Abstract
장루 조성술을 시행 받은 환자는 장루 관리로 인해 삶의 질 저하를 경험하게 되고 사회 심리적 적응에 어려움을 겪게 되므로 효율적인 장루관리 교육이 이루어져야 한다. 수술 전 장루관리 교육을 시행 시 장루관리에 적응할 수 있는 시간이 확보되어 수행능력 및 능숙도 향상, 불안과 우울 감소 효과가 있다. 더불어 동영상을 이용한 장루관리 교육을 시행하게 되면 종전의 구두로 설명하는 교육방법에 비해 비용 효율적이라 보고되고 있다. 본 연구는 결장암, 직장암 수술 시 장루 조성술을 시행할 환자들을 대상으로 수술 전 장루관리 교육을 실시한 후 자가간호지식, 불안과 우울, 자가간호수행, 재원일수, 장루합병증에 미치는 효과를 규명하고자 시도된 무작위 대조군 연구이다. 연구 대상자는 A 종합병원 대장항문외과 병동에 입원한 결장암, 직장암 수술 시 장루조성이 예정된 환자로 실험군 21명, 대조군 20명이었으며 자료수집은 2018년 11월 23일부터 2019년 5월 24일까지 진행되었다. 수술 후 장루교육 내용을 기반으로 문헌검색 및 전문가 검증을 통해 수술 전 장루관리 교육내용을 개발하였고 교육은 수술 전일 장루전문간호사에 의해 45분간, 1회 시행되었으며 내용은 동영상과 구두교육으로 구성하였다. 사전조사는 실험군과 대조군의 일반적 특성, 자가간호지식, 불안과 우울에 대해 설문 조사를 시행하였다. 대조군에게는 장루 전문간호사에 의해 수술 전일 장루 위치 표식(stoma site marking)만 시행되었다. 수술 후에는 실험군, 대조군 모두에게 장루 관리 집단교육과 개별교육이 시행되었고 서면화된 장루 관리 책자가 제공되었다. 사후조사는 수술 3주 경과시점인 퇴원 2주 후 첫 외래방문일에 실험군과 대조군 모두에게 일반적 특성 질문지를 제외한 자가간호지식, 불안과 우울에 대한 설문조사와 자가간호수행 능력을 관찰한 후 측정하였다. 첫 외래 방문 시 장루전문간호사가 장루 관련 합병증을 사정 후 체크리스트에 표기하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여 분석하였다. 대상자의 일반적 특성 및 질환 관련 특성은 백분율 및 평균과 표준편차를 이용한 기술통계로 분석하였고 실험군과 대조군의 동질성 검정은 두 집단 간 유의한 차이를 확인하기 위해 Chi-square test, Fisher's exact test, Mann-Whitney U test를 이용하여 검증하였다. 실험군과 대조군의 사전과 사후의 자가간호지식, 불안과 우울 변화를 알아보기 위해 Wilcoxon signed ranks test를 이용하였고, 실험군과 대조군의 결과변수의 실험 전・후 변화 차이는 Mann-Whitney U test로 검증하였다. 본 연구의 결과는 다음과 같다. 첫째, 수술 전 장루관리 교육을 시행 후 실험군과 대조군의 자가간호지식 점수 변화 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(Z=-5.599, p <.001)‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 자가간호지식 정도가 높을 것이다.’라는 가설은 지지되었다. 둘째, 실험군과 대조군의 불안 점수 변화 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(Z=-2.247, p= .025).‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 불안 점수가 낮을 것이다.’라는 가설은 지지되었다. 셋째, 실험군과 대조군의 우울 점수 변화 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(Z=-2.463, p= .014).‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 우울 점수가 낮을 것이다.’라는 가설은 지지되었다. 넷째, 실험군과 대조군의 자가간호수행의 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(Z=-5.543, p <.001).‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 자가간호수행 정도가 높을 것이다.’라는 가설은 지지되었다. 다섯째, 실험군과 대조군의 재원일수의 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(T=-2.980, p=.005)‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 재원일수가 감소할 것이다.’라는 가설은 지지되었다. 여섯째, 실험군과 대조군의 장루합병증 발생의 차이는 집단 간 통계적으로 유의한 차이가 있는 것으로 나타나(T=-4.980, p<.001)‘수술 전 장루관리 교육을 적용한 실험군은 대조군에 비해 장루 합병증 발생이 감소할 것이다.’라는 가설은 지지되었다. 본 연구는 수술 전 장루관리 교육을 이용하여 장루보유자의 자가간호지식, 자가간호수행 점수가 향상되고, 불안, 우울, 재원일수, 장루합병증이 감소하는 긍정적 효과를 확인하였다. 따라서 동영상을 이용한 수술 전 장루관리 교육을 장루조성이 예정된 환자의 간호학적 중재 요법으로 이용할 수 있는 근거를 마련하였다는 데 의의가 있다.;This study is a randomized controlled trial aiming to investigate the effects of preoperative stoma education on self-care knowledge, anxiety and depression, and self-care practice in patients planned to undergo ostomy during colon cancer or rectal cancer surgery. Inpatients of the colon and rectal surgery ward in A medical center in Seoul who are scheduled to undergo ostomy during colon cancer or rectal cancer surgery were enrolled in this study, with 21 in the intervention group and 20 in the control group. Data were collected from November 23, 2018 to May 24, 2019. Preoperative education about stoma care consisted of video and verbal education. The intervention group was shown a video called 'stoma care for a healthy daily life' using a tablet PC followed by 30 minutes of verbal education and five minutes of question and answer session. The education lasted for 45 minutes, during which the stoma site was marked and the patients were given an opportunity to practice using stoma care products. The control group did not receive stoma care education before surgery and was only marked for the stoma site by a stoma nurse specialist. After surgery, both groups received group and individual education about stoma care and were given a stoma care booklet. The post-test was performed three weeks after surgery, on the day of first outpatient visit two weeks after discharge. Both groups were given a survey including self-care knowledge and anxiety and depression, and self-care practice was observed and measured. On the first outpatient visit, an stoma nurse specialist evaluated patients for stoma-related complications and marked them on a checklist. The collected data were analyzed using the SPSS 22.0 software. Participants’ general characteristics and disease-related characteristics wereanalyzed with descriptive statistics using percentages, mean, and standard deviation, and differences between the intervention and control groups were analyzed with Chi-square test, Fisher's exact test, independent t test, and Mann-Whitney U test. Changes in self-care knowledge, anxiety, and depression after education in both the intervention and control groups were analyzed with the Wilcoxon signed ranks test, and the differences in the post-intervention changes between the intervention and control groups were analyzed with the Mann-Whitney U test. The results were as follows. First, there were significant differences between the intervention and control groups in the change of self-care knowledge score after preoperative stoma care education (Z=-5.599, p<.001). Thus, the hypothesis 'the intervention group that receive preoperative stoma care education will have greater self-care knowledge compared to the control group' was supported. Second, there were significant differences between the intervention and control groups in the change of anxiety and depression scores (Z=-2.599, p=.009). Thus, 'the intervention group that receive preoperative stoma care education will have lower anxiety and depression scores compared to the control group' was supported. Third, there were significant differences between the intervention and control groups in the change of self-care practice (Z=-5.543, p.<001). Thus, the hypothesis 'the intervention group that receive preoperative stoma care education will show greater self-care practice compared to the control group' was supported. According to our findings, preoperative stoma education lowered stoma-related complications, suggesting that preoperative education helps effective adjustment of stoma patients. Therefore, this study is meaningful in that it presented evidence supporting the use of preoperative stoma education as a nursing intervention for patients scheduled for ostomy.
