간호윤리에 대한 통합주의적 접근 필요성 고찰

Abstract

한국 간호사의 높은 이·퇴직 문제를 해결하기 위해서 근본적으로는 간호사의 근무 환경과 처우 개선이 중요하다. 그럼에도 불구하고 간호사의 희생, 헌신, 봉사를 강조하는 태도 역시 목격되곤 한다. 필자는 이러한 강조가 간호윤리를 잘못된 방향으로 이끌 수 있다고 판단하며, 간호윤리의 균형잡힌 개념 정립이 필요함을 주장한다. 간호윤리가 간호사의 열정을 부각시키고 간호사의 희생과 헌신을 강조한다면, 이러한 간호윤리는 현실적으로 실현 불가능한 이상적인 간호사 상을 제시함으로써, 전문직 수행에 필요한 현실적인 윤리규범의 중요성을 상대적으로 약화시키거나 간호윤리의 역할과 간호윤리교육의 내용까지도 오도시킬 우려가 있다. 그러므로 필자는 전문직 윤리로서의 간호윤리가 간호사의 전문직업성 확보에 기여하는 방향으로 나아가야 함을 주장한다. 본 논문에서는 간호윤리강령의 분석을 토대로 개별윤리로서의 간호윤리가 어떤 특징을 지니고 있는지 분석하였다. 분석 결과, 한국간호사 윤리강령과 윤리지침 각각의 항은 그 내용에 따라 규범, 이상, 덕으로 구분할 수 있고, 이는 버나드 거트(Bernard Gert)가 정의한 도덕의 구성요소와 일치하였다. 필자는 간호윤리의 기능과 역할에 대한 국내 문헌에서 국내 연구자들이 강조하고자 하는 측면이 무엇인지에 따라 간호윤리에 대한 규범 중심주의, 이상 중심주의, 덕 중심주의로 대별해 보았다. 간호윤리에 대한 위 세 가지 접근법에 따른 장단점을 비교해보았고, 이를 바탕으로 필자는 간호윤리에 있어 통합주의적 접근의 필요성을 주장하였다. 간호윤리에 대한 통합주의적 시각에서는 간호사로서 지켜야 할 규범을 준수하고, 윤리적 이상을 지향하며, 필요한 덕을 갖추고자 정진하는 간호사를 윤리적인 간호사로 본다. 그러므로 간호윤리교육 또한 규범, 이상, 덕의 요소를 조화롭게 다루며, 교육 목적 및 목표에 따라 개별 요소를 적절하게 다루고 평가해야 할 것이다. ;Improving the working environment and treatment of nurses is essential in order to solve the problem of high turnover and retirement of nurses in Korea. Nevertheless, attitudes that emphasize sacrifice, dedication and service of nurses are often observed. I believe that such an emphasis leads nursing ethics in the wrong direction, and further argue that it is necessary to establish the balanced concept of nursing ethics. If nursing ethics only highlight nurses’ passion and emphasize their sacrifice and dedication, then nursing ethics may neglect substantive ethical standard necessary for professional performance or misguide the role and educational content of nursing ethics by presenting a model of an ideal nurse that is practically impossible to realize. Therefore, I argue that nursing ethics as a professional ethics should begin to contribute to further securing professionalism in the field of nursing. In this paper, based on the analysis of the Code of Ethics for Korean Nurses, I analyzed the characteristics of nursing ethics as particular ethics. As a result of this analysis, the contents of the Code of Ethics for Korean Nurses were divided into norms, ideals and virtues, which were consistent with the components of morality defined by Bernard Gert. I have classified the views of Korean researchers on nursing ethics into norm-centeredness, idealism, and virtue-centeredness in nursing ethics based on what aspects they want to emphasize in nursing ethics. I compared the advantages and disadvantages of the above three approaches to nursing ethics, and based on this, I have asserted the need for an integrative approach in nursing ethics. From an integrative view of nursing ethics, ethical nurses are nurses who adhere to the norms they should follow as nurses, pursue ethical ideals and strive to have the necessary virtues. Therefore, nursing ethics education should also deals with the elements of norms, ideals and virtues in harmony, and each element should be appropriately considered and evaluated according to the educational goals and objectives.
