Abstract
다운증후군 태아와 정상 태아의 태반을 이용한 인간 유전체 발현 분석을 시행하여, 다운증후군에 동반된 질환 또는 합병증의 병태생리 기전에 대한 새로운 관점을 제시하고자 하였다. 2011년 3월부터 2016년 8월까지 제일병원 산부인과에 내원한 임신부 중 정상태아를 임신한 11명과 다운증후군 태아를 임신한 6명이 연구에 참여하였다. 이 중 5개의 정상 태아와 3개의 다운증후군 태아의 태반을 이용하여 GeneChip Human Genome U133 plus 2.0 array로 인간 유전체 분석을 시행하였고, bioinformatics tools을 이용하여 유전자 기능 및 유전자 상호 연관관계에 대한 분석을 시행하였다. 정상 태반과 비교 시, 다운증후군 태반에서 110종류의 유전자가 통계적으로 의미 있는 발현의 차이를 보였으며, 이중 77종류의 유전자가 상향 조절되어지고, 33종류의 유전자가 하향 조절되어짐을 확인하였다. 특히 상향 조절되는 유전자들 중 59.7% (n=46)가 21번 염색체에 존재하였고, 다운증후군과 정신지체, 신경행동 발달 장애 및 선천성 이상과 같은 다운증후군 합병증과 유의하게 연관이 있음을 확인하였다. 뿐만 아니라 21번 이외의 다른 염색체에 있는 유전자들이 다운증후군 태반에서 상향 또는 하향 조절되었다. 상호 신호 네트워크 분석 결과 53개의 유전자(상향 조절되는 유전자 40개와 하향 조절되는 유전자 13개)들이 신뢰지수 0.4(P < 1.39e-0.8)수준에서 동적 모델의 필수 부분임이 확인되었다. 이 네트워크 중 35개의 상향 조절되는 유전자와 8개의 하향 조절되는 유전자로 구성되어 있는 한 개의 중요 클러스터를 확인하였고, 이 클러스터의 중앙에 위치한 유전자는 SYNJ1으로 11개의 유전자와 연결되어 있었다. 본 연구는 다운증후군과 이의 합병증에서 나타나는 많은 생체 경로가 본 연구 결과에서 밝혀진 유전자들에 의해 조절 될 수 있음을 제시하였다. 이는 다운증후군 태반에 대한 전체적인 인간 유전체 발현 분석을 시행한 결과로써, 다운증후군의 병태생리를 밝히기 위한 다른 연구들의 근간이 될 수 있는 기초 연구로 사용될 수 있을 것이다.;To gain new insight of gene expression into the pathogenesis of trisomy 21 (T21) placenta, whole human genome expression analysis in placenta tissue samples from normal and T21 fetuses were performed. Between March 2011 and August 2016, pregnant women with normal (n=17) and T21 (n=11) fetuses who had antenatal care at the Department of Obstetrics and Gynecology, Cheil General Hospital, Dankook University College of Medicine were recruited. Whole human genome expression of placenta samples from normal (n=5) and T21 (n=3) fetuses was profiled using GeneChip Human Genome U133 plus 2.0 array and the functions of differentially expressed genes using bioinformatics tools were predicted. In the T21 placenta, one hundred-ten genes were significantly differentially expressed in comparision with the normal placenta. Among them, 77 genes were up-regulated and 33 genes were down-regulated in the T21 placenta. Especially, among up-regulated genes, over half (59.7%, n=46) were located on HSA21 and were significantly related to T21 and T21 complications, such as neurobehavioral manifestations, mental retardation, and congenital abnormalities. Moreover, various genes on other chromosome were up or down regulated in the T21 placenta. The interactive signaling network was statistically significant and suggested that 53 genes (40 up-regulated genes and 13 down-regulated genes) were an essential component of the dynamic complex of signaling under a confidence score of 0.4 (P < 1.39e-08). One major cluster of this networks was identified and consisted of 35 up-regulated genes and 8 down-regulated genes. Center gene of the cluster was synaptojanin 1 (SYNJ1) that was connected with 11 genes. This study showed possibility that these genes regulate many biological pathways that have been involved in T21 and T21 complications. Therefore, these results suggest a broad overview of whole human genome expression in the placentas of fetuses with T21 and could contribute to future research about genes associated with disease pathogenesis.
