북한이탈주민의 남한 간호사로서의 체험

Abstract

The number of North Korean defectors entering South Korea has been increasing annually. In line with the fact that 71% of them being women, and the majority being the core production population of 20s to 40s, the government’s policy to support the North Korean defectors is aimed at self-reliance and self-sufficiency through education and employment, also the selection and retention of their job is becoming an important issue. In particular, North Korean defectors who pursues a better life based on the professionalism and stability of their profession are increasingly choosing nurses as a career. However, in addition to adapting South Korean society with different systems, environments and cultures, it can be expected that adapting to the field will never be easy. Therefore, it will be necessary to understand the experience of North korean defectors as a nurse as an extension to the life in South Korea, and to explore the essential theme and semantic structure inherent in their experience in depth. The purpose of this study is to clarify the essential meaning structure of the experience of North Korean defectors as a South Korean nurse by applying van Manen' s hermeneutics phenomenological method. Participants of this study were 9 nurses who had worked for months after graduating from the Korean Nursing University. Data collection was conducted from May to November of 2017 through individual in-depth interviews, observations, and telephone interviews. The collected data were analyzed using the subdivision method, selective method, and holistic method as suggested by van Manen, along with the researcher's personal experience, literature, and works of art. As a result of the study, eight essential topics and 30 subtopics were derived. The first topic is “half anticipating and half worried to take the first step as a South Korean nurse”, This topic is about the process of a nursing college graduate getting a license and finding employment at a hospital. The second topic is the theme of “the life of a new but old nurse being worse than expected”, describing a new life that was mentally and physically difficult due to the old age and tag of being a North Korean defector, different from their expectations. The third topic is about the experience of nursing as the participants faced the patients, and was found to be “the realization of the invisible barriers and limitations in nursing patients.” The fourth topic is “Being stranger in the harsh culture of nursing organization.” illustrating the experience of prejudice, hostility, relative deprivation and the difficulties in communication due to difference in ways of thinking or vocabulary as North Korean defectors. The fifth topic is “overcoming the crisis with their own survival strategies and the support of the others”, containing the story of withstanding the difficult times, finding confidence again after a certain years of experience, and enduring the difficult hospital life with the help of the people around them and the family as their driving force. The sixth topic is about moving to a job that they can work for a long period of time with a consideration of their situation and position in order to live a comfortable life, which was deducted to “selecting the job that they can have for a long time as they comprise with the reality.” The seventh topic was “taking the pride of their job with satisfaction of hospital life”, which showed that they were able to live a more pleasant working life because they were satisfied with the workplace that was considerate of them although their income was reduced. Finally, the passion to leap into a more worthwhile life and the desire to fulfill their part with a sense of calling for unity in the land that gave freedom and opportunity to them was found in the topic of “pledging to contribute to nursing in South and North Korea trough my leaping life” The significance of this study is found in that it presents in depth understanding and various perspectives on North Korean defector nurses through its exploration of the meaning and essence of the vivid experience of North Korean defectors as South Korean nurses. The results of this study can contribute to the development of an effective education program and policy system for the adaptation of the hospital life and job retention of North Korean defector nurses in South Korea.;남한에 입국한 북한이탈주민의 수는 해마다 증가하고 있다. 이 중 71%가 여성이고, 대다수가 20-40대의 핵심생산인구인 것을 고려하여 정부의 북한이탈주민 지원정책도 교육과 취업을 통한 자립과 자활을 목표로 하고 있으며, 그들의 직업선택과 유지가 중요한 이슈로 대두되고 있다. 특히 직업의 전문성과 안정성을 기반으로 더 나은 삶으로 전환하고 싶은 북한이탈주민이 간호사를 진로로 선택하는 경우가 점차 증가하고 있지만 살아온 체제와 환경, 문화가 다른 남한 사회에서의 적응 뿐 아니라 임상간호현장에 적응하는 것은 결코 쉽지 않을 것으로 예상된다. 따라서 그들의 남한에서의 생활과 간호사로서의 체험을 연속선상에서 이해하고, 그 속에 내재된 본질적 주제와 의미구조에 대한 심층적인 탐색이 필요할 것이다. 이에 본 연구는 van Manen의 해석학적 현상학 방법을 적용하여 북한이탈주민의 남한 간호사로서의 체험의 본질적 의미구조를 밝히고자 하였다. 연구 참여자는 북한이탈주민 중 대한민국 간호대학 졸업 후 국가고시 합격하여 면허를 취득한 자로 현재 의료기관에 근무하거나 최근 6개월 이내 근무했던 간호사 9명이었으며 자료 수집은 2017년 5월부터 11월까지 참여자와의 면대면 심층면담과 관찰, 전화면담을 통해 이루어졌다. 수집된 자료는 연구자의 개인적 경험, 문학 및 예술작품 등의 자료를 포함하여 van Manen이 제시한 세분법, 선택적 방법, 전체론적 방법을 사용하여 분석하였다. 연구 결과, 총 8개의 본질적 주제와 30개의 하위주제가 도출되었다. 첫 번째 주제는 ‘기대 반, 걱정 반으로 ‘남한 간호사’로서 첫 발을 내딛음’으로, 이는 간호대학을 졸업한 북한이탈주민이 면허를 취득하여 병원에 취업하기까지의 과정에 대한 내용이다. 두 번째 주제는 ‘나이 많은 신규간호사 생활이 예상보다 더 혹독함’으로 기대와 달리 많은 나이와 북한 출신이라는 꼬리표 때문에 힘들었던 신규생활에 대한 주제이며, 세 번째 주제는 환자를 간호하면서 부딪힌 어려움과 막막함의 체험에 대한 내용으로 ‘환자를 간호함에 있어 보이지 않는 장벽과 한계를 실감함’이다. 네 번째 주제는 ‘냉혹한 간호조직문화 속에서 이방인이 되어감’으로 동료간호사와의 관계에서 편견과 적대감, 차별대우와 상대적 박탈감의 경험, 사고방식이나 어휘의 차이로 인한 의사소통의 어려움에 대한 내용이며, 다섯 번째 주제는 ‘자신만의 생존전략과 주변의 지지로 위기를 극복함’으로 힘들었던 시간을 버텨내고 어느 정도 연륜이 쌓인 뒤 다시 자신감을 찾고, 주변의 도움과 가족에 대한 책임감이 버틸 수 있는 힘이 되어 어려운 병원생활을 견디어 내는 내용이다. 여섯 번째 주제는 자신의 상황과 처지를 고려하여 오래 일 할 수 있는 근무지로 옮겨 심신이 편안한 삶을 전략적으로 선택하는 내용으로 ‘현실과 타협하여 오래 일할 수 있는 일자리를 선택함’으로 도출되었다. 일곱 번 째 주제는 ‘병원 생활에 대한 만족감으로 직업에 대한 긍지를 가짐’으로 경제적인 수입은 줄었지만 자신을 배려해주는 근무지에 대한 만족감으로 더욱 즐거운 병원생활을 할 수 있었다. 마지막으로, 좀 더 가치 있는 삶으로 도약하고자 하는 열망과, 자유와 기회를 준 땅에서 통일 이후의 시대에 대한 기대를 품고 자신들의 역할을 다하고자 하는 희망을 담은 ‘남북한 간호에 이바지하기 위해 도약하는 삶을 희망함’에 대한 주제이다. 본 연구의 의의는 북한이탈주민의 남한 간호사로서의 생생한 체험의 의미와 본질을 탐색함으로써 그들에 대한 심층적인 이해와 다양한 시각을 제시하였다는 데 의의가 있다. 본 연구의 결과는 북한이탈주민 남한 간호사의 병원 생활 적응과 직업유지를 위한 효과적인 교육 프로그램, 정책 및 지원제도 개발에 기여할 수 있을 것이다.
