Abstract
Tolvaptan 은 바소프레신 길항제로서 고혈량성 또는 정상혈량성인 저나트륨혈증에서 혈청 나트륨 수치를 높이는 효능으로 허가를 받아 사용되고 있다. 저나트륨혈증의 치료에서는 나트륨 과교정에 의한 삼투압성 탈수초 증후군(Osmotic demyelination syndrome) 발병 예방을 위해 일반 환자에서 24시간 이내 10-12 mEq/L의 나트륨 교정 상한을 두고 치료를 중재해야 한다. 현재까지 연구된 과교정을 예측하는 인자들은 혈청 나트륨 농도의 자동교정(autocorrection) 가능성이 있는 상태를 나타내는 인자들이다. 이러한 요인이 없는 환자들에서도 과교정은 관찰되고 있으나 이를 예측할 만한 위험요인에 대한 연구는 부족한 실정이다. 본 연구는 tolvaptan 투여 후 혈청 나트륨 농도 과교정 발생에 영향을 미치는 위험인자를 예측하고자 하였다. 향후 tolvaptan 투여 전 파악할 수 있는 환자의 정보를 통해 과교정 발생 가능성을 예측하여 적절한 혈청 나트륨 농도 조절 및 tolvaptan 투여 계획을 세우는데 참고 자료로 활용할 수 있도록 하고자 하였다. 2014년 1월부터 2019년 3월까지 가톨릭대학교 서울성모병원에서 고혈량성 또는 정상혈량성 저나트륨혈증에 대해 tolvaptan을 복용한 환자를 대상으로 하여 전자의무기록을 후향적으로 분석하였다. tolvaptan 사용 후 기저 대비 혈청 나트륨 농도가 과교정된 환자군과 과교정 되지 않은 환자군의 나이, 성별, 키, 체중, 기저 질환, 병용 약물 등 예상되는 위험인자를 비교하여 tolvaptan 투여 후 혈청 나트륨 농도 과교정 발생에 영향을 미치는 위험인자를 분석하고자 하였다. 본 연구 결과 tolvatan 투여 전 기저 혈청 나트륨 농도가 낮은 (sNa <126.5 mEq/L) 환자에서 sNa ≥ 126.5 mEq/L인 환자에 비해 혈청 나트륨 농도 과교정(overcorrection criteria: △sNa >10 mEq/L/24hr)에 대해 8.33의 오즈비를 나타냈다(95% confidence interval, 2.17-29.41; p<0.05). Tolvaptan 투여 전 기저 혈청 나트륨 농도가 126.5 mEq/L 미만으로 중등증 이상의 저나트륨환자에서 혈청 나트륨 농도 과교정에 대한 보다 적극적인 관찰을 하고, tolvaptan 투여 용량 및 투여 지속 여부에 대한 중재가 있어야 할 것이다.;Background: Tolvaptan is a vasopressin used with permission to increase serum sodium levels in hypervolemic or euvolemic hyponatremia. In the treatment of hyponatremia, the possibility of developing osmotic demyelination syndrome (ODS) by sodium overcorrection should be considered. In general patients, treatment should be mediated with a sodium correction upper limit of 10-12 mEq/L within 24 hours. Factors predicting overcorrection that have been studied so far are factors that indicate the possibility of autocorrection of serum sodium. Overcorrection has been observed in patients without these factors, but there is a lack of research on predictive risk factors. Objectives: The purpose of this study was to predict the risk factors for the development of overcorrection of sodium after tolvaptan administration. This study intended to predict the possibility of overcorrection by using information of the patient which can be identified before tolvaptan administration and to use it as a reference material for establishing appropriate serum sodium control and tolvaptan dosing plan. Methods: From January 2014 to March 2019, electronic medical records were retrospectively analyzed in patients who took tolvaptan for hypervolemic or euvolemic hyponatremia at theCatholic University Seoul St. Mary's Hospital. Comparing expected factors such as age, gender, height, weight, comobidities, concomitant medication etc. between non-overcorrection group and overcorrection group, the risk factors that affect serum sodium overcorrection after tolvaptan administration were predicted Results : In this study, patients with low baseline serum sodium levels (sNa <126.5 mEq/L) showed adjusted odds ratio of 8.33 for serum sodium overcorrection compared to patients with sNa ≥ 126.5 mEq/L. (95% confidence interval, 2.17-29.41; p<0.05) Conclusions : In case of hyponatremic patients with lower baseline serum sodium level than 126.5 mEq/L, it should be considered to monitor serum sodium level aggressively and intervene in tolvaptan dose and treatment plan.
