Abstract
치과 임플란트 식립 시 치은의 부족으로 치조골 증대술에 어려움이 있는 경우 자가 팽창 조직 확장기를 이용하여 연조직 증대를 얻을 수 있다. 본 연구는 새로운 형태의 스퀘어형 조직 확장기를 이용하여 기존 실린더형 조직 확장기와 최종 팽창 직경의 비교를 통해 새로운 형태의 조직 확장기의 수직 팽창성을 평가하고 두가지 조직 확장기를 이용한 치은 증대 시 조직 확장기 상방 치은 두께 변화를 비교하여 치은의 양적 증대를 정량적으로 평가하고자 하였다. 부가적으로 확장기의 조직 내 삽입 시 문제로 생각될 수 있는 캡슐양 결합조직의 두께를 측정하여 평가하였다. 총 11마리의 성견(생후 12-14개월, 체중 30 kg내외)을 이용하였고 성견 7마리는 하악 협측 골막 하에 좌 우측 구분하여 새로운 형태의 스퀘어형 조직 확장기 14개를 를 삽입하였다(소형 5, 중형 5, 대형 4). 나머지 성견 4마리의 하악 협측 골막 하에 같은 방식으로 기존의 실린더 형 조직 확장기 8개를 삽입하였다(소형 2, 중형 2 대형 4). 조직 확장기 삽입 4 주 후 각각의 조직 확장기의 팽창 직경, 팽창된 조직 확장기 상방 치은 및 정상 치은의 두께, 조직 확장기를 둘러싸는 캡슐양 결합조직의 두께를 조직계측학적으로 측정하였다. 조직 확장기의 종류 및 크기에 따른 차이는 Mann-Whitney검정을 통해 분석하였고 치은 두께의 변화를 확인하기 위해 별도로 일표본 검증을 이용하여 분석하였다. 본 실험을 통해 실린더형 조직 확장기에 비해 스퀘어형 조직 확장기의 최종 팽창 평균 직경이 컸고 대형에서 실린더형에 비해 스퀘어형 조직 확장기의 수직 팽창성이 큰 것을 확인할 수 있었다(p<0.05). 조직 확장기의 종류와 크기에 관계없이 팽창된 조직 확장기 상방 치은 두께 변화가 없어(p<0.05) 절대적인 치은의 양적 증대를 정략적으로 확인하였다. 조직 확장기 삽입 4주 후 모든 경우에서 조직 확장기 주위로 캡슐양 결합조직이 관찰되었으나 염증 반응 및 특이소견 없이 캡슐양 결합조직의 두께는 모두 1.5mm이내로 계측되어 확장기 주변 캡슐양 결합조직은 향 후 수반되는 골이식에 영향을 주지 않을 것으로 사료된다.;Due to lack of gingiva during the dental implant placement, soft tissue can be augmented by using a self-inflating tissue expander. This study was aimed to evaluate vertical inflatability of the new type of tissue expander, a square-type tissue expander, by comparing the final inflation diameter with that of the existing cylinder-type tissue expander. Furthermore, it is designed to make a quantitative assessment of the augmentation through checking that there is no change in the gingiva thickness, because the absolute quantitative augmentation of the gingiva can be confirmed if there is no change in the thickness of upper gingiva of the tissue expander during the gingiva augmentation using the two types of tissue expanders. Additionally, the thickness of encapsulated fibrous tissue, which could be a problem during the placement of expander into the tissue, was measured and evaluated. A total of 11 adult dogs (at least 12-14 months old, weighing around 30 kg) were used in this study. 14 new square-type tissue expanders were placed onto the left and right buccal periosteum of mandibles of 7 dogs (5 small breeds, 5 medium breeds, 4 large breeds), while 8 conventional cylinder-type tissue expanders were placed onto the left and right buccal periosteum of mandibles of the remaining 4 dogs (2 small breeds, 2 medium breeds, 4 large breeds). After four weeks of tissue expander placement, each tissue expander was measured histomorphometrically in terms of the inflation diameter of each tissue expander, the thickness of the upper gingiva of the inflated tissue expander and the normal gingiva, and the thickness of the encapsulated fibrous tissue surrounding the tissue expander. Mann-Whitney test was conducted to analyze the differences in types and sizes of the tissue expanders, and one sample t-test was separately conducted to analyze the changes in gingiva thickness. In this study, we found that the average final inflation diameter was larger in the square-type tissue expander than the cylinder-type tissue expander, and the vertical inflatability of the square-type tissue expander was greater in the large breeds than that of the cylinder-type tissue expander (p<0.05). The absolute gingival augmentation was quantitatively confirmed as there was no change in the thickness of the upper part gingiva of the inflated tissue expander (p<0.05), regardless of the type and the size of the tissue expander. Encapsulated connective tissue was observed around the tissue expander in all cases after four weeks of tissue expander placement. However, because there was no inflammatory reaction or any specific findings, and the thickness of all encapsulated fibrous tissue was measured to be less than 1.5mm, it was considered that the amount of the encapsulated connective tissue around the expander would not affect the following bone grafting.
