Abstract
일본의 의료보험제도는 비교적 낮은 GDP대비 의료비 비중으로 양질의 건강수준을 제공한다고 좋은 평가를 받고 있다. 따라서 본 연구의 목적은 일본의 의료보험제도를 고찰함으로써 한국 의료보험제도의 개혁방향을 제시하고자 한다. 연구방법은 문헌고찰을 통해 ‘일본의료보험제도’라는 주제로 발표된 국내 학회지 논문들을 분석하였다. 1922년 일본은 처음 국민의료보험을 제정하고 이후 1961년 빠르게 전국민 국민보험가입이라는 성과를 나타냈다. 2차세계전 이후 경기침체로 인해 의료비 국고재정이 악화되었으나 정부는 재정안정화를 위해 끝없이 노력하였다. 또한 고령화로 인해 노인을 위한 고령자의료보험을 따로 창설하였다. 시행 당시 노인은 무료로 의료서비스를 제공받을 수 있었지만 급격한 의료비증가로 후기고령자제도를 통해 노인들도 의료비를 적정한 수준에서 부담하게 하였다. 한국은 처음 의료보험 제정 당시 일본의 의료보험을 기반으로 만들어져 비슷한 면이 많다. 본 연구에서는 수가체계, 본인부담률, 혼합진료를 중심으로 두 나라의 의료보험을 비교하였고, 이를 통해 앞으로 다가올 한국의 제도개혁 방향을 제시하고자 하였다. 현재 한국은 고령화가 빠르게 진행되고 있고, 의료에 대한 본인부담금은 계속 올라가고 있다. 그에 비해 일본은 DPC라는 독자적인 수가체계가 있고, 혼합진료가 남용되는 한국과 달리 제한적인 혼합진료제도로 새로운 기술은 받아들이되 낮은 의료비 본인부담률과 비급여 비용을 유지하고 있다. 따라서 일본의 세세하고 안정적인 수가체계, 혼합진료를 제한적으로 가능하게 하는 것, 독자적인 노인제도를 통해 본인부담금을 유지시키려 노력하는 정부의 모습을 적극적으로 수용하여 한국의 제도개혁에 좋은 영향을 기대할 수 있을 것이다. ;Japan’s Healthcare Insurance System is known for providing good quality of healthcare service regardless of its low portion of medical expenses relative to GDP. Therefore, the purpose of this study is to suggest reform directions of the Korean Healthcare Insurance System by examining the Japanese healthcare insurance system. The research methodology is based on literature reviewing and analyzation of domestic academic articles and journals published under the topic of ‘Japanese Healthcare Insurance System’. In 1922, Japan first instituted the national Healthcare Insurance, and by 1961, the entire nation was quickly subscribed for the national Healthcare Insurance. Since the Second World War, the national healthcare finances had worsened due to the economic downturn, but the Japanese government had endlessly endeavored to stabilize its healthcare finances. In addition, healthcare insurance for the elderly was created separately due to the aging Japanese society. At the time of elderly healthcare administration, the elderly was able to receive medical services free of charge, but due to the rapid increase in medical expenses, the elderly people were charged at an appropriate level in order to secure future elderly generation expenses. Korea based its first healthcare insurance on the Japanese healthcare insurance so there are many commonalities. The purpose of this study is to compare the healthcare insurance system of two countries in areas such as medical charging systems, rates of out-of-pocket payments and mixed medical service procedures to suggest reform directions of the Korean healthcare insurance system. Currently, Korea is aging rapidly with increasing rates of out-of-pocket payments. In contrast, Japan has a unique medical charging system called DPC and unlike Korea, where mixed medical services are abused, Japanese healthcare system has accepted partial mixed medical services by implementing new technologies but maintaining a low medical expense cost. Therefore, it is expected that Japanese healthcare systems such as these, a stable medical charging system, mixed medical service procedure and the government's efforts to maintain co-payment of the independent elderly healthcare expenses can be a positive impact on the healthcare systems reform of Korea.
