Effect of Blood Pressure Control on Cardiovascular Mortality in Chronic Kidney Disease

Abstract

만성 신장 질환은 성인 인구의 10 ~ 15 %에 영향을 미치는 일반적으로 장기적인 질병이며, 만성 신장 질환의 발병률은 전 세계적으로 증가 할 것으로 예측되었다. 만성 신장 질환은 말기 신질환 및 심혈관 질환의 주요 위험 요소이기 때문에, 엄격한 혈압 조절을 통한 만성 신장 질환의 효과적인 관리는 심혈관 질환의 위험을 줄이고, 만성 신장 질환의 진행을 느리게 하기 위한 중요한 요소로 보고되었다. 그러나 최근 10년 간 한국의 대규모 자료를 이용한 연구는 부족한 실정이다. 따라서 이 연구에서는 한국의 대규모 코호트 자료인, 국민 건강 보험 서비스에서 제공하며 12년간 514,866명의 자격 정보, 병원 기록, 건강 검진 기록 등이 포함된 건강 검진 코호트 데이터베이스 (2002-2013)를 이용하여 분석해보고자 하였다. 이 연구에서는 먼저 2012-2013년 데이터를 이용하여 만성 질환의 단계에 따라 혈압 조절이 잘 되지 않는 군의 분포를 살펴보았다. 또한 Cox proportional regression with time dependent effects를 이용하여 혈압 조절이 모든 원인으로 인한 사망, 심혈관 질환으로 인한 사망, 그리고 말기 신질환에 미치는 영향을 분석하였다.;Chronic kidney disease (CKD) is a common long-term condition affecting 10 to 15% of the adult population and the prevalence of CKD is projected to increase worldwide. As CKD is a major risk factor for the end-stage renal disease (ESRD) and cardiovascular disease, effective management of CKD with strict blood pressure (BP) control is important to reduce cardiovascular risk and slow the progression of CKD. However, there is a lack of studies about this disease with the large Korean population data in the recent decades. Therefore, we analyzed health medical examination cohort database (2002-2013), provided by the Korean National Health Insurance Service, including qualification record, hospitalization record, health examination record of 514,866 participants during 12 years. In this study, we examined to investigate the prevalence of inadequate blood-pressure control in adults with CKD based on CKD stage. In addition, we analyzed the effect of inadequate blood-pressure on all causes of mortality, cardiovascular mortality and ESRD according to CKD and Non-CKD group through Cox proportional regression with time dependent effects.
