요양병원 간호사의 사전연명의료의향서에 대한 지식 및 태도가 암성 통증관리 수행에 미치는 영향

Abstract

This study was conducted to investigate how nurses' knowledge and attitudes toward the Advance Directive for Life-Sustaining Care would have influence on their performance of cancer pain management at long-term care hospitals which participated in the hospice pilot project. This study was designed as a descriptive study through self-report questionnaire and carried out from April 16 to May 31, 2019 by selecting five long-term care hospitals among the hospitals around the country which participated in the hospice pilot project and targeting 137 nurses who worked at those five elderly care hospitals. The collected data was analyzed using SPSS Statistics for Windows, Version 23.0, and descriptive tests of the frequency, frequency percentage, mean, and standard deviation were used to describe the general demographic characteristics of the subjects and their knowledge and attitude toward the Advance Directives for Life-Sustaining Care. The performance of the subjects’ cancer pain management was analyzed using t-test and ANOVA. In addition, The relation between the knowledge and attitude of the Advanced Directives and the performance status of cancer pain management was analyzed by correlation coefficient. Finally, the effect of the subject’s knowledge and attitude about the Advance Directives for Life-Sustaining Care was analyzed using the multiple regression analysis. The results of this study are as follows; First, if we investigate the demographic characteristics of the subjects, their mean age was 40.38 ± 11.48 years old and the proportion of female subjects was higher than that of males. 74 people (54%) had religions and 66 people (48.2%) had bachelor degrees. In terms of their positions, 113 subjects (82.5%) were general nurses. The mean career of the nurses was 11.38±7.94 years, and 67 subjects (48.9%) said that they experienced 21 deaths or more of their patients. 82 subjects (59.9%) said they had experienced hospice care education programs. If we look in the nurses’ pain management experiences, 124 subjects (90.5%) said that they had the experience of pain management, and the mean participation frequency in pain management education programs was estimated at three times. In addition, 120 subjects (87.6%) said they had the experience of taking care of patients, and the mean period of patient care was estimated at 3.78±3.51. In terms of their experiences of taking care of terminal cancer patients, 112 subjects (81.8%) responded that they had such experiences. Second, if we investigate differences in the subjects’ knowledge, attitude and performance of pain management for cancer patients, the variable which showed a statistically significant difference in the subjects’ knowledge about the Advance Directives for Life-Sustaining Care was found to be their positions. In addition, no variable relating to the demographic characteristics of the subjects showed any significant difference in the attitude toward the Advance Directives for Life-Sustaining Care. In contrast, those variables which showed significant differences in the performance of cancer pain management were education level, frequency of experiencing the death of patients, hospice care-related education experience, pain care experience, pain care-related education experience, and experience of taking care of terminal cancer patients; the more experiences they had, the higher their cancer pain management performance. Third, The Nurse`s Knowledge and attitude of Advanced Directives and the correlation of the cancer pain management performance were confirmed. There was a significant positive correlation between the knowledge and the attitude of Advanced Directives. There was a significant positive correlation between the attitude and the performance of cancer pain management toward Advanced Directives. Lastly, we conducted the regression analysis for in-depth analysis on the subjects’ knowledge and attitude toward the Advance Directives for Life-Sustaining Care and their cancer pain management performance. The attitude of the Advanced Directives were found to have statistically significant influence on the cancer pain management performance. ;본 연구는 호스피스 시범사업을 운영중인 요양병원에서의 사전연명의료의향서에 대한 간호사들의 지식 및 태도가 암성 통증관리 수행에 미치는 영향에 대한 연구로 설문지를 통한 자가 보고식 서술적 조사연구로써 2019년 4월 16일부터 2019년 5월 31일까지 전국의 호스피스 시범사업 요양병원 중 연구 참여에 동의한 5개의 요양병원의 간호사 137명을 대상으로 진행하였다. 수집된 자료는 SPSS WIN 23.0프로그램으로 분석하였으며, 대상자의 일반적 특성, 사전의료의향서에 대한 지식과 태도는 빈도, 백분율, 평균, 표준편차로 산출하였고, 대상자의 암성 통증관리 수행 실태는 t-test, ANOVA로 분석하였다. 또한, 대상자의 사전연명의료의향서의 지식 및 태도, 암성 통증관리 수행 실태의 관계는 상관계수로 분석하였고, 대상자의 사전연명의료의향서에 대한 지식 및 태도가 암성 통증관리 수행에 미치는 영향은 다중회귀분석(Multiple regression analysis)으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 일반적 특성을 살펴보면, 대상자의 평균 연령 40.38±11.48이며, 여성 126명(92.0%)으로 여성의 비율이 높았고, 종교를 보유한 사람은 74명(54%), 4년제 학위를 보유한 사람은 66명(48.2%)이었다. 대부분 일반 간호사 113명(82.5%)였으며, 경력은 11.38±7.94년으로 나타났다. 또한, 담당한 환자의 죽음을 경험한 횟수로는 21회 이상이 67명(48.9%)이었으며 임종관련 교육이수 경험으로는 82명(59.9%)이 경험이 있다고 응답하였다. 통증관리 경험의 경우 124명(90.5%)으로 평균 3회 정도 교육을 받은 것으로 나타났으며 암 환자 간호 경험은 120명(87.6%)이 있다고 응답하였고, 평균 3.78±3.51년으로 조사되었다. 말기 암환자 간호경험의 경우, 112명(81.8%)이 경험이 있다고 응답하였다. 2. 대상자 근무지의 조직적 특성을 살펴보면, 총 5개 기관이 참여하였고, 모두 비영리 기관으로써 개인 2시관(40%), 사회복지법인 1기관(20%), 의료법인 2기관(40%)이었으며 평균 10년 8개월 운영하였고, 간호사 1인당 평균 환자 수는 9.48 ± 3.46명이었다. 3. 대상자의 사전연명의료의향서 지식, 태도 및 암성 통증 관리 수행 차이를 살펴보면, 사전연명의료의향서에 대한 지식과 유의한 차이를 보인 변수는 직위였다. 또한, 대상자의 일반적 사항을 살펴보면, 암성 통증관리 수행과 유의한 차이를 보인 변수는 교육수준, 담당한 환자의 죽음을 경험한 횟수, 임종관련 교육이수경험, 통증 관리경험, 통증관리 교육경험, 말기 암 환자 간호경험이었고 교육수준이 높고 경험이 있을수록 상대적으로 높은 암성 통증관리 수행을 보였다. 4. 대상자의 사전연명의료의향서의 지식 및 태도, 암성 통증관리 수행의 상관관계를 확인하였고, 사전연명의료의향서에 대한 지식과 사전연명의료의향서에 대한 태도의 경우 유의한 양의 상관관계를 보였고, 사전연명의료의향서에 대한 태도와 암성 통증관리 수행의 경우, 통계적으로 유의한 의미의 양의 상관관계를 나타냈다. 5. 사전연명의료의향서 지식과 사전연명의료의향서 태도를 다중회귀 분석하여 암성 통증관리 수행에 미치는 영향의 결과는 사전연명의료의향서 태도는 암성 통증관리 수행에 유의한 의미의 결과를 나타내는 것으로 확인되었다.
