Abstract
The ICU is a department where nurses’ responsibility and roles for alarms are extremely crucial because its patients’ severity and alarm frequency are high. Unfortunately, there have been reports of patient deaths caused by ICU nurses’ inadequate responses to alarms. Therefore, it is necessary to understand how ICU nurses respond to alarms and find ways to improve their responses. The purpose of this qualitative study was to explore ICU nurses' experiences of alarm responses and to describe the essential structure of the experiences. The participants were 10 ICU nurses with a wide range of experience as a RN from 4 months to 7.4 years (3.2 years in average) at tertiary general hospitals in Seoul, Korea. Data was collected from September 2016 to May 2017 through semi-structured individual interviews until it was saturated. Analysis of collected data was conducted using Giorgi’s phenomenological method and it revealed 6 constituents and 15 sub-constituents that are essential for ICU nurses’ experiences of alarm responses. First constituent was “unintentionally failing to promptly react to alarms” and its sub-constituents were “not hearing alarms and missing them” and “failing to actively and confidently respond to the alarms because of lack of experience.” Second constituent was “recognizing the importance of alarms and staying alert of them” and its sub-constituents were “realizing the importance of alarms through alarm-related experiences” and “keeping ears open to any alarms.” Third constituent was “trying to grasp the alarm condition accurately by using multiple senses” and its sub-constituents were “using the alarm tones to differentiate the source equipment type and location,” “using numerical and graphical displays to discern false alarms,” and “personally assessing patients albeit the alarms may be false alarms.” Forth constituent was “feeling fatigued as a result of constant alarm irritation” and its sub-constituents were “being bothered by alarm noise pollution,” “feeling rushed to respond to alarms,” and “feeling overwhelmed by unresolved alarms.” Fifth constituent was “putting patient safety first and flexibly responding to alarms” and its sub-constituents were “adjusting alarm settings to reflect patient conditions” and “prioritizing alarm resolution based on alarm severity.” Sixth constituent was “having a sense of mutual accountability of alarms among nurses” and its sub-constituents were “actively participating in resolving other nurses' alarms in an emergency,” “paying attention to other nurses' alarms”, and “letting other nurses know that a alarm is getting resolved.” This study found the following essential structure of ICU nurses' experiences of alarm responses. ICU nurses unintentionally failed to promptly react to alarms at times, but they recognized the importance of alarms and stayed alert of them. They tried to grasp the alarm condition accurately by using multiple senses. Although they felt fatigued as a result of constant alarm irritation, they put patient safety first and flexibly responded to alarms. They had a sense of mutual accountability of alarms among nurses in order to save patients’ life. This study provides a holistic understanding of ICU nurses’ alarm responses, which could be utilized as the basis for improving their alarm responses.;중환자실은 환자의 중증도와 알람 발생 빈도가 높아 알람에 대한 간호사의 책임과 역할이 중요한 부서이다. 그러나 알람에 대한 간호사의 부적절한 반응으로 환자 사망을 초래한 사고들이 보고되고 있어 간호사의 입장에서 그들이 알람에 어떻게 반응하는가를 이해하고 개선 방안을 모색할 필요가 있다. 이에 본 연구는 중환자실 간호사의 알람 반응 경험을 탐색하고 그 경험의 본질적 구조를 밝히고자 수행되었다. 연구 참여자는 서울 소재 상급종합병원에 근무 중인 중환자실 간호사 10명이었으며, 임상 경력은 최소 4개월에서 최대 7년 5개월로 평균 3년 2.5개월이었다. 자료 수집은 2015년 9월부터 2016년 5월까지 반구조적 개별 면담을 통해 자료가 포화 상태에 도달할 때까지 진행하였다. 면담 횟수는 각 1회, 면담 소요 시간은 최소 30분에서 최대 120분으로 평균 48분이었다. 수집된 자료는 Giorgi 의 현상학 연구 방법을 적용하여 분석하였다. 그 결과 중환자실 간호사의 알람 반응 경험은 6개의 구성요소와 15개의 하위 구성요소로 도출되었다. 첫 번째 구성요소는 ‘마음과는 달리 알람에 신속하게 대응하지 못함’이며, 이에 대한 하위 구성요소는 ‘알람 소리를 듣지 못해 알람을 놓침’, ‘경험 부족으로 알람에 적극적이고 능동적으로 대처하지 못함’이었다. 두 번째 구성요소는 ‘알람의 중요성을 인지하고 경각심을 가짐’이며, 이에 대한 하위 구성요소는 ‘알람에 대한 경험을 통해 알람의 중요성을 깨달음’, ‘알람을 놓치지 않도록 소리에 주의함’이었다. 세 번째 구성요소는 ‘오감을 통해 알람 발생 상황을 정확하게 파악하려 노력함’이며, 이에 대한 하위 구성요소는 ‘알람 소리를 듣고 의료기기의 종류와 위치를 구별함’, ‘의료기기의 수치와 그래프를 보고 위양성 알람을 가려냄’, ‘위양성 알람일 가능성이 있더라도 환자 상태를 직접 사정함’이었다. 네 번째 구성요소는 ‘마음 놓을 틈을 주지 않는 알람으로 인해 피로감이 심함’이며, 이에 대한 하위 구성요소는 ‘알람으로 인한 소음 때문에 신경이 쓰임’, ‘알람에 빨리 대응해야 한다는 압박감을 받음’, ‘쉽게 해결되지 않는 알람에 심신이 지침’이었다. 다섯 번째 구성요소는 ‘환자 안전을 우선시하면서 알람에 유연하게 대처함’이며, 이에 대한 하위 구성요소는 ‘환자의 상황에 맞게 알람 설정을 조정함’, ‘알람을 위급도에 따라 우선순위를 정하여 해결함’이었다. 여섯 번째 구성요소는 ‘동료 간 알람에 상호 책임감을 가짐’이며, 이에 대한 구성요소는 ‘응급 상황이면 누구라도 알람 해결에 적극적으로 나섬’, ‘담당 환자가 아니어도 알람에 관심을 기울임’, ‘알람에 대응할 때 동료들이 신경 쓰지 않도록 소리 내어 알려줌’이었다. 본 연구를 통해 드러난 중환자실 간호사의 알람 반응 경험의 본질적 구조는 다음과 같다. 중환자실 간호사는 때로 마음과는 다르게 알람에 신속하게 대응하지 못할 때도 있었지만 알람의 중요성을 인지하고 알람에 경각심을 가지고 있었으며, 알람 발생 시 오감을 통해 알람 발생 상황을 정확하게 파악하려 노력하였다. 마음 놓을 틈을 주지 않는 알람으로 인해 심한 피로감을 느끼기도 하였으나 환자 안전을 우선시하면서 알람에 유연하게 대처하고, 동료 간 알람에 상호 책임감을 가지며 환자의 생명을 지키기 위해 노력하였다. 본 연구는 이와 같은 결과를 통해 중환자실 간호사의 알람 반응에 대한 총제적인 이해를 제공하였다는 데 의의가 있다. 중환자실 간호사의 알람 반응을 개선하는 방안을 모색하는 데 본 연구의 결과를 기초 자료로 활용할 수 있을 것이다.
