Abstract
Given potential of ritodrine to stimulate calcium channel on cardiac muscle cell, this study aimed to investigate relationship between CACNA1C gene mutation and adverse drug reaction of ritodrine. Genomic DNAs were extracted from 169 patients’ blood samples from Ewha Womans University Mokdong Hospital. Data were collected by reviewing the paper-based and electronic medical records. Five single nucleotide polymorphisms (SNPs) of CACNA1C which have more than 20% of minor allele frequency were selected (rs10774053, rs215994, rs215976, rs2239128, and rs2041135) and analyzed using the SNaPshot assays. A statistical methods were used to evaluate the effects of genetic factors on ritodrine-induced ADRs. Gestational age at the time of administration, weight, body mass index, cumulative dose, and total infusion time were significant as baseline characteristics. Multivariate analysis was carried out in three ways: adjusting BMI, weight, or excluding both. SNP rs10774053 and rs2239128 were statistically significant in all models and rs215976 was significant when controlling BMI. Variant allele carriers had higher risk of ADRs in rs10774053 (OR = 5.029, 95% CI 1.389~18.213) and rs215976 (OR = 2.019, 95% CI 1.003~4.067), while C allele carriers of rs2239128 had lower OR (OR = 0.288, 95% CI 0.096~0.864). Weight was also significant when adjusting with weight (OR = 0.953, 95% CI 0.910~0.997) and gestational age at the time of administration showed significance when excluding weight and BMI (OR=0.909, 95% CI 0.831~0.995). The PAR of rs10774053, rs215976, and rs2239128 were 77.65%, 31.74% and 21.76%, respectively. Our results suggested that CACNA1C gene mutation was associated with adverse drug reactions of ritodrine in Korean preterm labor patients.;Ritodrine은 임신 37주 이전에 조기분만의 가능성이 있는 산모에게 투여되는 자궁수축 억제제로 심장 관련 부작용이 발생하는 것으로 알려져 있다. 본 연구에서는 유전자의 단일염기다형성(Single nucleotide polymorphism, SNP)이 ritodrine의 부작용 발생에 미치는 영향을 알아보고자 하였다. 이대목동병원에서 조기 진통으로 ritodrine을 투약 받은 산모 169명을 대상으로 본 연구를 진행하였다. 전자의무기록(Electronic medical record, EMR)을 통해 연령, 투약 시 재태주수 등의 인구학적 자료를 수집하였고, 산모들의 혈액에서 추출한 DNA로 총 5종의 CACNA1C 유전자 단일염기다형성(rs10774053, rs215994, rs215976, rs2239128, and rs2041135)을 분석하였다. 산모의 단일염기다형성 및 인구학적 특성이 ritodrine의 부작용 발생에 미치는 영향을 알아보기 위하여 카이제곱검정, t-검정을 사용하였으며, 단변량 분석 결과 p 값이 0.2 미만인 변수들을 대상으로 로지스틱 회귀분석을 실시하였다. 단변량 분석 결과 인구학적 요인 중 약물 투여 시 재태주수, 몸무게, 신체질량지수(Body mass index, BMI), 누적 용량, 총 사용 시간이 부작용이 발생한 군과 발생하지 않은 군 사이에 통계학적으로 유의한 차이를 보였다(p<0.05). 다변량 분석 결과 rs10774053, rs2239128이 부작용 발생에 유의하게 영향을 미쳤으며 BMI로 보정하는 경우 rs215976도 유의한 연관성이 있음을 확인하였다. rs10774053의 G 대립유전자(OR = 5.029, 95% CI 1.389~18.213), rs215976의 T 대립유전자(OR = 2.019, 95% CI 1.003~4.067)를 가진 환자들은 그렇지 않은 환자들에 비해 부작용 위험이 높았으며, rs2239128의 C 대립유전자를 가진 환자들은 TT 유전형을 가진 환자들에 비해 부작용 위험이 낮았다(OR = 0.288, 95% CI 0.096~0.864). 몸무게로 보정하는 경우 몸무게도 부작용 발생에 대한 통계적 유의성을 보였고(OR = 0.953, 95% CI 0.910~0.997), 몸무게와 BMI를 제외하고 다변량 분석을 시행한 경우 약물 투여 시 재태주수가 부작용 발생에 영향을 미치는 것으로 드러났다(OR=0.909, 95% CI 0.831~0.995). 인구기인위험도(Population attributable risk, PAR)는 rs10774053, rs215976, rs2239128 각각 77.65%, 31.74%, 21.76%로 계산되었다. 본 연구 결과, 한국인 조기진통 산모의 CACNA1C 유전자 변이가 ritodrine에 의한 부작용 발생과 연관성이 있음이 확인되었다. 이 연구의 결과는 ritodrine의 유해반응을 예측하여 조기진통 환자의 개인 맞춤형 치료를 개발하는 데 기여할 것으로 기대된다.
