인층을 대상으로 하는 호스피스·완화의료에서 음악을 통한 심리적 중재 시 기초자료로 활용될 수 있으리라 사료된다.;This study is a case study of psychological well-being of terminal cancer patients through song psychotherapy. The subjects of this study were two elderly patients with terminal cancer at home who developed metastases and recurrences despite medical treatment. Due to the characteristics of terminal cancer patients, three short-term approaches were taken over two weeks, and song psychological treatment such as song listening, song reminiscence, song discussion, and singing was applied according to the patient's situation and level of participation. Based on the needs of elderly terminal cancer the goal was established experiencing positive relationship, finding meaning and purpose of life and identifying positive changes to improve psychological well-being. And the patient's responses to psychological well-being were analyzed verbally and non-verbally. Song psychotherapy for terminal cancer patients could improve psychological well-being of the two elderly patients. Therefore, this study could be used as evidence for applying song psychotherapy to psychological care of terminal cancer patients. Especially in hospice and palliative care for the elderly, it can be used as a basic data for psychological intervention through music.
