- Abstract
- 본 연구는 복부 수술을 받은 성인 환자의 낙상에 대한 건강신념과 낙상 예방 수행에 관한 서술적 조사연구이다. 연구 대상자는 상급종합병원에 입원 후 복부 수술을 받은 지 5일 이내의 환자 136명이다. 연구자가 소속된 기관의 연구 심의 위원회에서 승인을 받은 후 2017년 9월 21일부터 10월 25일까지 자료수집이 이루어졌다. 연구 도구는 최유정(2004)의 손 씻기에 대한 건강신념 측정 도구를 박민경(2014)이 수정, 보완한 낙상 예방 활동 건강신념 도구와 한미정(2014)이 개발한 낙상 예방 수행 도구를 전문가 자문 후 수정, 보완하여 사용하였다. 수집된 자료는 SPSS 24.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson Correlation, 다중 선형 회귀분석을 이용하여 분석하였다.
본 연구의 결과는 다음과 같다.
1. 복부 수술을 받은 환자의 낙상과 관련된 건강신념은 평균총점 5점 중 지각된 민감성 3.60±0.442, 지각된 심각성 3.58±0.753, 지각된 유익성 4.15±0.232, 지각된 장애성 2.93±0.595, 행동계기 3.98±0.061이었다. 복부 수술을 받은 환자의 낙상 예방 수행은 평균 총점 5점 중 3.86±0.236으로 보통이상의 수행 정도로 보고되었다.
2. 대상자의 일반적 특성에 따른 건강신념 중 통계적으로 유의한 차이를 보인 건강신념은 다음과 같다. 지각된 민감성은 나이(F=2.668, p=.035)에 따라 유의한 차이가 있었으나, Duncan 사후검증에서는 의미 있는 결과를 보이지 않았다. 또한 낙상 고위험군이 일반군에 비해 지각된 민감성이 유의하게 높았다(t=-2.548, p=.012). 지각된 심각성은 학력(F=3.669, p=.028)에 따라 유의한 차이를 보였으며, Duncan 사후검증 결과 학력이 대졸 이상일 경우에 중졸이하보다 점수가 높았다. 지각된 장애성은 수술 후 기간 (F=3.036, p=.019)에 따라 차이가 있었으며, Duncan 사후검증 결과, 수술 후 기간이 4일째에 속한 환자가 수술 후 1~3일째 기간에 속한 환자보다 지각된 장애성이 높았다. 또한, 지각된 장애성은 일반군보다 낙상 고위험군(t=-2.371, p=.019)에서 점수가 높았으며 진단명에 따라서도 유의한 차이가 있었다(F=2.876, p=.025). Duncan 사후검증 결과, 간암을 진단받은 환자가 대장암, 췌담도암, 간기증자, 기타 복부 수술을 받은 환자보다 지각된 장애성이 높았다.
3. 낙상에 대한 건강신념과 낙상 예방 수행의 상관관계를 파악한 결과, 지각된 민감성(r=0.651, p<.001), 지각된 심각성(r=0.536, p<.001), 지각된 유익성(r=0.594, p<.001), 행동계기(r=0.509, p<.001)가 낙상 예방 수행과 양의 상관관계가 있었다.
4. 다중 선형 회귀분석 결과 회귀 모형은 낙상 예방 수행에 대해 47.3%의 결정 계수를 보여(F=15.944, p<.001) 일반적 특성인 나이, 성별, 학력과 건강신념이 낙상 예방 수행의 분산을 47.3% 설명해주고 있음을 알 수 있다. 회귀 모형에 포함된 변수 중 지각된 민감성(β=.440, p=.001)과 지각된 유익성(β=.244, p=.026)이 낙상 예방 수행에 영향을 미치는 것으로 나타났다.
복부 수술을 받은 환자의 낙상에 대한 건강신념 중 지각된 민감성과 지각된 유익성이 낙상 예방 수행에 영향을 미치는 것을 알 수 있어 환자의 건강신념을 고려하여 낙상 예방 교육을 시행하면 낙상 예방 수행이 향상될 것을 기대할 수 있다. 환자가 낙상 위험을 정확히 인식하고 낙상 예방의 유익성을 이해할 수 있도록 지각된 민감성과 지각된 유익성에 관련된 부분을 강조한 낙상 예방 교육이 필요하다.
;The purpose of this study was to discern the influences of health belief on fall-prevention performance among adult patients who experienced abdominal surgery. Data were obtained from 136 patients who had undergone abdominal surgery within 5 days between September 21 and October 25, 2017, after receiving approval from the institutional review board. Experts reviewed and modified the health-belief measurement tool and the fall-prevention guideline developed by the Korean Nurses Association and revised by Park (2014). The collected data were analyzed using SPSS version 24.0. Descriptive statistics, t-test, ANOVA, Pearson correlation and regression analysis were utilized.
Study results follow:
1. Among individual perceptions of health belief for falls, perceived susceptibility was 3.60 ± 0.442, perceived severity was 3.58 ± 0.753, perceived benefits was 4.15 ± 0.232, perceived barriers was 2.93 ± 0.595, and cues of action was 3.98 ± 0.061 among the total score of 5. The mean score of fall-prevention performance was 3.86 ± 0.236.
2. Age (F=2.668, p =.035) had a significant influence on the perceived susceptibility. The high-fall-risk group (F=-2.548, p=.012) had higher perceived susceptibility scores than the lower risk group. Educational background back ground (F=3.669, p=.028) had a significant influence on the perceived severity of health beliefs. In the case of lower than junior high school graduation, the score was lower than of university graduation. The length of a postoperative period (F=3.036, p=.019) was statistically influential on perceived barriers. High fall-risk patients (F=-2.371, p=.019) had significantly higher perceived barrier scores than lower risk patients. Perceived barriers were also significantly different according to diagnosis (F=2.876, p=.025) and showed the highest score in liver cancer patients and pancreatic cancer patients.
3. Perceived susceptibility (r = .651, p < .001), perceived severity (r = .536, p < .001), perceived benefits (r = .594, p < .001), and cues of action (r = .509, p < .001) had positive correlations with fall-prevention performance.
4. The multiple regression analysis showed statistically significant results explained by 47.3% of fall prevention performance (F = 15.944, p < .001). Fall prevention performance explained the variance affected by perceived susceptibility (β= .440, p = .001) and perceived benefits (β= .244, p = .026).
As mentioned above, perceived susceptibility and perceived benefits had a significant influence on fall-prevention performance. Results indicated that to improve the fall-prevention performance of abdominal-surgery patients, health beliefs should be of concern and educational programs should provide perceived susceptibility and perceived benefits to patients.
