치아 총생 심화도에 따른 분리작업방법에 의한 가상셋업의 비교

Abstract

본 연구의 목적은 가상셋업 상에서 개개 치아를 분리하는 방법에 따라 셋업 모델의 차이가 있는지 평가하고, 총생의 양에 있어서도 인접치를 분리하는 방법에 따라 셋업 모델에 차이가 있는지 비교, 평가하는 것이다. 본 연구는 2017년 1월부터 2019년 8월까지 이화여자대학교 목동병원 치과진료부에 부정교합을 주소로 내원하여 교정 진단을 받은 환자 중, TriosⓇ (3Shape dental systems, Copenhagen, Denmark) 로 스캔한 STL 파일이 있는 고도의 총생이 있는 악궁 20케이스, 경도의 총생이 있는 악궁 20케이스를 선별했다. 가상 셋업 프로그램인 Ortho AnalyzerⓇ (3Shape dental systems, Copenhagen, Denmark)로 먼저 치아분리를 진행했고, 인접치를 1줄과 2줄로 조합해서 분리하는 combined-cutting line method, 인접치를 1줄로만 분리하는 1-cutting line method, 인접치를 2줄로만 분리하는 2-cutting line method를 사용했다. 그 후, 치아분리한 모델을 각각 가상셋업 하여 악궁둘레, 악궁길이, 견치간 너비, 소구치간 너비, 대구치간 너비를 측정하고, 1-cutting line method에서 combined-cutting line method를 뺀 값, 2-cutting line method에서 combined-cutting line method를 뺀 값을 각각 구했다. 통계분석은 SPSS 23.0(for Windows, NY, Armonk, IBM corporation)을 사용했으며, 고도의 총생과 경도의 총생이 있는 케이스 각각을 paired t-test을 통해 항목들의 차이를 검증하고, 고도의 총생과 경도의 총생 간 수치 변화량 차이는 independent two-samples t-test을 통해 유의성을 검증하였다. 그 결과, 고도의 총생 케이스에서는 1-cutting line method에서 combined-cutting line method를 뺀 값과 2-cutting line method에서 combined-cutting line method를 뺀 값이 악궁 둘레, 견치간 너비 제2소구치간 너비, 악궁 길이에서 통계적으로 유의한 차이가 있었고(p＜0.05), 경도의 총생이 있는 case에서는 각 항목들 모두 유의한 차이가 없었다. (P＞0.05) 고도 총생 case와 경도 총생이 있는 케이스에서 각 항목들의 수치 변화량을 비교했을 때, 고도 총생 케이스에서는 각 항목들의 수치 변화량이 크게 나타났고 악궁 둘레, 견치간너비, 제2소구치간 너비, 악궁 길이에서 통계적으로 유의한 차이를 보였고, 특히 악궁 길이에서 가장 큰 차이를 보였다.(p＜0.05) 또 1줄로만 치아를 분리를 할 때는 셋업 모델 상 악궁 둘레, 견치 간 너비, 소구치 간 너비, 대구치 간 너비가 커지는 경향성 및, 2줄로만 치아를 분리할 때는 작아지는 경향성이 있었다. 그 항목들의 차이는 고도 총생 케이스에서 더 큰 것으로 나타났다. 치아분리 과정은 가상셋업의 완성도 및 결과에 많은 영향을 주고, 특히 고도 총생 케이스일 경우 영향을 많이 받는다. 따라서 임상적으로 고도 총생 케이스를 가상셋업 할 때는 총생 양상에 따라 인접치를 1줄 및 2줄로 적절히 선택해서 보다 신중한 치아 분리를 행해야 한다는 결론을 얻었다.;The purpose of this study is to evaluate the tooth segmentation method on the virtual setup model, and to compare the differences between mild and severe crowding cases. Tooth segmentation was performed on 3D virtual setup software. For each case, tooth segmentation was performed in three different ways; a combined-cutting line method of separating the proximal surface with one and two cutting lines, only 1-cutting line method, only 2-cutting line method. After tooth segmentation, each model was virtually set-up and the measured values were the arch perimeter, arch length, inter canine width, inter premolar width, and inter molar width. The values of 1-cutting line method cases and 2-cutting line method cases were compared with those of combined-cutting line method. The paired t-test, independent two-samples t-test were performed(p<0.05) for statistical analysis. The results are as follows: For the severe crowding cases, 1-cutting line method and 2-cutting line method in comparison to the combined-cutting line method presented statistically significant difference in the value of arch perimeter, inter canine width, inter 2nd premolar width, arch length on the severe crowding cases.(p＜0.05) For the mild crowding cases, the result in comparing the gap of values turned out to be not statistically significant. The severe crowding cases showed a large range of value difference depending on the tooth segmentation methods. There were statistically significant differences.(p＜0.05) The study showed that when performing a virtual setup of the severe crowding, it is important to carefully select the tooth segmentation method considering crowding patterns.
