Abstract
반복 시계열 데이터는 임상 현장에서 얻을 수 있는 데이터로 일회 측정 데이터에 비해 정보의 양이 많으므로 질병 예측능을 향상시킬 가능성이 있으나, 이에 대한 명확한 분석 방법은 잘 알려져 있지 않다. 본 연구는 반복 시계열 데이터를 인공지능 기반 딥러닝 방법을 이용하여 분석하여 개발한 심혈관질환 예측모델이 심혈관질환의 고위험군을 효과적으로 분류할 수 있는지에 대해 확인하고, 이를 고식적인 콕스 통계분석 모델과 비교하고자 하였다. NHIS-HEALS 코호트를 이용하여 콕스 모델과 딥러닝 모델을 개발하였고, 개발된 모델을 내부 검증 코호트와 두 개의 새로운 코호트 (NHIS-NSC 코호트, Rotterdam study 코호트)를 이용하여 외부 검증을 시행하였다. 기저에 심혈관질환에 없는 환자 및 2회 이상 검진 데이터가 있는 환자들만을 선택하여 분석을 진행 하였으며, NHIS-HEALS 코호트에서 412,030명, NHIS-NSC 코호트에서 178,875명, Rotterdam study 코호트에서 4,296명을 이용하여 분석을 시행하였다. 내부 검증 결과, 딥러닝을 이용한 심혈관질환 예측 모델의 C-statistics(95% 신뢰구간)는 남자의 경우 0.896 (0.886-0.907) 여자의 경우 0.921 (0.908-0.934)로 콕스 모델과 비교하여 분류능의 향상이 확인되었다. NHIS-NSC를 이용한 외부 검증에서도 딥러닝을 이용한 모델은 C-statistics(95% 신뢰구간)가 남성의 경우 0.868 (0.860-0.880), 여성의 경우 0.889 (0.880-0.910)로 콕스 모델에 비해 분류능이 향상되었다. Rotterdam study를 이용한 유럽인에서의 외부 검증 결과에서도 딥러닝 모델은 C-statistics(95% 신뢰구간)가 남성의 경우 0.860 (0.824-0.897), 여성의 경우 0.867 (0.838-0.903)으로 콕스 모델에 비해 분류능이 향상됨을 보여주었다. 결론적으로, 반복 시계열 데이터를 이용한 데이터 분석을 할 때, 머신 러닝 기반 딥러닝 알고리즘을 이용할 경우 심혈관질환 발생에 대한 유용한 예측 모델을 개발할 수 있었으며, 이는 기존에 사용되는 고식적인 콕스 모델에만 기반한 분석 방법에 비해 우월한 심혈관질환 발생 예측능을 보였다.;Risk classification by integrating serial data inputs is clinically relevant and has the potential to improve disease prediction accuracy. We aim to explore the additional discriminative accuracy of a deep learning algorithm with recurrent neural network using repeated-measures data for identifying people who are at high risk for future development of cardiovascular disease (CVD), compared to Cox hazard regression model. Two CVD prediction models were developed from repeated-measures NHIS-HEALS dataset: a Cox hazard regression model and a deep learning model. Performance of each prediction model was assessed in the internal and two external validation cohorts in Koreans (NHIS-NSC) and in Europeans (Rotterdam Study). A total of 412,030 adults in the NHIS-HEALS; 178,875 adults in the NHIS-NSC; and the 4,296 adults in Rotterdam Study were included. Mean ages was 52 years (46% women) and there were 25,777 events (6.3%) in NHIS-HEALS during the 10-year follow-up. In internal validation with NHIS-HEALS, the deep learning approach demonstrated a C-statistics (95% confidential interval [CI]) of 0.896 (0.886 to 0.907) in men and 0.921 (0.908 to 0.934) in women and improved reclassification compared with Cox regression (net reclassification index [NRI], 24.8% in men, 29.0% in women). In external validation with NHIS-NSC, deep learning demonstrated a C-statistics (95% CI) of 0.868 (0.86-0.88) in men and 0.889 (0.88-0.91) in women, and improved reclassification compared with Cox regression (NRI, 24.9% in men, 26.2% in women). In external validation applied to the Rotterdam Study, deep learning demonstrated a C-statistics (95% CI) of 0.860 (0.824-0.897) in men and 0.867 (0.830-0.903) in women, and improved reclassification compared with Cox regression (NRI, 36.9% in men, 31.8% in women). In conclusion, a deep learning algorithm analysis with recurrent neural network resulted in ethnically-generalizable predictive models for CVD outcomes that exhibited greater discriminative accuracy than Cox model approaches.
