노인 간호에 대한 종합병원 간호사의 지식, 태도, 수행에 관한 연구

Abstract

본 연구는 간호사의 노인 환자 간호에 대한 지식, 태도, 간호 수행에 대해 응급실과 병동 간호사를 비교하여 급성기 노인 환자 간호에 대한 지식, 태도, 간호 수행의 차이를 발견하기 위한 조사연구이다. 연구 대상자는 서울시 소재의 종합병원 3곳의 응급실 간호사와 병동 간호사 134명이었고, 연구도구는 Courtney 등 (2000b)이 개발한 Older patients in acute care survey (OPACS)의 일부분을 번역, 수정하여 사용하였다. 본 연구의 결과는 다음과 같다. 1. 노인 환자 간호에 대한 지식에서는 응급실 간호사가 병동 간호사 보다 많은 노인 환자들의 관절은 뻣뻣하다는 지식이 더 높은 것으로 나타났고 그 차이가 통계적으로 유의했다(t=-2.353, p=.020). 2. 노인 환자 간호에 대한 태도에서는 응급실 간호사가 병동 간호사 보다 노인 환자는 자신의 치료에 대한 결정을 내릴 수 있다는 것에 더 동의했고 그 차이가 통계적으로 유의했으며(t=-1.991, p=.049), 응급실 간호사가 병동 간호사 보다 너무 많은 노인 환자들이 생명 유지 치료를 받고 있다는 것에 더 동의했고 그 차이가 통계적으로 유의했다(t=-2.162, p=.032). 3. 노인 환자에 대한 간호 수행에서는 병동 간호사가 응급실 간호사 보다 노인 환자를 간호 하는 것이 어렵다고 하였고(t=1.983, p=.049), 노인 환자를 입원 시킬 때 더 많은 시간을 할애한다고 하였으며 그 차이가 통계적으로 유의했다(t=-2.992, p=.003). 이 외에 9가지 문항에서 응급실 간호사와 병동 간호사 간의 차이가 있었다.;The purpose of this study was to compare Emergency Department (ED) and ward nurses' knowledge, attitude and practices about older patients. The subjects (N=134) were ED and ward nurses working at three general hospital in Seoul. The research instruments was translated, modified and used as part of the Older patients in acute care survey (OPACS), developed by Courtney et al. (2000b). The result of this study is as below. 1. In terms of knowledge about geriatric nursing, ED nurses were more knowledgeable than ward nurses in the knowledge that the joints of older patients are stiff. And it was statistically significant(t=-2.353, p=.020). 2. In terms of attitude about geriatric nursing, ED nurses agreed more than ward nurses that older patients could make decisions about their treatment. And it was statistically significant(t=-1.991, p=.049). And ED nursed agreed more than ward nurses that too many older patients receive life-sustaining treatments. And it was statistically significant(t=-2.162, p=.032). 3. In terms of practice about geriatric nursing, ward nurses thought it was more difficult than ED nurses to care older patients(t=-1.983, p=.049) and spent more time to prepare and older patients for discharge(t=-2.992, p=.003). And these were statistically significant. In addition, there were differences in other 9 items between ED and ward nurses.
