This study evaluated the role of a primary cementless proximal porous-coated prosthesis in revision arthroplasty of the hip with substantial bone loss. It included 14 men and 22 women with a mean age at the time of the index revision of 56.8 years (range, 39 to 68 years). They were followed up for a minimum of 5 years (mean 6.5 years; range, 5 to 8 years). The Harris Hip score improved from a mean of 40 points (range, 22 to 55 points) before revision surgery to a mean of 85 points (range, 51 to 93 points) at the latest review. One femoral stem (3%) was revised for aseptic loosening. Two stems (5.5%) had a varus shift, and these were interpreted as radiologic loosening. All allografts were predictably united to the host femur. The medium-term result of this technique in selected cases was excellent, and the clinical outcome was very satisfactory. © 2004 Elsevier Inc. All rights reserved.-
