미술치료 전공 석사생의 정신과 폐쇄병동 미술치료 임상실습 경험에 대한 예술기반연구

Abstract

본 연구는 미술치료 석사생의 폐쇄병동 미술치료 임상실습 경험과 그 의미를 들여다보는 것이다. 오늘날 국내에서 미술치료에 대한 관심의 급증은 미술치료사를 양성하기 위한 교육기관의 입지와 교육과정의 체계적인 프로그램의 증가로 나타나고 있다. 즉, 미술치료사는 예술을 사용하는 비치료적 예술가와 구분 지을 필요가 있고(임성윤, 김수정, 2018) 이러한 과정을 위해 미술치료학 전공의 석사생은 강도높은 훈련과 이론적 지식을 기반으로 한다. 미술치료의 정신건강 분야에 대한 임상적 접근의 증가는 미술치료의 전문성을 충분히 갖추고 치료적 목적으로 실시하기 위한 것으로 볼 수 있다. 이러한 훈련과정인 석사생의 실습과 정신과 임상적 접근의 미술치료의 중요성은 전문적인 미술치료사의 역할 및 과정의 중요성을 보여주고 있다. 그에 비해, 국내 미술치료 논문은 아직까지 석사생의 정신과 임상실습에 대한 충분한 연구가 이루어지고 있지 않고 있다. 이는 미술치료 석사생에게 전문성을 갖춘 미술치료사로 성장하기 위한 실제적인 경험과 임상 현장의 실제의 재현을 제공하는데 한계가 있다고 보았다. 이러한 임상실습 현장의 이해가 석사생에게 어떠한 경험으로 다가왔는가에 대한 심층적인 연구가 필요하다. 지금까지 국내에 보고된 정신과 미술치료 임상실습에 관련한 선행연구는 석사생들의 경험과 그 의미를 들여다보는데 미흡하며, 이들에 대한 경험을 인터뷰로 재경험 하는데 그 한계가 있다고 보았다. 특히, 정신과 폐쇄병동 임상실습의 경우 각 병동에 대한 차이가 있음에도 불구하고 현실적인 환경의 접근성으로 인해 폐쇄병동에 대한 경험의 이야기가 많지 않다. 특수한 환경의 폐쇄병동 임상실습 경험의 과정을 바라볼 수 있는 다각도적 관점과 충분한 정보의 이해를 위한 질적자료가 필요하다. 본 연구의 목적은 정신과 폐쇄병동 임상실습에 대한 석사생들의 경험은 무엇이고, 그들이 체험한 과정의 의미와 그 본질을 이해하는 것이다. 이를 통해 미술치료 석사생에게 정신과 폐쇄병동 미술치료에 대한 기초자료를 제공하고자 한다. 본 연구의 문제는 ‘예술작업의 재현을 통해 드러나는 미술치료 석사생의 정신과 폐쇄병동 임상실습 경험은 무엇인가?’이다. 본 연구는 경험의 재현을 통한 생생한 전달을 위해 예술기반연구(Art-based research)방법을 적용하고 이를 기반으로 그들의 임상실습 경험의 본질을 보고자 하였다. 본 연구자는 실습생으로서 폐쇄병동 미술치료 임상실습을 체험하고, 지속적인 예술작업을 통해 그 체험을 바라보았다. 이를 통해 석사생의 폐쇄병동 임상실습 경험은 환경과 내담자의 특성과 인간의 내적, 외적 경험 자체에 대한 탐구라고 보았으며, 이러한 개인의 경험은 언어로 명료화 할 수 없는 주제라고 생각하였다. 이와 같은 연구자의 예술적 탐구 과정을 통해 본 연구의 연구 문제를 정의할 수 있었다. 따라서 미술치료 석사생들의 정신과 폐쇄병동 임상실습 체험에 대한 그들의 경험을 언어 그 이상의 자유로운 예술적 표현을 통해 그 체험을 보고자 하였다. 본 연구의 연구 참여자는 폐쇄병동 미술치료 임상실습을 경험한 기간이 최소 6개월 이상인 국내 미술치료 전공 석사생 5명으로 선정하였다. 자료수집 기간은 2018년 7월 11일부터 10월 22일까지 4개월 동안 진행되었으며, 연구의 자료는 주제에 따른 미술작품과 이에 따른 심층 면담 자료를 중심으로 본 연구자의 관찰 일지, 녹음 자료, 인터뷰 마다 작업한 그림 작품이 수집 및 활용되었다. 연구의 모든 과정은 연구자의 예술작업의 과정을 통해 구성되었고 각 연구 참여자를 대상으로 4회씩 연구자 작품에서 도출된 네 개의 카테고리에 대한 미술작업과 심층 면담을 통해 얻은 자료를 기반으로 살펴보았다. 이들은 총 20개의 미술작품을 남겼으며 각 회기당 90분동안 심층 면담과 함께 실시하였다. 이렇게 수집된 자료는 ABR의 절차에 따라 다시 연구자의 예술 작업을 통해 분석 및 통합하였다. 연구 결과는 석사생의 폐쇄병동 미술치료 실습 경험의 묘사를 토대로 종합적으로 드러난 의미의 요소들의 테마를 네 가지로 요약할 수 있다. 첫째, 석사생은 폐쇄병동 미술치료 임상실습에 대한 경험을 예술로서 표현하며 그 안에 담겨있는 의미를 새롭게 발견하였다. 경험에 대한 다양한 감정과 자신에게 미치는 영향이 무엇인지, 공통적으로 특별한 실습현장의 경험으로 느끼는 이유가 무엇인지 발견하게 되었다. 둘째, 석사생은 폐쇄병동 환경과 환자들의 낯선 환경 속에 적응하며 개인이 지니고 있던 고정관념의 변화와 치료사로서 편견을 배제하고 다각적 관점의 폭을 넓히며 스스로 변화되고 있음을 경험하게 되었다. 셋째, 폐쇄병동 임상실습 경험 자체가 석사생의 성장과정이라고 경험하였다. 넷째, 치료에 임하는 치료사적 태도의 배움과 깨달음의 과정 속에서 치료사로 성장하고 있음을 경험하게 된다. 이는 점차 미술치료사로서 책임감과 향후 나아가야할 방향의 정체성을 찾아가는 계기가 되었으며 석사생의 성장을 통해 경험이 유의미하다고 느껴지는 것이 무엇인지 그 의미의 본질을 발견하게 되었다. 본 논문은 국내 미술치료 석사생이 실제로 경험할 수 있는 한정적인 정신과 폐쇄병동 임상실습에 대한 간접경험을 바탕으로 그 경험의 선 이해와 학습을 통해 실제 임상실습 현장에 나가는데 도움을 줄 수 있는 기초 자료의 의미를 갖는다. 또한, 본 논문에 사용된 미술작품은 경험에 대한 공감과 개인경험의 기술을 바라보는데 이해의 폭이 더 넓어질 수 있을 것으로 본다. 따라서, 본 논문은 미술치료가 국내 임상 현장에서 더욱 전문성 있는 분야로 성장하며 치료사에게 기대되는 역할수행을 위해 준비하는 석사생에게 훈련을 위한 참고 자료가 되는데 이바지할 것으로 기대한다. ;The purpose of this study is to examine the experience and its meaning of art therapy clinical training in closed ward by master's level students of art therapy. Today, the surge of interest in art therapy in Korea is showing the increasing number of the location of educational institutions and systematic programs of curriculum to train art therapists. In other words, art therapists need to be distinguished from non-therapeutic artists using art (Lim & Kim, 2018). For this process, the master's level students majored in art therapy are based on intensive training and theoretical knowledge. The increase in the clinical approach to the field of mental health of art therapy can be seen that is fully equipped for the therapeutic purpose with the sufficient expertise in art therapy. This training course, which is the practice of master's level students and the importance of art therapy in psychiatric clinical approaches, shows the importance of professional art therapist's role and process. In contrast, the Korean art therapy thesis has not yet been sufficiently studied about the psychiatric clinical practice of the master's level students. This regards that there are limitation to the master's level students of art therapy to provide the practical experience and actual reappearance of clinical scene to grow as a professional art therapist. In-depth study is needed that understand how these understandings of the field of clinical practice were recognized to the master's level students as which experiences. Until now, previous studies related to clinical practice of psychiatric art therapy reported in Korea were insufficient to examine the experiences and meanings of the master's level students, and it has been considered that there is a limit to re-experiencing experiences of these students through interviews. In particular, in the clinical practice of a psychiatric closed ward, despite the differences in each ward, there are not many stories of experience with a closed ward due to the realistic accessibility of the environment. The multifaceted viewpoint which can see the process of the clinical practice experience in a closed ward in special environment and qualitative data for understanding of the sufficient Information are needed. The purpose of this study was to understand the practicum experiences of master's level students about clinic practice in psychiatric closed ward, and the meaning and nature of the process they experienced. Through this, this researcher would like to provide the basic data on the art therapy of the psychiatric closed ward to the art therapy master's level students. The research question was 'What is the experience of clinical practice in the closed ward by art therapy master's level students revealed through the reappearance of the art work?'. This study, for the vivid transmission through the reappearance of experiences, applies the art-based research methods and tries to see the essence of their clinical practice experiences. The clinical practice experiences in the closed ward by the master's level students which were treated in this study were considered as the investigation of the characteristics of the environment and the client and the inner and outer experiences of the human being themselves. These individual experiences seem to be a theme that cannot be clarified in language because these seem to have more than emotions, thoughts, and feelings. Therefore, this researcher would like to see the experiences of art therapy master's level students in their experience of clinical practice in the psychiatric closed ward through the free artistic expression beyond language. This researcher experienced the art therapy clinic room of closed ward, and tried to see what the experience was through continuous art works. The process of artistic inquiry was able to define the research problem of this study, and it was able to reveal the meaning and essence of the experience leading to the process, result and conclusion of the research method for the study participants.. The participants of this study were selected as 5 master's level students majored in art therapy who have at least 6 months of experiences the art therapy clinical practice in the closed ward. The data collection period was 4 months from July 11, 2018 to October 22, 2018. The data of the research were collected and used from the researcher's observation diary, recording materials, interviews based on the art works by theme and resulting in-depth interview data. The whole process of the study was composed by the process of the researcher's art works. And it was studied per four times from each participants based on the data obtained through in-depth interviews and art work on the four categories derived from the artworks of researcher. They left a total of 20 art works and conducted in-depth interviews for 90 minutes per session. The results of the study can be summarized into four themes of the elements of meaning that are revealed comprehensively based on the description of art therapy practice experience in the closed ward by master's level students. First, the master's level students expressed their art therapy clinical practice experiences in closed ward as the art, and newly discovered the meaning contained in it. These could be found out that the various emotions of experience and what are their effects on you?, and in common, why they felt through the special experience from the practice field. Second, the master's level students were adapted to the closed ward environment and the unfamiliar environment of the patients. And they have experienced that they are changing itself by excluding the change of stereotypes they had and the prejudice as the therapist, and widening the range of the various viewpoints. Third, they experienced that the experiences of closed ward clinical practice itself were the growth process of the master's level students. Fourth, in the process of learning and enlightenment of the therapeutic attitude, they could experience that they are growing as a therapist. This gradually led to the sense of responsibility as an art therapist and the identity of the future direction and the essence of what it means to feel the experience through the growth of the master's level students were discovered. In this paper, based on the indirect experiences of limited psychiatric closed ward clinical training which practiced by master's level students majored in art therapy in Korea, it has the meaning of basic data that can be helped to go to actual clinical practice field. In addition, the art works used in this paper can broaden the understanding to see the empathy of experience and skill of personal experience. Therefore, this paper hopes that art therapy will become a more specialized part in the clinical field in Korea, and to the master's level students who preparing to perform a role as the therapist it would be a reference for training.
