한국의 수면유도제 장/단기 복용자의 일반적 특성 비교 및 장기복용 위험 요인 분석

Abstract

수면장애의 유병률이 증가하면서 수면유도제 처방건수가 증가하고 있으며 또한 수면유도제 장기 복용률도 증가하고 있다. 장기적인 수면유도제의 복용으로 약물의존성이 생기거나 수면유도제의 지속적인 효과로 인해 사고가 일어나는 등의 사회적 문제가 야기되고 있다. BDZ계열 또는 Z-drug 수면유도제의 장기복용으로 인한 부작용을 예방하기 위해서는 약물 사용 이전에 장기 복용자의 여러 가지 특성을 살펴보는 것이 필요하다. 본 연구는 표본 코호트 데이터베이스(NHIS-NSC)를 이용하여 표본 코호트 데이터베이스 내 건강검진 정보가 있는 20세 이상의 수면장애 환자를 대상으로 수면유도제 장단기 복용자들의 특성을 비교 분석하였다. 수면유도제 장기 복용위험 요인 분석 결과 수면유도제 복용자 중 고연령군, 남성, 흡연자, 비음주자에서 장기 복용위험이 높았다. 또한 저체중군과 co-morbidity가 많은 군일 수록 장기복용위험이 더 높았다. Z-drug계열의 수면유도제인 zolpidem의 경우 성별과 연령 흡연여부에 따라 장/단기 복용위험의 차이는 없었다. 그러나 비음주자, co-morbidity가 많은 군에서 장기 복용위험이 높았다. 또한 zolpidem을 단독 사용하는 군보다 zolpidem과 다른 BDZ계 수면유도제를 병용하고 있는 경우에 장기 복용위험이 더 높았다. 이번 연구에서는 표본 코호트 데이터베이스 내 건강검진 정보가 있는 20세 이상의 이상의 한국 성인들을 대상으로 수면유도제 장기, 단기 복용군의 일반적 특성을 비교하였고 그리고 복용자들의 공존질환과 수면유도제 장기복용 위험과의 관련성을 분석하였다. 이번 결과를 참고로 수면장애 환자에서 생활습관의 개선 등의 비 약물적 보조치료 방안을 모색해 볼 수 있을 것이다. 본 연구에서는 수면유도제의 장기복용 위험요소들을 파악하고자 하였다. 수면유도제 장기복용 위험이 있는 대상자를 사전에 파악하고, 수면유도제를 안전하고 효과적인 치료제로써 활용하는 것에 본 연구 결과가 활용 가능할 것으로 기대된다. ;As the prevalence of sleep disorders increases, the number of prescribed sleep inducing agent increases and the long - term use of sleep inducing agents is also increasing. The risk of long-term use of zolpidem was higher when zolpidem was used in combination with other BDZs sleep inducing agents than zolpidem alone. The purpose of this study was to compare the general characteristics of long - term and short - term use of sleep inducing agents in Koreans over 20 years of age with health screening information in a sample cohort database and to analyze the relationship between the coexisting disease and the risk of long - term use of sleep inducing agents. The results of this study can be used to find non-pharmacological adjunctive therapies such as improvement of lifestyle in patients with sleep disorders. The purpose of this study was to identify the risk factors for long-term use of sleep inducing agents. The results of this study are expected to identify patients who are at risk of long-term use of sleep inducing agents and to use sleep inducing agents for safe and effective.
