말기암환자를 돌보는 간호사의 '민감한 간호' 경험

Abstract

암은 국내 사망원인 1위를 차지하는 주요한 질병으로 암 사망자 수가 증가하면서 말기암환자의 수는 지속적으로 증가할 것으로 예측되고 있어 이에 따른 사회적 관심이 요구된다. 말기암환자는 심한 통증을 비롯한 신체적 증상 외에도 불안과 우울 등의 부정적 정서반응과 영적 고통을 경험하는 것을 고려해 보면 이들의 증상관리를 포함한 질 높은 간호를 제공하기 위해 말기암환자를 돌보는 간호사는 환자의 요구에 민감성을 가져야 한다. 이에 본 연구는 말기암환자를 돌보면서 민감한 간호를 제공하는 간호사의 경험을 탐색하고자 시도되었다. 본 연구는 2016년 7월 20일부터 2018년 1월 15일까지 말기암환자를 5년 이상 돌본 경험이 있는 15명의 간호사를 대상으로 면대면 심층면담과 전화 면담을 통해 자료가 포화될 때까지 수집하였다. 수집된 자료는 Colaizzi (1978)의 현상학 연구방법을 적용하여 분석되었다. 자료 수집과 분석은 동시에 이루어졌으며, 자료는 면담 필사본, 연구자 메모, 현장노트를 이용하여 분석하였으며, 45개의 하위주제, 19개의 주제, 9개의 주제모음이 도출되었다. 본 연구의 결과, 말기암환자를 돌보는 간호사의 '민감한 간호'는 '이전의 경험에 대한 반성적 사고를 가짐', '죽음에 대해 수용적 태도를 지님', '환자와 친밀한 관계를 형성함', '환자의 요구에 주의 깊게 귀 기울임', '환자의 상태에 맞추어 응대함', '환자의 문제에 신속하게 대응함', '중대한 상황에 최적의 직관력을 발휘하여 대처함', '환자와 가족이 임종과정에 참여하도록 도움', '동료와의 팀워크에 개방적 사고를 가짐'으로 도출되었다. 말기암환자를 돌보는 간호사들은 자신과 가족의 질병경험과 이전의 환자를 돌본 경험을 통해 말기암환자의 입장을 되새기며 이전의 부족함을 되풀이하지 않는 반성적인 사고를 가졌다. 무엇보다도 자신의 생사관을 가지고 죽음에 직면한 환자에게 다가가 소통하였고, 때로는 환자와 가족들이 지향하는 죽음에 대한 신념과 가치를 수용하며 배려하는 태도를 보였다. 간호사들은 가족을 대하듯 환자를 정성스럽게 보살피며 환자와 함께 한다는 마음가짐으로 친밀한 관계를 유지하면서 환자의 요구변화에 주의 깊게 관심을 기울이고, 말과 행동에 조심하여 응대하며 환자의 상태에 맞추어 응대하였다. 간호사들은 환자의 어려움에 적극적으로 다가가 신속하게 대응하며, 환자가 직접적으로 표현을 하지 않아도 명민하게 환자의 문제를 알아차렸고, 죽음에 임박한 상황에 직관력을 발휘하여 대응하였다. 특히 환자의 임종 순간에 가족이 환자의 곁을 지키고 환자와 가족이 마지막 인사를 나눌 기회를 마련해주어야 한다는 당위성을 우선으로 제시하였다. 간호사들은 동료 간에 환자를 돌보면서 서로 격려하고 지지하였고, 동료들의 역할모델을 통해 간호사로서의 민감성을 높이고 팀워크를 다지는 개방적 사고를 나타냈다. 이와 같이 말기암환자를 돌보는 간호사가 경험하는 민감한 간호에는 간호사로서의 반성적 사고와 죽음에 대한 수용적 태도를 취하며, 환자와의 친밀한 관계 속에서 경청하며 눈높이에 맞추어 신속하게 대응하고 때로는 직관력을 발휘하여 임종의 순간을 놓치지 않고, 동료들과의 팀워크에 개방적 사고의 필요성을 제시하였다. 따라서 말기암환자를 돌보는 간호사들은 개인적으로 자신을 돌아보며 삶과 죽음에 대한 가치관을 확립하고, 환자들에게 친밀성, 경청, 환자 상태에 맞추어 응대, 신속성, 직관력을 발휘하며, 동료들의 역할모델을 통해 배우고 지지하면서 민감한 간호를 경험하였다. 본 연구의 결과는 말기암환자를 돌보는 간호사의 경험을 통해 민감한 간호의 태도와 간호행위에 대한 구조를 밝히고 이에 대한 이해를 높였다는데 간호학적 의의가 있다.;Cancer mortality is on the rise and the number of patients with terminal cancer is predicted to increase consistently, calling for public attention to this issue. Considering that patients with terminal cancer suffer from physical symptoms including severe pain, negative emotional responses such as depression and anxiety, and spiritual pain, nurses caring for this group of patients must be sensitive to their needs in order to provide quality nursing care that includes symptom management. Therefore, this study aimed to explore the experiences of nurses who provide sensitive nursing to patients with terminal cancer. Fifteen nurses who had provided care to patients with terminal cancer for at least five years were enrolled in this study. Data were collected until saturation via in-depth face-to-face interviews and phone interviews from July 20, 2016, to January 15, 2018. The collected data were analyzed using phenomenological analysis as developed by Colaizzi (1978). The data collection and analysis were performed simultaneously. Data from the interview transcripts, investigator’s notes, and field notes were analyzed and 45 subthemes, 19 themes, and 9 theme clusters were identified. We found that sensitive nursing provided by nurses caring for patients with terminal cancer involved 'reflective thinking about previous experiences', 'receptive attitude toward deat', 'intimate bond with patients', 'attention to patients’ needs', 'providing personalized care', 'prompt response to patients’ problems', 'response to serious situations with the best intuitive knowledge', 'supporting patients and their families to attend the dying process'and 'an open mind to teamwork with colleagues'. Nurses caring for patients with terminal cancer demonstrated reflective thinking, through which they reminded themselves of their patients’ perspectives based on their previous patient experiences and their own or their family’s disease history, and they avoided repeating the same mistakes. Most of all, they communicated with patients nearing death based on their view of life and death, and sometimes, they accepted and respected patients’ and their families’ beliefs and values concerning death. The nurses sincerely cared for patients like they would for their own family, maintained an intimate relationship with a caring mindset, paid close attention to changes in patients’ needs, were careful with their words and behaviors, and provided customized care according to the patient’s state. The nurses actively and promptly addressed patients’ difficulties, clear-sightedly recognized patients’ problems even if patients did not explicitly express them and used their intuitive knowledge to cope with issues in dying situations. In particular, the nurses expressed the appropriateness of patients’ families being given the opportunity to attend and bid farewell to patients at the moment of their death. The nurses encouraged and supported one another while caring for patients. In addition, they also demonstrated an open mind that increased their sensitivity as nurses and promoted teamwork based on role models provided by colleagues. As shown here, sensitive nursing provided by nurses caring for patients with terminal cancer was found to entail a reflective attitude as a nurse, a receptive attitude toward death, maintaining an intimate bond with patients, listening and promptly addressing patients’ needs, not overlooking the moment of death through the best intuition, and having an open mind to promote teamwork with colleagues. Therefore, nurses who provided care to patients with terminal cancer experienced sensitive nursing as they reflected on themselves looked over themselves, established their own values concerning life and death, established an intimate bond with patients, paid attention to patients, treated patients depending on their state, promptly responded to their needs using their best intuition, learned from role models, and supported their colleagues. The results of this study present meaningful nursing implications in that they revealed the attitude toward and structure of sensitive nursing and also enhanced the understanding about them by analyzing the experiences of nurses caring for patients with terminal cancer.
