EQ-5D 도구와 HINT-8 도구 간의 Mapping을 이용한 전환식 도출 연구

Abstract

HINT-8은 한국인의 특성에 맞는 건강관련 삶의 질을 보다 정확하게 측정하는 것을 목표로 개발되었다. 하지만 도구가 최근에 개발되었고, 후속연구의 부족으로 활용도가 미미한 상태이다. 이에, 본 연구에서는 EQ-5D와 HINT-8 도구를 통해 얻어진 각각의 삶의 질 효용 값을 이용하여 EQ-5D 지수 산출을 위한 전환식을 도출하고, 이를 통해 HINT-8의 활용 가능성을 알아보고자 하였다. 연구를 위해 우리나라 일반인 220명을 대상으로 EQ-5D-3L, EQ-5D-5L, EQ-VAS, HINT-8 도구를 이용하여 설문을 실시하였으며 설문 내용은 SPSS 25.0을 이용하여 분석하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 건강관련 삶의 질 지수의 평균은 EQ-5D-3L에서 0.94, EQ-5D-5L에서 0.91, EQ-VAS에서 0.84, HINT-8에서 0.84로 나타났다. 천장효과는 EQ-5D-3L에서 51.4%, EQ-5D-5L에서 47.3%, HINT-8에서 1.8%이었다. 둘째, 대상자의 일반적 특성에 따른 삶의 질 지수를 살펴본 결과 성별과 연령은 삶의 질에 영향을 주지 않는 것으로 나타났다. 주관적 건강상태는 삶의 질 지수와 비례하였으며, 차이도 통계적으로 유의하였다. 셋째, EQ-5D 도구와 HINT-8 도구 간의 관련성을 살펴본 결과 EQ-5D-3L과 EQ-5D-5L간의 상관관계가 0.82로 가장 높았으며 EQ-5D-3L과 HINT-8이 0.69, EQ-5D-5L과 HINT-8이 0.78로 EQ-5D와 HINT-8 도구 간의 관련성이 높았다. EQ-5D 도구와 HINT-8의 문항 간 관련성을 살펴보면 서로 유사한 통증과 우울 문항에서 각각 상관계수가 높게 나타났다. 넷째, EQ-5D 도구와 HINT-8 도구 간의 전환식을 도출한 결과 모델의 설명력은 47~70%로 모델을 설명하는데 적합하였다. 회귀계수의 모순이 발생한 경우 2개의 수준을 통합하였다. HINT-8 설문값을 EQ-5D-3L 지수로 전환하였을 때 산출될 수 있는 범위는 0.474~1점이었으며, EQ-5D-5L 지수로 전환될 수 있는 범위는 0.259~1점이었다. 위의 결과를 종합한 본 연구의 결론 및 의의는 다음과 같다. 첫째, HINT-8의 경우 EQ-5D를 이용할 때보다 천장효과가 크게 감소함을 알 수 있었다. 이는 HINT-8을 이용한 설문이 우리나라 일반인의 건강상태를 더 정확 하게 반영함을 시사한다. 둘째, EQ-5D-3L, EQ-5D-5L과 EQ-VAS 간의 상관관계보다 HINT-8과의 상관관계가 더 크게 나타났는데, 이는 EQ-5D 도구와 HINT-8 도구 간의 관련성이 높다는 것을 의미한다. 셋째, 본 연구는 HINT-8과 EQ-5D 간의 mapping을 수행한 첫 연구이기 때문에 후속 연구가 나올 때까지 본 연구에서 도출된 mapping식을 사용할 수 있을 것으로 생각한다. 또한 추후 좀 더 큰 규모의 인원을 대상으로 연구를 진행할 때 본 연구가 참고자료로 활용될 수 있을 것이다. ;HINT-8 was developed with the aim of measuring more accurately the health-related quality of life that fits the characteristics of Koreans. However, instrument have recently been developed and utilization is limited due to a lack of follow-up research. Thus, the objective of this study is to derive a conversion formula using mapping between EQ-5D and HINT-8 instruments and to see the usability of HINT-8. 220 people were surveyed using EQ-5D-3L, EQ-5D-5L, EQ-VAS, and HINT-8 instruments, and the survey results were analyzed using SPSS version 25.0. The main results of this study are as follows: First, average of health utility scores was 0.94 in EQ-5D-3L, 0.91 in EQ-5D-5L, 0.84 in EQ-VAS, and 0.84 in HINT-8. The ceiling effect was 51.4% in EQ-5D-3L, 47.3% in EQ-5D-5L, and 1.8% in HINT-8. Second, based on the health utility scores according to people general characteristics, it was found that gender and age did not affect the quality of life. Subjective health conditions were in proportion to the health utility scores, and differences were also statistically significant. Third, the correlation between EQ-5D-3L and EQ-5D-5L utility scores(r=0.82) was the highest, also between EQ-5D-3L and HINT-8(r=0.69), and between EQ-5D-5L and HINT-8(r=0.78) also had high correlation. The association of questionnaires between the EQ-5D and HINT-8, it was showed that pain and depression questionnaires had high correlation coefficients, respectively. Fourth, the results on deriving a conversion formula between EQ-5D and HINT-8 instruments, was 47~70% and it seems to reasonable to explain the conversion formula model. If the regression coefficients were inconsistency, two levels were aggregated. When converting the HINT-8 to the EQ-5D-3L score, the range that could be calculated was 0.474 to 1, and to the EQ-5D-5L score was 0.259 to 1. The conclusions drawn from this study are as follows: First, for HINT-8 it was found that the ceiling effect was significantly reduced compared to using EQ-5D. This suggests that the survey using HINT-8 reflects the health states of the general people more accurately. Second, the correlation with HINT-8 was greater than that between EQ-5D-3L, EQ-5D-5L and EQ-VAS. This means that there is a high correlation between the EQ-5D and the HINT-8 instruments. Third, because this is the first study to perform mapping between HINT-8 and EQ-5D, the conversion formula derived from this study may be used until further studies have been developed. It can also be used as a reference when conducting research on larger scale in the future.
