- Abstract
- 본 연구는 파킨슨병 내담자들의 상지 운동 조절력 향상을 위해 노래 부르기 전략을 사용한 드럼 타악기 연주 중재를 실시하여 내담자들의 상지 운동 조절력 변화에 대해 상세하게 알아보고자 하는 것이 목적이다. 총 3명의 파킨슨병 내담자들을 대상으로 8회기 중재를 시행하였다. 상지 운동 개시와 청각-상지 운동간 동조화를 상지 운동 조절의 하위 요인으로 설정하고 연주 개시까지 걸린 시간 및 연주를 통해 청각-상지 운동간 동조화가 일어나기까지 걸린 시간이 단축된 경우 상지 운동 조절력이 향상되었다고 조작적으로 정의하였다. 중재 전-후 상지 운동 조절력의 변화를 살펴보기 위해 전자드럼패드와 MIDI 프로그램을 사용해 두 가지 요인을 단계적으로 나눈 과제를 제시하였고, 측정된 변화를 비교 분석하였다. 또한 운동 기능향상을 목적으로 한 중재 프로그램이 실제 내담자들의 수지기능과 일상생활 동작에 어떠한 변화를 초래하는지 알아보고자 Box and Block Test와 Jebsen & Taylor Hand Function Test를 실시하였다. 실시된 중재는 청각 자극으로써 제공되는 다양한 속도의 리드믹 큐에 맞춰 리듬 연주하기를 반복적으로 시도하여 청각-운동간 동조화를 통해 결국 내담자가 스스로 상지 운동 패턴을 예측, 계획, 조절, 실행 하도록 하여 결국 상지 운동 조절력을 향상 시킬 수 있도록 하였다. 또한 운동과제 수행 시, 노래 부르기를 전략으로 사용함으로 상지 움직임을 촉진시키고 내담자의 과제수행 결과로 만들어진 음악적 산물로 보상받는 음악 경험을 통해 긍정적인 정서를 불러일으키게 함으로써 활동에 대한 참여도를 향상시키고 높은 강도의 반복적인 움직임에도 참여수준을 일정하게 유지할 수 있도록 하였다. 연구 결과, 내담자 모두 상지 운동 조절력에 변화가 나타났음을 알 수 있었다. 또한 악기연주 시 노래 부르기를 병행함으로 내담자들의 긍정적 정서가 불러일으켜졌으며 이러한 현상은 내담자들의 활동에 대한 흥미와 참여도를 증진시켜 과제 수행의 지속을 도와 결국 상지 기능을 향상시키는데 긍정적인 영향을 미쳤음을 알 수 있었다. 본 연구는 퇴행성 뇌 손상이 원인인 파킨슨병 환자를 대상으로 노래 부르기가 전략으로 사용된 드럼연주 중재가 내담자들의 상지 운동 기능에 미치는 영향에 대해 수집된 데이터를 전체 대상의 양적 변화와 내담자별 질적 변화를 함께 제시하여 상세히 분석하여 제시하였다. ;The purpose of this study is to provide an overview of improving upper limb movement of clients suffering from Parkinson's disease by playing percussion instruments with singing. 8 sessions of music intervention were conducted to 3 patients with Parkinson’s disease. Sub-factors of control function of upper limb movement in this study were initiation of upper limb movement and auditory-upper limb movement entrainment. These two factors were tested using electronic drum pads and MIDI Program to measure changes in upper limb function before and after the intervention. Furthermore, Box and Block test and Jebsen & Taylor Hand Function Test were implemented to find out whether this intervention caused any changes of hand functions and movement of daily lives. The result of this study suggested that clients' control of upper limb movement was improved by predicting, forecasting, controlling, implementing patterns of upper limb movement by playing instruments following the rhythmic cue with varying tempo. Also, when clients play drum, singing task was added to stimulate and maintain upper limb movement of clients with high repetition. As a result, this study found out there were changes in upper limb movement. Also, singing while playing instruments caused positive emotion and led clients to keep interested in the tasks and motivated enough to continue their tasks. Quantitative changes of the all clients and qualitative changes of each client were confirmed. This study suggested that the use of singing strategy can be effective in drum playing interventions for clients with Parkinson’s disease.
