Abstract
트랜스젠더는 태어날 때 지정된 성별과 스스로 인식하는 성별정체성이 일치하지 않는 사람들로, 그 차이 때문에 신체적, 정신적 불편과 고통을 겪게 된다. 이러한 불편과 고통을 성별위화감이라 하는데 이것은 트랜스젠더의 음성과 의사소통 방식에도 발생한다. 자신이 원하는 음성과 실제 음성과의 차이 때문에 트랜스젠더들은 말할 때 위축되고 개인 생활과 사회 생활에 제한을 겪게 된다. 이러한 음성 문제를 다루기 위하여 국외의 언어병리학자들은 트랜스젠더를 대상으로 다양한 연구과 중재를 진행하여 왔다. 트랜스젠더들의 음성을 음향학적, 생리학적, 청지각적으로 평가하여 그들의 음성 특징을 확인하고 이를 바탕으로 중재가 이루어져 왔다. 또한, 최근에는 트랜스젠더가 음성 문제로 겪게 되는 불편, 고통은 전문가에 의한 객관적 음성평가보다 트랜스젠더에 의한 주관적 음성평가가 더 중요함이 확인되면서 그에 대한 연구 역시 진행되고 있다. 하지만, 국내에서 트랜스젠더의 음성에 관한 연구는 현재까지 활발히 이루어지고 있지 않으며 그들의 주관적 음성평가는 한 번도 이루어진 적이 없다. 이에 본 연구에서는 주관적 음성평가 도구인 음성장애지수(Voice Handicap Index-10; VHI-10)을 통하여 트랜스젠더가 자신의 음성에 대하여 어떻게 인식하고 있는지 살펴보고자 하였다. 본 연구는 '한국 트랜스젠더 건강연구'의 일부분으로 진행되어 트랜스젠더의 건강상태를 확인하는 설문 문항 중 음성 문제를 확인하는 음성장애지수 10문항이 포함되었다. 설문에 응한 응답자는 트랜스여성(Male-to-Female) 145명, 트랜스남성(Female-to-Male) 78명이었으며, 이들과 비교가 될 일반인 집단은 과거 음성치료 경험이 있던 사람과 직업적 음성사용자 등을 제외한 83명이었다. 세 집단의 VHI-10 총점 평균은 트랜스여성 집단은 16.57, 트랜스남성 집단은 10.83, 일반인 집단은 3.27으로, 세 집단의 VHI-10 총점은 통계적으로 유의한 차이가 있었다(H=88.750, p<.005). 두 집단 간에 총점을 비교한 결과, 트랜스여성의 총점이 일반인 집단에 비하여 통계적으로 유의하게 높았으며(z=8.993, p<.005), 트랜스남성 역시 일반인 집단에 비하여 통계적으로 유의하게 높은 점수를 보였다(z=5.914, p<.005). 또한, 트랜스여성과 트랜스남성의 총점을 비교한 결과, 트랜스여성이 트랜스남성에 비하여 통계적으로 유의하게 점수가 높았다(z=3.897, p<.005). Arffa 외(2011)는 VHI-10 규준데이터를 산출하면서 그 총점이 11점이 넘게 되면 그것은 “비정상적(abnormal) 점수”로 여겨진다고 하였는데, 본 연구의 트랜스여성은 그 점수를 넘었고, 트랜스남성은 그 경계에 가까웠다. 또한, VHI-10을 고안한 Rosen 외(2004)는 연구에서 음성장애군별로 VHI-10 총점의 평균을 제시하였는데, 트랜스여성은 성대결절/성대폴립/성대낭종 장애군의 총점(15.51)과 유사한 점수를 보였으며 트랜스남성은 위산역류 장애군의 총점(10.49)와 유사한 점수를 보였다. 이러한 결과를 통해 트랜스여성, 트랜스남성 두 집단 모두 음성에 대한 인식 수준이 경도 음성장애 환자군과 유사함을 알 수 있었는데, 이것은 트랜스여성의 주관적 음성평가를 진행한 국외의 선행 연구 결과와 일치하였다. VHI-10 문항별 점수가 각 집단별로 어떠한 차이가 있는지 확인해 본 결과, 트랜스여성은 일반인에 비하여 모든 문항에서 통계적으로 유의하게 높은 점수를 보였다. 특히, 트랜스여성은 음성으로 인한 감정적 불편, 고통을 보여주는 E23, E25, F19에서 높은 점수를 보였는데, 그 값은 윤영선 외(2008)의 연구에서 제시된 음성장애 환자군의 문항별 점수 보다 더 높았다. 트랜스남성의 경우, 일반인에 비하여 F1, F3 문항을 제외한 8개의 문항에서 점수가 통계적으로 유의하게 높았는데 이를 통해 트랜스남성도 일반인에 비하여 자신의 목소리에 대하여 민감하게 인식하고 있음을 알 수 있었다. 트랜스여성과 트랜스남성 두 집단을 문항별로 비교해 보았을 때는 F1, F3, P17, F19, E23, E25 여섯 문항에서 트랜스여성의 점수가 트랜스남성에 비하여 유의하게 높게 나타났다. 이러한 차이는 호르몬 치료로 신체의 일부분은 여성화 되지만 음성은 변하지 않는 트랜스여성이 자신의 음성으로 불편과 고통을 더 크게 느끼면서 정서적으로 위축되기 때문이라 사료된다. 본 연구 결과를 통하여 트랜스여성과 트랜스남성 모두 일반인에 비하여 자신의 음성을 더 예민하게 인식하고 있음을 확인하였다. 특히 트랜스여성의 경우 성별 이행 과정 중 음성의 변화를 겪지 못하면서, 자신의 음성에 대해 성별위화감을 크게 느끼고 이는 곧 그들의 개인 생활과 사회 생활에도 영향을 미치는 것으로 확인되었다. 향후 트랜스젠더 음성연구가 더욱 활발해지면 트랜스젠더용으로 개발된 주관적 음성평가도구인 Transgender Self-Evaluation Questionnaire (TSEQ)와 Transsexual Voice Questionnaire for Male-to-Female Transsexuals(TVQMtF) 등을 사용하여 트랜스젠더들이 특히 어떤 상황에서 음성으로 인한 더 큰 어려움을 겪고 있는지 확인하고 이를 바탕으로 그들을 위한 중재 방안이 세워져야 할 것이다.;Transgender people have gender identity different from their assigned sex at birth. Because of the difference, they experience the distress which is called as ‘gender dysphoria’ in DSM-5. Voice plays an important role in gender recognition, so transgender people can experience gender dysphoria in their communication when their voice does not match with gender identity they perceive. Although transgender people do not have any voice disorder, their voice affect quality of their lives profoundly. To understand transgender’s discomfort with voice and communication, there is a growing body of research about their voice physiology, voice perception and quality of life, the application of specific treatment and intervention abroad. However, few research has been conducted to assess Korean transgender’s perceived voice quality. The goal of this study is to assess perceived voice quality of Korean transgender people using Voice Handicap Index-10 (VHI-10) and to compare it with that of general population. Furthermore, this research sought to examine how voice quality perception is different among Male to Female transgender(MtF) group compared to Female to Male transgender(FtM) group. VHI-10 is a short form of VHI-30, which is a self-assessment tool by patients with voice disorders. This study analyzed a nationwide cross-sectional survey of 232 Korean transgender people which included 145 MtF and 78 FtM transgender people. As a control group, 83 non-transgender adults without voice disorder completed the VHI-10 questionnaire. The mean score of VHI-10 was 16.57 among MtF and 10.83 among FtM transgender people. Compared to the control group whose mean score was 3.27, VHI-10 score was statistically significantly higher among MtF (z=8.993, p<.005) as well as FtM transgender people (z=5.914, p<.005). Considering that previous research suggested that the VHI-10 score of 11 or higher could be used to identify people with abnormal perceived voice quality, MtF transgender groups had mean score which are not within the normal range. When this study examined each of the 10 items of VHI-10 separately, MtF transgender people had statistically significantly higher score compared to the control group in all of 10 items. FtM transgender people had statistically significantly higher score in 8 items except the three items (e.g., F1, F3). When the score of MtF was compared to that of FtM transgender people, MtF transgender people have statistically significantly higher score in F1, F3, P17, F19, E23, E25 compared to FtM transgender. Especially, MtF group have high score regarding emotional questions. Thjs study found that both of Mtf and FtM transgender people has poorer perceived voice quality compared to non-transgender people without voice disorder. Especially, MtF transgender people, who cannot get benefit to change their voice in a process of hormone treatment, reported poor perceived voice quality, leading to experiencing difficulties in their social lives.
