Abstract
폐렴구균에 감염된 환자에서 콜레스테롤을 억제하는 약물로 알려진 스타틴(statin) 을 투여하였을 때 치사율과 사망률이 줄어들었다는 보고가 있다. 하지만, 스타틴이 어떠한 방식으로 폐렴구균에 의한 감염을 조절하는지에 대해 정확한 메커니즘이 밝혀진 바 없다. 스타틴은 콜레스테롤을 억제하는 기능과는 별개로 항염증 효과를 지니고 있는데, 우선적으로 폐 상피세포에서 폐렴구균 감염 동안 스타틴이 항염증 효과가 있는지 확인하고자 하였다. 또한, 이미 스타틴이 조절한다고 알려진 기전이 아닌 새로운 메커니즘에 관한 가능성을 염두하였다. 따라서, 지방과 밀접한 관계가 있으며 항염증 효과를 지닌다는 fatty acid binding protein (FABP)과 스타틴이 관련이 있는지 규명하고자 알아보고자 하였다. 사람 폐 상피세포인 A549와 BEAS-2B 세포에서 QPCR과 western blot 실험을 통해 FABP의 발현양상을 분석하고, S . p n e u m o n i a e D39 용균액에 의한 IL-8의 발현에 대한 스타틴의 역할과, 스타인에 의한 IL-8 억제 작용에서 FABP의 역할을 확인하고자 하였다. 폐렴구균 strain 중 혈청형 2(serotype 2)에 해당하는 S. pneumoniae D39 용균액을 처리하였을 때 증가한 IL-8의 발현이 스타틴에 의해 감소하여 스타틴이 폐렴구균에 의한 염증반응 동안 IL-8의 발현을 억제하여 항염증 효과를 지닌다는 것을 확인하였다. 또한, 폐 상피세포에서 FABP 단백질 중 FABP-2, 4, 5의 발현이 다른 FABP에 비하여 비교적 높게 발현됨을 확인하였으며, S. pneumoniae D39 용균액에 의해 FABP-2, 4, 5의 발현이 증가함을 확인하였다. FABP-2, 4, 5를 knock-down 시켰을 때, FABP-4 siRNA에서는 제외하고 FABP-2, 5 siRNA에서 S. pneumoniae D39 용균액에 의한 IL-8 발현의 증가를 더욱 더 증폭시켰다. 따라서, FABP-2와 FABP-5가 폐렴구균 처리에 의해 증가한 IL-8의 발현을 음성적으로 조절한다는 것을 확인하였다. 또한, FABP-2, 4, 5의 발현이 스타틴에 의해 증가하였으며, FABP-2, 5 siRNA에 의해 FABP-2, 5가 억제되었을 때, 스타틴에 의한 S. pneumoniae D39 용균액으로 유도된 IL-8에 대한 발현 억제효과가 소실됨을 확인하였다. 이러한 연구결과는, 스타틴이 FABP-4가 아닌 FABP-2, 5를 향상시킴으로써 폐렴구균 감염에서 염증반응을 조절할 수 있음을 의미한다. 즉, 본 연구 결과는 폐렴구균에 의한 염증반응에서 항염증 효과를 보이는 스타틴이 FABP-2, 5를 통해 염증을 억제한다는 것을 밝혀냈고, 더 나아가 폐렴구균 감염에서의 스타틴의 염증 조절 기작을 제시하고 있다.;It has been reported that the administration of the cholesterol-suppressing drugs known as statins (e.g. simvastatin, fluvastatin and pitavastatin) reduces the morbidity and mortality caused by Streptococcus pneumoniae . However, no exact mechanism has been found to explain how statins regulate infections caused by this pathogen. Statins have an anti-inflammatory effect apart from its cholesterol suppressing function. Thus, I tried to investigate whether statins exhibit an antiinflammatory effect during S . pneumoniae -induced inflammation in alveolar epithelial cells using the pneumococcus strain D39. It was confirmed that statins have an anti-inflammatory effect as they caused a decrease in the expression of S . p n e u m o n i a e D39 lysates-induced IL-8. Consequently, I searched a new mechanism which could explain this effect. Therefore, I focus my study on fatty acid binding protein (FABP), which is closely related to lipids and has an anti-inflammatory effect. I tried to determine whether there is a relation between FABP and statin. My experiments showed that FABP-2, 4 and 5 were highly expressed in alveolar epithelial cells, but their expression was enhanced after treatment with S. pneumoniae D39 lysates. Moreover, when FABP-2, 4 and 5 were knocked down, the increase in the expression of IL-8 caused by S . p n e u m o n i a e D39 lysates was further amplified in samples where FABP-2 and 5 were silenced, but there was no effect when FABP-4 was knocked-down. Thus, it was confirmed that FABP-2 and 5 negatively regulate the expression of IL-8, which increases due to pneumococcal treatment. Furthermore, while the treatment with statin enhanced the expression of FABP-2, 4 and 5, the silencing of FABP-2 and 5, after siRNA transfection, eliminated the effect of the statin in suppressing the S. pneumoniae D39 lysates-induced IL-8 expression. This means that statin could negatively regulate the inflammatory response to S. pneumoniae by upregulating FABP-2 and 5, but not FABP-4. In other words, it seems that the antiinflammatory effect of statin depends on the upregulation of FABP-2 and 5, which downregulate the expression of IL-8 during the inflammatory response induced by S. pneumoniae D39 lysates.
