Abstract
본 연구의 목적은 노인의 사회적 배제와 가정생활스트레스, 우울 간의 종단적 관계를 밝힘으로써 노인 우울 예방에 기여하고자 하는 것이다. 특히 노인의 경우 우울이 심화될 경우 치료가 어렵고 예후가 좋지 않기 때문에, 우울에 대한 예방이 무엇보다 중요하다. 그동안 우울예방에 있어서는 혼자 사는 독거노인을 중심으로 관심이 집중되어 있어, 부부가 함께 사는 노인의 우울예방에는 한계가 있었다. 이에 본 연구에서는 우울 예방에 있어서 상대적으로 관심이 적은 부부가 함께 사는 전기노인을 대상으로 하였다. 이를 위해 한국복지패널 8차(2013년 조사) ~ 12차(2017년 조사)의 반복 측정된 자료에서, 60세 이상 74세 이하의 부부만 함께 사는 노인 2,097명을 추출하여 최종분석에 사용하였다. 노인의 사회적 배제와 가정생활스트레스, 우울이 시간에 흐름에 따라 어떻게 변화하고 있는지를 잠재성장모형을 통해 살펴보고, 이들 간의 종단적 구조관계를 매개모형을 통해 확인해보고자 하였다. 또한 다집단 잠재성장모형 및 이를 확장한 다집단 잠재성장매개모형을 구축하여 성별에 따른 차이를 살펴보았다. 분석을 위해 STATA13과 MPLUS 7.0의 통계패키지를 활용하였다. 본 연구의 주요 결과를 요약하면 다음과 같다. 첫째, 부부가 함께 사는 노인의 우울평균은 60점 만점에 6.33~8.18점이었으며, 16점을 기준으로 한 우울 의심비율은 전체 응답자의 12.23%였다. 이 비율은 우리나라 전체 노인의 유병률에 비해서는 다소 낮은 수치라고 볼 수 있다. 우울은 시간이 지남에 따라 선형의 형태로 점차 낮아지는 것으로 나타났다. 이러한 감소형태는 개인 간의 차이를 보이지 않았다. 이는 우울의 정도에 있어서 부부가 함께 사는 노인이 매우 동질한 집단임을 의미하는 것이다. 둘째, 노인부부가구 노인의 사회적 배제의 수준을 횡단으로 살펴보면, 개인의 월평균 근로 및 연금소득, 기타소득을 기준으로 판단한 결과 경제적 배제의 경우 매우 높은 것으로 나타났다. 경제적 배제의 경우 시간의 흐름에 따라 변화가 없는 것으로 나타났다. 건강배제의 경우 3개월 이상 투병하는 경우가 많았으며, 사회활동배제의 경우 경제활동, 자원봉사/기부활동, 종교활동, 인터넷 사용의 총 4개의 영역 중 2개 이상의 영역에서 배제되고 있었다. 건강배제와 사회활동배제는 시간이 지남에 따라 선형으로 증가하는 것으로 나타나, 부부가 함께 사는 노인이 점차 배제되어 가는 것을 알 수 있었다. 셋째, 부부가 함께 사는 노인의 경우 가정생활스트레스는 평균적으로 보통 이하로 나타났으며, 시간이 지남에 따라 점차 선형으로 낮아지는 것으로 나타났다. 넷째, 잠재성장모형에서는 변화율과 변화율 간의 관계를 핵심으로 하며, 이를 근거로 살펴볼 때 사회활동배제의 변화율만이 우울에 직접적인 영향 및 가정생활스트레스의 변화율을 매개로 우울에 간접적으로 영향을 미쳤다. 다섯째, 경제적 배제가 우울에 직접적 또는 간접적으로 영향을 미치지 않았으나 가구소득을 기준으로 한 저소득 여부에 따라 우울의 수준이 다르게 나타났다. 이는 부부노인의 경우 개인의 소득보다는 가구 전체의 경제적 상황이 더 중요함을 의미한다. 여섯째, 건강배제는 기존의 노인 전체 또는 독거노인을 대상으로 한 선행연구에서 우울에 매우 중요한 영향을 미친다고 꾸준히 보고되었으나, 종단적인 영향은 미치지 않는 것으로 나타났다. 일곱째, 성별차이를 분석해 본 결과 우울, 가정생활스트레스, 사회적 배제에 있어 여성이 모든 시점에서 남성에 비해 더 취약한 것으로 나타났으나, 변화율에 있어서는 차이가 없는 것으로 나타났다. 또한 사회적 배제가 가정생활스트레스를 통해 우울로 가는 경로에서의 매개효과 역시 성별에 따른 차이를 보이지 않았다. 이는 곧 종단적인 변화에 있어서는 성별차이가 없는 것을 의미한다. 이에 따라 여성노인의 취약성 및 정신건강의 위험성을 강조하기 위해, 남성노인이 사회복지적 개입에서 소외되는 것에는 주의해야 함을 의미하는 것으로 분석된다. 본 연구는 그동안 독거노인에 비해 상대적으로 관심에서 소외되었던 부부가 함께 사는 노인을 대상으로 우울 예방을 위한 기초자료를 제공하였다는 데 의의가 있다. 특히 종단자료를 활용하여 시간의 흐름에 따른 사회적 배제, 가정생활스트레스, 우울의 성장궤적을 확인하고, 이들의 변화량과 변화량 간의 관계를 살펴보았다. 또한 사회적 배제가 우울로 가는 경로에서 노년기 부부가 함께 생활하는 시간이 증가한다는 것에 주목하여, 가정생활스트레스가 이를 매개하는지를 살펴보았다. 마지막으로 본 연구에서는 사회적 배제가 우울로 가는 경로 및 가정생활스트레스의 매개효과에 성별에 따른 차이가 있는지를 경험적으로 확인하였다. 이를 통해 노인부부의 우울예방을 위한 정책 및 서비스 개발에 기여할 것으로 기대한다. ;This study aimed to contribute to the prevention of depression among older people by understanding the longitudinal relationship among social exclusion, stress in family life and depression. When depression in older people is already significant, it is difficult to cure. Therefore, to find ways to effectively prevent depression in older people, this study was conducted on young-old couples who are relatively excluded from studies or/and intervention programs. Data from 2,097 young-old people between 60 and 74 years old and living only with a spouse that were extracted from the Korea Welfare Panel Study between the 8th survey in 2013 and 12th in 2017 were analyzed. It was explored that how the relationships of longitudinal structure among the social exclusion, stress in family life and depression had changed over time through latent growth model and the longitudinal structure among the said three variables was also explored through mediation model. Furthermore, the effects of the moderated mediation by gender were also examined by building the multiple group latent growth model and multiple group latent growth curve mediation model, which is an expanded model of the multiple group latent growth model. The STATA 13 and MPLUS 7.0 were employed for the analysis. The results can be summarized as follows: First, with 16 as the baseline score, the proportion of respondents with probable depression was 12.23%, which is somewhat lower than that of older people with depression in general. This percentage has shown a linear decrease over time, but there was no difference among individual respondents based on the shape of the decrease. It may mean that the respondents who are living only with a spouse are homogeneous in terms of the degree of depression. Second, in the cross-sectional analysis of the degree of social exclusion of respondents living only with a spouse, the degree of economic exclusion was very high in terms of monthly average income from labor, pension or/and others and showed no change over time. As to the exclusion by health condition, struggles against illness that last over three months often showed. As to the exclusion from social activity, respondents have experienced exclusion from over two kinds of activities among four kinds such as economic, volunteer/donation and religious activities and Internet utilization. Exclusions by health conditions and from social activities showed a linear increase, indicating that the older people living only with spouse have gradually been excluded. Third, the degree of stress in family life of respondents living only with a spouse was found to be lower than average and has shown a linear decrease over time. Fourth, only the change rate of exclusion from social activities directly affected depression and indirectly through mediation of the change rate of stress in family life according to the latent growth model that focuses on the change rates and the relationship among the change rates. Fifth, whether the respondents are in the low-income group affected the degree of depression, even though the economic exclusion has not affected the degree of depression, neither directly nor indirectly. This shows that, with the older people living only with spouse, the household income is more significant than personal income. Sixth, the exclusion by health condition has not shown the longitudinal effect on depression, even though previous studies for older people or older people living alone have reported that exclusion by health conditions significantly affect depression. Seventh, in the results of analyzing the differences from the degree of depression, stress in family life and social exclusion by gender, female respondents were more vulnerable than male respondents in all five times of survey. However, between male and female respondents, there was neither difference from the change rates of all three variables nor difference from the mediation effect of social exclusion in the path to depression through stress in family life. This indicates that there were no differences from longitudinal changes by gender. It also suggests that it should be cautious that older males may have been neglected by the social welfare interventions designed to emphasize the vulnerability of older females and their mental health. This study is significant because it provides the fundamental materials for preventing depression in older people living only with a spouse by focusing on them since they have been relatively neglected in previous studies compared with older people living alone. In this study, the growth trajectory of social exclusion and stress in family life over time and their change rate and the relationships among those change rates were explored by utilizing the longitudinal data. Furthermore, it was a new trial to investigate whether the stress in family life that may be caused by the increase in time that older couples spend together mediates the path from social exclusion to depression or not. Finally, it was empirically verified that there were no gender differences from the change rates of the path from social exclusion to depression and the mediation effects of stress in family life. The results are expected to contribute to the development of policy and services for preventing depression in older couples.
