Abstract
Diabetic kidney disease (DKD) is the leading cause of end stage renal disease (ESRD), but the current pharmacological treatment for DKD is limited to renin-angiotensin system (RAS) inhibitors. NADPH oxidase (Nox)-derived reactive oxygen species (ROS) are increasingly recognized as the key factor in inflammation and extracellular matrix accumulation in DKD. APX-115 (3-phenyl-1-(pyridin-2-yl)-4-propyl-1-5-hydroxypyrazol HCl) is a novel orally active pan-Nox inhibitor and APX-115 derivative-01 is a derivative of APX-115. The present study, thus, examined the protective effects of APX-115 and APX-115 derivative-01 on streptozotocin (STZ)-induced diabetic mice and db/db diabetic mice, respectively. In experiment I, diabetes was induced by intraperitoneal injection of STZ (50 mg/kg/day for 5 days) in C57BL/6J mice. APX-115 (60 mg/kg/day) was administered orally to diabetic mice for 12 weeks. To compare the efficacy of APX-115, a separate group of diabetic mice were treated with an angiotensin receptor antagonist, losartan (1.5 mg/kg/day), which is the standard treatment strategy against kidney injury in diabetic patients. APX-115 effectively prevented kidney injury such as albuminuria, glomerular hypertrophy, tubular injury, podocyte injury, fibrosis, and inflammation as well as oxidative stress in diabetic mice as much as losartan. In addition, both APX-115 and losartan effectively inhibited mitochondrial and peroxisomal dysfunction associated with lipid accumulation. In experiment II, leptin receptor deficient C57BLKS/J-db/db mice were administered orally with APX-115 derivative-01 (10 and 60 mg/kg/day) or APX-115 (60 mg/kg/day), for 14 weeks. APX-115 derivative-01 at 10 mg/kg/day effectively prevented kidney injury such as albuminuria, glomerular hypertrophy, tubular injury, fibrosis, and inflammation as well as oxidative stress in diabetic mice as much as APX-115 at 60 mg/kg/day. These data suggest that APX-115 and APX-115 derivative-01 may become novel therapeutic agents against DKD.;당뇨병성 신증은 말기 신부전증을 유발하는 가장 흔한 원인질환이고, 현재 레닌-안지오텐신계 억제제 이외에는 탁월한 치료법이 부재한 상태로서 좀 더 효과적인 치료 전략이 필요한 실정이다. 당뇨병 진행 시 고혈당으로 인해 증가 된 활성산소가 당뇨병성 신증을 포함한 혈관 합병증의 주원인으로 받아들여지고 있다. 따라서, 활성산소를 생성하는 NADPH oxidase (Nox)는 당뇨병성 신증의 발생에 중요한 역할을 할 것으로 생각된다. 첫번째 연구에서는 streptozotocin (STZ)으로 유발한 제 1형 당뇨병성 신증에서 새로운 Nox 억제제인 APX-115의 신보호효과를 확인하였다. C57BL/6 생쥐에 STZ 50 mg/kg을 복강내로 5일간 투여하여 당뇨를 유발하였으며, APX-115 60 mg/kg을 12주간 매일 경구로 투여하였다. 또한 현재 사용중인 약물인 레닌-안지오텐신계 억제제의 효능과 비교하기 위해 losartan 1.5 mg/kg을 투여하였다. APX-115는 뇨중 알부민 배설, 혈청 cystatin C, 사구체의 비대, 세뇨관 손상, 신장 섬유화, 신장 염증, 산화성 스트레스를 losartan 만큼 감소시켰다. 또한 APX-115와 losartan은 신장 내 지방축적에 주요 원인으로 작용하는 미토콘드리아와 퍼록시좀의 기능장애를 개선하였다. 두번째 연구에서는 제 2형 당뇨병성 신증 모델인 leptin이 결핍된 db/db 생쥐의 신손상 진행에 미치는 영향을 규명하였다. db/db 쥐에 APX-115 derivative-01 10 과 60 mg/kg 또는 APX-115 60 mg/kg/day 를 매일 14주간 경구투여하였다. APX-115 derivative-01는 10 mg/kg 용량부터 뇨중 알부민 배설, 혈청 cystatin C, 사구체의 비대, 뇨관 손상, 섬유화, 염증 그리고 산화성 스트레스를 감소시켰다. 결론적으로 APX는 당뇨병성 신증에 동반된 신장의 섬유화, 염증, 산화성 스트레스를 완화시키고, 미토콘드리아와 퍼록시좀 생체 내 합성관련 유전자들의 발현을 높임으로써 신장 내의 지방축적을 효과적으로 개선할 수 있다. 이상의 결과로 보았을 때, Nox 억제제인 APX는 당뇨병성 신증에 대한 효과적인 새로운 치료 약물이 될 수 있을 것으로 생각된다.
