Abstract
자폐범주성장애(ASD) 아동의 사회기술에 대한 다각적인 이해 및 그에 따른 적절한 개입은 음악치료 분야에서 지속적인 관심의 대상이었다. ASD의 신경학적 기제 및 사회기술발달과 관련된 최근 연구 결과는 해당 대상군이 보이는 사회기술의 장애가 단지 사회적 정보의 지각이나 인지적 이해의 어려움에만 국한되지 않고, 운동적인 측면, 즉 지각된 감각운동적인 정보를 토대로 움직임을 구성하고 실행하는 데 있어서의 어려움에 기인함을 밝히고 있다. 더불어 사회적 행동 발달과 운동발달 간 연관성, 대인 간 공동 행동(joint action), 동기화(synchrony)가 상호 관계 및 관계에 대한 지각에 미치는 영향에 대한 보고는 사회기술의 인지적 측면과 운동적 측면의 연관성을 뒷받침한다. 이러한 사회기술발달과 관련된 관점의 변화로 인하여, ASD 대상군의 사회기술을 이해하는 데 있어 인지적인 측면을 주로 강조하던 경향에서 운동적인 측면으로까지 그 관심이 확장되고 있다. 또한 감각운동적인 측면에 대한 개입이 ASD 아동의 사회기술향상을 위한 중재에 있어 필수적임이 제안되고 있다. 이러한 관점의 변화, 관련된 이론 및 임상 근거를 바탕으로 본 연구에서는 ASD 아동의 사회기술 향상을 위한 리듬 적용 중재 프로토콜을 개발하고 그 적용 가능성 및 효과를 확인하고자 하였다. 이를 위하여 총 2개의 세부 연구를 진행함으로써, 세부 연구 1에서는 중재 프로토콜을 구성하고 세부 연구 2에서는 구성된 중재 프로토콜의 단기간 효과와 실행 가능성을 검증하고자 하였다. 먼저 세부 연구 1은 2개의 단계로 진행되었다. 1단계에서는 리듬 연주가 사회기술로써 이해되고 또한 리듬 적용 중재 프로토콜에 활용될 수 있는 가능성을 확인하고자, 일반 아동과 ASD 아동을 대상으로 사회기술 지표 및 상호 간 리듬 연주 시 타이밍 지표(즉, 동기화 오류)를 측정하고, 측정된 지표에 영향을 미치는 잠재적 요인을 분석하였다. 이렇게 밝혀진 이론적 근거와 리듬 연주와 사회기술과의 연관성을 바탕으로 2단계에서는 ASD 아동 대상 사회기술향상을 위한 리듬 적용 중재 프로토콜을 구성하고, 프로토콜의 내용 타당도 및 적용 가능성을 평가하였다. 1단계에서는 일반 아동 42명과 ASD 아동 10명을 대상으로 기초 연구를 실시하였다. 각 참여자들에게 교사 평정 사회기술척도(K-SSRS)와 모방 과제, 얼굴 표정 확인 과제가 실시되었다. 상호 간 리듬 연주 과제에는 총 3가지의 과제가 포함되었는데, 제공되는 리드믹 큐에 맞추어 연주하기, 타인(즉, 연구자)의 리듬 연주 타이밍에 맞추어 연주하기, 리드믹 큐가 제공된 상태에서 타인의 리듬 연주 타이밍에 맞추어 연주하기가 그것이다. 또한 각 연주 과제 시에는 리드믹 큐가 100bpm, 느린 템포(60-80bpm), 빠른 템포(120-140bpm)의 세 가지 조건으로 제공되었다. 각 대상군의 사회기술지표와 상호 간 리듬 연주 시 동기화 오류 지표를 측정하여 그룹 간 차이를 분석한 결과, ASD 그룹은 실시된 사회기술지표 세 가지 모두에서 일반 아동 그룹에 비해 유의하게 낮은 점수를 보였다. 또한 상호 간 리듬 연주 시 타이밍 지표와 관련하여, ASD 그룹은 리드믹 큐가 제공되는 경우에 그렇지 않은 경우보다 동기화 오류가 유의하게 감소하는 추이를 보였고, 느린 속도로 리드믹 큐가 제공되는 경우 100bpm 혹은 빠른 속도 조건에서보다 동기화 오류가 유의하게 증가하는 것으로 나타났다. 또한 동기화 오류에 있어서의 그룹 간 차이를 분석한 결과, 연주 과제 유형에 따른 그룹 간 차이는 유의하지 않은 반면, 리드믹 큐의 속도 조건에 따른 차이는 통계적으로 유의한 것으로 나타났다. 또한 리듬 연주를 사회기술 향상을 위한 중재 프로토콜에 적용 가능한지를 알아보기 위해 측정된 지표를 바탕으로 탐색적 요인 분석이 실시되었다. 분석 결과, 일반 아동 그룹의 경우에는 총 세 가지의 잠재적 요인, 즉 체화된 관계성(Embodied Intersubjectivity), 운동 표상(Motor Representation), 예측적 운동 조절(Anticipatory Control)이 도출되었다. ASD 그룹의 경우에는 일반 아동 그룹과 공통적인 세 가지 요인 외에 자기 조절(Self-Regulation)이 추가적으로 도출되었다. 이는 ASD 그룹의 경우, 사회기술발달에 있어 자기 조절이 다른 사회기술 요인과 독립적으로 작용하는 것을 시사하고, 이와 더불어 관련 중재에 있어 사회적 맥락에서 자신의 행동을 조절하는 기술이 선행적으로 개입될 필요가 있는 것으로 사료된다. 요인분석 결과와 관련하여 그룹 간 차이는 흥미로운 결과를 보여주는데, 일반 아동 그룹의 경우, 사회기술지표와 동일한 요인의 영향을 받는 것이 100bpm 조건에서의 타인과 함께 연주하기였던 데 반해, ASD 그룹의 경우, 사회기술지표와 연관된 타이밍 지표가 모두 리드믹 큐가 제공된 상태에서의 리듬 연주하기였고 속도 조건과 관련하여서는 느린 속도에서 리드믹 큐가 제공되는 경우의 리듬 연주하기가 사회기술지표와 동일한 기제로 작용되는 것으로 나타났다. 이러한 결과는 ASD 아동의 경우, 리드믹 큐가 제공되는 경우 사회적 정보를 처리하고 지각된 정보에 따라 운동을 조절하는 기술이 관여될 수 있으며, 특히 느린 간격의 타이밍에 맞추는 시도가 사회적 기술과 연관되어 있음을 시사한다. 요약하자면, 1단계에서 확인된 결과는 상호 간 리듬 연주가 사회기술향상을 위한 리듬 적용 중재 프로토콜에 활용될 수 있다는 근거를 제시하며, 이와 더불어 구체적인 연주 조건과 리드믹 큐의 사용, 속도 조정에 대한 기초 자료를 제공해주었다. 세부 연구 1의 2단계에서는 선행 연구를 통하여 고찰된 이론적 근거와 리듬 적용 가능성과 관련하여 확인된 내용을 바탕으로 리듬 적용 프로토콜이 구성되었다. 개발된 중재 프로토콜의 구성 요소는 참여(Engagement), 대인 간 협응(Interpersonal coordination), 적응적 조정(Adaptive adjustment)로 각 요소에 개입하는 과정이 단계적으로 이루어지도록 구성되었다. Engagement의 단계에서는 ASD 아동이 자발적으로 악기 및 환경을 탐색하는 기회가 제공되고, 이 때 파악된 움직임의 속도와 강도를 반영하여 규칙적인 박 패턴의 리듬 구조가 제공된다. 이후 타인, 사물(악기) 및 환경에 대한 인식이 확인되면 Interpersonal coordination의 단계로 넘어가게 된다. 이 단계에서는 아동의 움직임 속도에 근거하여 설정된 빠르기로 리드믹 큐가 제공된 상태에서 동기화된 움직임, 즉 아동과 타인(치료사)이 동일한 속도로 동일한 움직임을 지속하는 과정이 유도된다. 마지막 단계는 Adaptive adjustment로 이 단계에서는 속도의 조정, 움직임의 추가 혹은 조합 등이 시도되면서 아동이 지속적으로 변화하는 타인의 운동 정보에 따라 자신의 움직임을 조정하도록 유도된다. 이 때 속도 조정은 2단계 결과를 바탕으로 100bpm 혹은 그보다 빠른 속도에서 시작해, 이후 느린 속도로 조정되도록 제시되었다. 개발된 프로토콜은 총 6명의 음악치료 전문가에 의해 그 타당도가 평가되었다. 그 결과, 해당 프로토콜은 그 목표의 적절성, 과정의 적합성, 기대 효과의 중요성, 적용 가능성의 측면에서 모두 적합한 수준인 것으로 평가되었다. 특히 적용 가능성의 점수가 가장 높은 것으로 나타나 음악치료 임상 현장에서 유용하게 적용될 수 있는 장점이 있음이 시사되었다. 세부 연구 2는 개발된 프로토콜을 실행하고, 그 효과성 및 실행 가능성(feasibility)을 확인하는 연구로, 자폐범주성장애로 진단받은 총 8명의 아동이 참여하였다. 각 아동은 8주 동안 주 1회, 30분씩, 총 8회의 개별 세션에 참여하였다. 각 세션은 개발된 프로토콜에서 제시된 단계와 절차, 전략을 따라 연구자에 의해 진행되었다. 프로토콜 참여 전후에는 사회기술평정척도와 모방 과제 수행력, 상호 간 악기 연주 과제 시 동기화 오류 정도가 측정되었고, 세션 내에서의 행동 변화를 관찰하기 위해 각 2, 4, 6, 8회기 시에 녹화된 영상에서 약 3분 정도를 임의적으로 선택하여 해당 구간에서의 목표 행동을 관찰, 분석하였다. 목표 행동은 눈맞춤, 공동 행동에의 참여 및 타인과의 동기화된 움직임 시도로 설정되었고, 각 행동에 대한 조작적 정의와 관련된 사전 훈련을 마친 음악치료사 전문가와 음악치료 전공 석사 학생에 의해 그 행동의 발생 비율이 분석되었다. 연구 결과, 리듬 적용 중재 프로토콜의 참여한 ASD 아동은 프로토콜 참여 후, 사회기술지표에서의 향상이 있는 것으로 나타났다. 사회기술평정척도의 경우, 하위 항목 중 자기 조절 영역에서 가장 큰 향상이 나타났는데 이는 협력이나 자기 주장과 관련된 기술은 세션 내에서 직접적으로 목표된 것이 아니라는 점을 고려할 때, 중재를 통해 직접적으로 목표된 행동에서의 변화가 가장 현저하게 나타난다는 선행 연구를 뒷받침하는 것으로 사료된다. 상호 간 악기 연주 과제 시 동기화 오류 정도 변화의 경우, 조정된 속도 조건에서의 변화가 큰 것으로 나타났다. 이러한 결과는 보통 빠르기의 속도(예를 들어, 100bpm)에서의 운동 조절은 보다 자동적인 운동으로 처리되는 반면, 자신의 운동 속도보다 빠르거나 느린 속도에 맞추는 시도에서는 보다 많은 주의가 요구되고 타인에 반응하여 조절하는 기술이 개입된다는 선행 연구와 맥락을 같이하는 것으로 보이며, 해당 프로토콜이 그러한 사회기술에 효과적으로 개입할 수 있음을 시사한다. 세션 내에서 관찰된 행동변화와 관련하여서는 2회기와 4회기 간 변화의 폭이 현저하며, 그 이후에는 변화의 폭이 감소하는 흥미로운 결과가 나타났다. 이는 리듬 적용 프로토콜이 사회적 맥락에서 자신의 움직임을 조절하는 기술을 습득하거나 발달시키는 초기 과정에 효과적으로 개입할 수 있는 가능성을 시사하며, 동시에 보다 장기적인 효과 및 효과의 유지를 위해서는 장기간 중재 개입의 필요성을 보여준다. 이와 같이 총 2개의 세부 연구로 진행된 전체 연구의 결과, 리듬 적용 프로토콜이 ASD 아동의 사회기술향상을 위한 중재로써 적용 가능함을 확인하였고, 프로토콜의 적용이 해당 대상군이 사회적 맥락에서 운동을 조절하는 기술을 발달시키는 데 있어 긍정적인 결과로 이어질 수 있음을 보여주었다. 본 연구는 ASD 아동의 사회기술 발달을 이해하고 개입하는 데 있어, 기존 연구에서 초점을 두었던 사회인지적인 측면에서 그 초점을 사회운동적인 측면에까지 확장하는 새로운 패러다임을 적용하고, 리드믹 큐의 적용을 ASD 대상군에게까지 적용할 수 있다는 근거를 마련했다는 측면에서 그 의의가 있다고 볼 수 있다. 또한 프로토콜을 구성하고 제시하는 데 있어 목표의 개념화에서부터 중재 실행, 효과성 평가에 이르는 일련의 과정에서 이론적, 임상적 근거에 바탕을 두고 단계적 접근의 틀을 마련했다는 점에서 근거 중심의 임상 적용 방법과 그 효과성을 체계적으로 제시했다고 볼 수 있다. 또한 프로토콜을 통해 제시된 세부적인 개입 절차와 전략은 음악치료 전문가들이 해당 대상군과의 임상을 진행하는 데 있어 용이하게 활용할 수 있는 체계적인 자료를 제공함으로써 중재 기법으로써의 접근성, 반복적이고 체계적인 수준의 실행 가능성을 높였다고 평가된다. 본 연구를 통해 확인된 ASD 아동의 사회기술향상을 위한 리듬 적용 가능성 및 그 효과를 바탕으로 보다 장기간의 개입, 아동의 발달 수준 차이에 따른 적용 수준, 연주 조건 및 이자 간 관계 유형 등의 측면에서 후속 연구가 진행된다면, 해당 대상군에게 보다 적합하고 효과적인 중재로써 수정 보완될 수 있을 것으로 사료된다.;Children with autism spectrum disorder (ASD) show atypical development or persistent deficits in social skills as one of the primary diagnostic features of this disorder. As such, social skills development of individuals with ASD is a main focus of intervention efforts. Current perspectives on the mediating factors behind social skills development in ASD emphasize the interplay between social cognitive and social motor functioning. Thereby, there are calls for interventions that target motor aspects when intervening in atypical social skills development with this population. Although social skills related outcomes are well documented in the music therapy literature, attempts to intervene in social skills focusing on motor dimensions in ASD are limited. Given that music therapy has still been included as an emerging intervention (National Autism Center, 2015), development of an evidence-based intervention that can be replicated and generalized across individuals and settings is also needed. Therefore, this study aimed to develop such an intervention protocol for improving the social skills of children with ASD based on evidenced use of rhythm for motor control of individuals with neurological disorders. This study was conducted with two sub-studies. Study 1 aimed to propose and develop a rhythm-mediated intervention protocol for improving the social skills of children with ASD, and Study 2 investigated the effectiveness and feasibility of the developed protocol. In Study 1, a stepwise approach to developing the protocol was adopted, including investigating the applicability of dyadic rhythm playing to the intervention protocol via factor analysis (Phase 1), and constructing the intervention protocol (Phase 2). In Study 2, the developed intervention protocol was implemented with children with ASD, and its effects and feasibility were evaluated. Results of Study 1 identified the applicability of dyadic rhythm playing to music therapy intervention for children with ASD. The results of factor analysis demonstrated that four generated factors (i.e., Embodied Intersubjectivity, Motor Representation, Anticipatory Control, and Self-Regulation) influenced dyadic rhythm playing of children with ASD. This indicates that the presence of cueing and the tempo adjustment contributed to mediation of social skills, supporting the use of dyadic rhythm playing with the provision of rhythmic cueing for a rhythm-mediated protocol. The constructed rhythm-mediated intervention protocol included three components: engagement, interpersonal coordination, and adaptive adjustment. A stepwise approach to rhythm mediation for social skills was also presented. In terms of population appropriateness, goal specificity, and applicability, the social validity of the constructed intervention protocol was rated as sufficient by credentialed music therapists. Study 2 demonstrated that children with ASD could improve their social skills after receiving the rhythm-mediated intervention. They showed significant changes in social skills measures. Synchronization errors when tapping to the timing of a partner's rhythm playing at slow and fast tempo tended to decrease after intervention. Behavioral changes observed during sessions demonstrated that rhythmic cueing effectively facilitated children with ASD to engage in the joint action and adapt their movements in time with another person, while future studies are needed to substantiate the maintenance of such initial changes. In sum, the results of this study support that rhythmic cueing can effectively mediate motor control by providing the anticipatory temporal referent not only in the motor dimension but also in the social dimension, thereby proposing that rhythm mediation can be an effective intervention for improving social skills of children with ASD. As an innovative approach, this rhythm-mediated intervention protocol sets forth a new paradigm for understanding and intervening with this population by facilitating motor control processes in a social context with the evidenced use of rhythmic cueing. In addition, the entire process of developing a music therapy intervention protocol from identifying the mechanism for behavioral change to implementing and evaluating the intervention protocol aligns with evidence-based practice, while bridging a newly evidenced theoretical framework with clinical implementation. Guidelines for the structured application of the rhythm-mediated intervention allow music therapy professionals to implement and replicate the procedures without advanced training. As an initial step in developing a new intervention protocol for children with ASD, this study corroborated that the developed intervention protocol is acceptable for this population. However, further studies with controlled designs and larger sample sizes are needed to generalize the intervention outcomes across the population. In addition, it is suggested that further studies control the level of social and motor development and the degree of disorder-specific behavioral symptoms and incorporate different levels of movements, cueing, and playing conditions.
