dc.description.abstract우리나라 장애인 현황을 살펴보면 15-19세 장애 군에서 지적장애가 50% 이상이며, 그 중 3급 이상의 경도지적장애가 80%를 차지한다. 상당수의 경도지적장애 청소년은 일반학교 특수학급에서 통합교육을 받고 있으며, 그들은 지적능력과 개념적, 사회적, 실제적 영역의 적응기술인 적응행동의 결함으로 인해 의사소통, 학습, 행동 등 전반적인 측면에서 곤란을 겪는다(AAIDD, 2010). 지적장애 아동의 경우 인지적 결함으로 언어의 전반적 측면에서 지체되어 있고, 정보처리체계 손상으로 자기규제 장치인 실행기능에 어려움이 있다. 이로 인해 목표어 사이에 의미 전달과 관계없는 낱말이나 음절을 삽입하는 ‘간투사’, 발화 사이에 나타나는 소리 없는 침묵인 ‘주저’ 그리고 다음절 낱말, 어절 및 구의 ‘반복’과 같은 비유창성을 출현하게 하며 말 속도에 영향을 미친다. 따라서 지적장애인은 여러 요인의 작용으로 비유창하고 말 속도가 느린 경향을 보인다. 의사소통의 기본 바탕인 언어 연쇄는 말하는 이의 언어학적 산출과 듣는 이의 언어학적 이해를 통해 이뤄진다. 그러나 언어 연쇄 과정에 문제가 생기면 언어학적 차원에서의 어려움인 언어장애, 말 산출 과정에서 어려움인 말장애, 듣는 과정에서의 어려움인 청각장애를 초래하며, 이는 의사소통 장애의 원인으로 사회적 관계 형성을 어렵게 한다. 이처럼 느리고 비유창한 경향이 나타나는 지적장애인의 말 특성은 언어를 통한 의견 표현, 또래관계 형성 및 관계 유지, 사회적 규범 준수와 같은 사회화 과정을 저해하게 된다. 따라서 지적 장애인들의 비유창성 원인과 유형을 알아보는 것은 이론적, 임상적 차원에서 의미를 가질 것이다. 본 연구의 목적은 언어적 복잡성이 다양한 과제를 활용하여 경도지적장애 학생과 일반 학생 간의 총비유창성 비율과 정상적비유창성(Others Disfluencies: OD), 비정상적비유창성(Stutter-like Disfluency: SLD) 비율 차이를 비교하며 이를 통해 경도지적장애 학생의 비유창성 경향을 알아보는 것이다. 연구대상은 경기 지역에 거주하는 중학교 1 – 3학년 경도지적장애 학생과 생활연령 및 성별이 같은 일반 학생 각 15명으로 집단별 그리고 과제별 비유창성 비율 및 특성을 살펴보기 위해 모든 대상자에게 P-FAⅡ(심현섭·신문자·이은주, 2010)의 ‘읽기’ (보고픈 할머니), ‘이야기다시말하기’(아낌없이 주는 나무), ‘단일그림설명하기’(우리동네, 운동회) 과제를 실시하였다. 비유창성 분석에는 Ambrose & Yairi(1999)의 말더듬 진단기준 및 비유창성 분류기준에 따라 비정상적비유창성(SLD: stuttering-like disfluency)과 정상적비유창성(OD: other disfluencies)으로 나누며 정상적비유창성(OD)은 주저, 간투사, 수정(음운수정, 의미수정, 문법수정), 다음절단어/구 반복을 포함하며, 비정상적비유창성(SLD)은 부분낱말반복, 1음절낱말반복, 연장, 막힘, 깨진 낱말을 포함한다. 비유창성 유형별 비율의 집단 간 차이를 알아보기 위해 다변량분산분석(MANOVA)을 실시하였으며, 세 가지 과제에서 나타난 집단 간 비유창성 비율 차이를 분석하기 위해 이원혼합 분산분석(two way mixed ANOVA)을 실시하였다. 과제종류와 집단에 따라 유의미한 차이가 나타났을 경우 Bonferroni 사후 검증을 실시하였다. 연구결과를 요약하면 다음과 같다. 첫째, 비유창성 유형에 따른 집단 간 비유창성 비율에 차이가 있었다. 경도지적장애 학생은 일반 학생보다 높은 총비유창성, 정상적비유창성, 비정상적비유창성 비율을 보였다. 또한 비유창성 유형(정상적‧비정상적)에 대한 집단 내 비율 차이가 나타났는데, 두 집단 모두 정상적비유창성 비율이 비정상적비유창성 비율보다 더 높았다. 둘째, 과제종류에 따른 집단 간 비유창성 비율을 살펴본 결과, 총비유창성, 정상적비유창성, 비정상적비유창성 비율 모두 차이가 있었다. 다만 경도지적장애 학생은 ‘읽기’ 과제에서만 비정상적비유창성이 정상적비유창성 비율보다 높게 나타났다. 이는 선행연구결과 경도지적장애 학생의 읽기특성에 기인한 것으로 유추할 수 있다. 과제종류에 따른 비유창성 비율에서도 차이가 나타났다. 두 집단 모두 ‘읽기’ 과제에 비해 ‘이야기다시말하기’ 과제와 ‘단일그림설명하기’ 과제에서 총비유창성, 정상적비유창성 비율이 더 높았지만 비정상적비유창성은 유의한 차이가 나타나지 않았다. 따라서 총비유창성과 정상적비유창성은 과제종류와 집단 간의 이차 상호작용이 통계적으로 유의하게 나타난 반면 비정상적비유창성은 과제종류와 집단 간 이차 상호작용이 통계적으로 유의하지 않았다. 셋째, 집단 간 비유창성 유형 특성을 비교해본 결과, 경도지적장애 학생은 주저, 간투사, 의미수정 일반 학생은 간투사, 주저, 의미수정 순으로 비유창성 하위유형비율이 높게 나타났다. 11개의 하위유형 중 집단 간 두드러진 비율 차를 보이는 것은 주저, 간투사, 의미수정이었다. 본 연구 결과는 경도지적장애 학생 집단이 일반 학생 집단에 비해 비유창성 출현이 더 많음을 나타내며 이는 지적능력과 비유창성 출현율의 상관관계가 높다고 보고한 선행연구 결과와 일치하였다. 경도지적장애 학생의 비유창성 특성을 살펴보면 비유창성 유형 내에서 특정 하위유형이 차지하는 비율이 일반 학생과 유사하였다. 이는 일반 학생의 언어적 비유창성 특성과 동일한 양상으로 경도지적장애 학생도 언어를 계획 및 형식화하여 산출하는 과정 상 어려움으로 언어적 비유창성이 나타난다고 해석할 수 있다. 또한 과제유형에 따라 비유창성 비율이 유의한 차이가 있음을 알 수 있었다. 비유창성은 내적 요인인 개인의 언어능력과 외적 요인인 과제 복잡성의 영향으로 가변적 성격을 띤다. 이러한 맥락에서 위 연구결과는 정확한 비유창성 평가를 구축하기 위해 임상 현장에서 다양한 과제 적용의 필요성과 평가 및 중재에 실질적으로 적용할 수 있는 과제 개발의 중요성을 시사한다.;The purpose of this study is to investigate characteristics of speech disfluency – both OD(Others Disfluency) and SLD(Stutter-Like Disfluency) – in adolescents with mild Intellectual Disability and age�matched control adolescents according to speech production tasks. The subjects of this study were fifteen adolescents with mild intellectual disability and fifteen age�matched normal adolescents in first to third grades of middle school. They were tested on Korean Wechsler intelligence Scale for Children-Ⅳ(K-WISC-Ⅳ), Receptive-Expressive Vocabulary Test(REVT), Reading, story retelling, and picture description tasks were used to elicit utterances from the participants. Three questions were addressed in this study. 1) Are there significant differences in the frequency of OD and SLD between the Adolescents with Mild Intellectual Disability and the control group? 2) Are there significant differences in the frequency of OD and SLD among the types of tasks (i.e., reading, story retelling, and picture description)? 3) What are the qualitative characteristics of speech disfluencies in the Adolescents with Mild Intellectual Disability? There was no statistically significant difference in gender and years of education between the groups except IQ(FSIQ) scores, and Analysis of covariance was used to adjust the difference of IQ(FSIQ) scores. Reading, story retelling, and picture description tasks were conducted to elicit utterances from the participants. The findings indicated that Adolescents with Mild Intellectual Disability produced significantly more stuttering-like disfluencies (SLD) and other disfluencies (OD) when compared to the control group during all three tasks. To investigate the disfluency characteristics of the Adolescents with Mild Intellectual Disability in different speaking tasks, This study used reading, story retelling, and picture description tasks from the Paradise-Fluency Assessment-Second Edition (P-FA-II; Sim, Shin & Lee, 2010) to collect participants’ speech samples. All participants were tested individually in a quiet room in their schools. The experimental tasks were completed in one session that lasted no longer than an hour. Three P-FA-II tasks were administered to collect the speech samples: reading, story retelling, and picture description. During the reading task, participants read the text: “My longing grand mother”. In the second, the participants heard the story of “The giving tree”, and then, They retold the whole story while six sequential pictures of the story were presented as a prompt. During the picture description task, participants described two pictures presented to him. Two pictures were used to elicit spontaneous utterances from the adolescents, and the pictures illustrated two different situations. All speech samples were collected and transcribed on videotape. A total of 1200 syllables (i.e., 600 from the reading task and 300 each from the story retelling and picture description tasks) were collected, The number of disfluent syllables (both OD and SLD) per 100 syllables (i.e., percentage) was calculated for each task to make the task results comparable. The types of disfluencies in the OD categories were hesitation, interjections, revision and multi-syllable/phrase repetitions. The types in the SLD categories were single-syllable word repetitions, part-word repetitions, and prolongations, blocks, and broken words. To examine the differences in the characteristics of revision between the two groups; the revision was divided into three types. Frequencies for each of OD and SLD per 100 syllables were calculated for each participant and were used to compare group mean differences. Results from this study were as follow The result showed significant difference in disfluency frequency according to types of speech production tasks between groups. Also, there were significant difference in disfluency frequency according to types of tasks. The findings indicated that adolescents with mild Intellectual Disability produced significantly more prone to stuttering-like disfluencies (SLD) and other disfluencies (OD) when compared to the control group during all three tasks. Further statistical analysis showed that adolescents with mild Intellectual Disability produced more OD during the story retelling task than the other two tasks, whereas significant differences in OD were observed in picture description among the three tasks in the control group. The second aim of this study was to investigate the effects of the three different tasks on the frequency of OD and SLD across the two groups. For the frequency of OD, there was a significant difference for the task, story retelling task generated significantly more OD than did the reading task . Also, the picture description task generated significantly more OD than did the reading task. However, no significant differences were observed in the frequency of OD between the story retelling and picture description tasks. For the frequency of SLD, there was no significant difference for the task, Finally, adolescents with mild Intellectual Disability exhibited a higher proportion of SLD in reading task than the control group. These results are consistent with previous studies that adolescents with mild Intellectual Disability cannot achieve well on reading abilities in aspects of reading comprehension in the level of complex sentence or paragraph. And also it was hard to comprehend meaning of various word and the relationship between words was confusing to adolescents with mild Intellectual Disability. A significant difference was found between different types of disfluency. The frequency of hesitation and interjection was significantly higher than the others. In terms of types of disfluency, hesitation and interjection were most frequently observed. This is probably because hesitation and interjection are related to cognitive processing, especially, language planning. Further, both groups more often exhibited lexical revision among revisions. the frequency of lexical revision was significantly higher than grammar revision. Revisions increase when the language demand exceeding the child’s capacity. Additionally, revisions are the product of an interaction between constructing components of internal language and monitoring ability. This study suggests that both groups take on a similar aspect. Furthermore, These results mean that significant difference in disfluency according to types of tasks, and need using on different types of tasks when assessing speech disfluency.-
