- Abstract
- 연구배경: 체중 감소와 영양 불량은 암환자에게 자주 나타나는 증상이며, 질병의 진행뿐만 아니라 수술, 약물 치료, 방사선 치료 등 치료과정에도 심화될 수 있다. 이러한 영양 불량은 삶의 질과 활동 수준을 저하시키며 치료 관련 부작용 증가, 치료에 대한 반응 저하, 생존율 저하를 야기한다. 하지만 당뇨환자의 경우 혈당 조절에 실패할 것을 우려하여 담당 의사의 판단 하에 영양 공급을 제한하는 임상례가 있다. 이 경우 적절한 영양 공급이 이루어지지 않아 치료 예후 및 삶의 질에 부정적인 영향을 주는 것을 개선하고자 본 연구를 계획하였다.
연구목적: 본 연구는 암환자 중 영양집중지원팀(Nutrition support Team, NST)에 의한 영양집중지원을 받은 당뇨환자를 대상으로 에너지(칼로리, 열량) 목표량을 공급한 군(80%이상, 120%미만)과 공급하지 않은 군(80%미만 또는 120%이상)의 목표 혈당 유지를 비교하고자 하였다.
연구방법: 2017년 9월 1일부터 2018년 8월 31일까지 본원 NST에서 영양집중지원을 받은 암환자를 대상으로 전자의무기록을 후향적으로 분석하였다. 대상이 된 암환자 중 당뇨 진단을 받은 환자만을 분석에 포함하였으며, 혈당 모니터링 기록이 없는 환자, 영양 공급량이 명시된 경과기록이 없는 환자는 제외하였다. 포함된 환자를 에너지 목표량을 공급한 군(80%이상, 120%미만)과 공급하지 않은 군(80%미만 또는 120%이상)으로 나누고 영양집중지원기간 동안의 목표 혈당 유지를 비교 분석하였다.
연구결과: 연구기간동안 영양집중지원 받은 임상례 690건 중, 암 진단을 받은 당뇨 환자례는 총 113건이었다. 이중 혈당 기록이 누락된 건과 영양 공급량이 명시된 경과기록이 누락된 건이 26건, 환자 중복 10건을 제외하고 총 77명이 포함되었다. 단 영양집중지원 전 초기 혈당이 80 mg/dL미만인 3명은 저혈당 기준 80 mg/dL미만 분석에는 포함하지 않았다. 연구 결과 Sulfonylurea(SU)계 혈당 강하제를 투약 받은 사람은 그렇지 않은 사람에 비해 저혈당 발생율이 높았으며(95% CI: (70 mg/dL미만)1.2-219.0, (80 mg/dL미만)1.3-26.0), 초기 혈당이 200 mg/dL이상인 군(95% CI: 1.3-28.7)과 에너지 목표량의 120%이상을 공급한 군(95% CI: 1.3-333.2)에서 고혈당이 더 많이 발생했다.
결론: SU를 투여하는 환자는 정기적인 혈당 모니터링을 하면서 투여 용량을 조절할 필요가 있다. 초기 혈당이 200 mg/dL이상인 암환자의 경우 영양집중지원 전 혈당 조절이 선행되어야 하며, 영양집중지원을 받는 환자는 에너지 목표량의 120%이상을 공급하지 않도록 주의가 필요하다. ;Background:
Weight loss and malnutrition are common symptoms in cancer patients and can be deepened not only in the progress of the disease but also in the treatment process such as surgery, drug treatment, and radiation therapy. These malnutritions reduce the quality of life and activity level, resulting in increased treatment-related side effects, decreased response to treatment, and decreased survival rate. However, there is a clinical case in which diabetic patients are restricted from supplying nutrition under the judgment of their physicians because they are concerned that blood sugar control will fail. In this case, we planned this study to improve the negative effects on the prognosis and quality of life because proper nutrition was not provided.
Method: From September 1, 2017 to August 31, 2018, we analyzed the electronic medical records backwardly for cancer patients who received nutritional support from NST. Only the patients diagnosed with diabetes were included in the analysis, except those who did not have blood glucose monitoring records, and those who did not have a history of nutritional supply. The subjects were divided into the group that supplied the energy target (more than 80% and less than 120%) and the group that did not supply (less than 80% or more than 120%).
Results: Of the 690 clinical cases that were nutritionally supported from September 2017 to August 2018, 113 cases of diabetic patients were diagnosed with cancer. A total of 77 people were included, excluding 26 cases of missing blood glucose records and cases of missing nutritional records and 10 cases of patient duplication. Studies have shown that people who take a sulfonylurea (SU) have 16.2((less than 70 mg/dL), 95% CI: 1.2-219.0) and 5.8((less than 80 mg/dL), 95% CI: 1.3-26.0) times more hypoglycemia than those who do not take it. And people with initial blood glucose above 200 mg/dL had a hyperglycemia 6.1 times more than those who did not.(95% CI: 1.3-28.7) The group that supplied more than 120% of the energy target had 20.6 times more hyperglycemia.(95% CI: 1.3-333.2)
Conclusion: Patients receiving SU need to adjust their doses while monitoring regular blood sugar. Cancer patients with initial blood sugar of 200 mg/dL or more should be pre-regulated for blood glucose control before nutrition support, and patients receiving nutritional support should be careful not to supply more than 120% of their energy target. To this end, NST requires careful evaluation of the patient's underlying disease and the amount of medication and nutrition.
