Abstract
본 연구는 파킨슨병 환자의 음성 문제 개선에 초점을 맞춘 합창 중재 프로그램을 실시하여 객관적, 주관적 음성 평가를 측정한 실험연구이다. 대상자 모집은 자발적 참여 의사를 밝힌 실험군 6명, 통제군 5명을 모집하였다. 대상자들은 사전에 컴퓨터 난수표로 결정된 순서에 따라 두 그룹으로 배정하였으며 중재 기간은 2016년 10월 21일부터 2016년 12월 2일까지 50분, 총 8회기 중재를 진행하였다. 합창 중재 프로그램의 효과를 알아보기 위해 음성분석 프로그램을 통해 최대발성시간(MPT), 강도(Intensity), 기본 주파수(Fo)를 측정, 설문지를 통한 음성장애지수(VHI), 음성과 관련된 삶의 질 평가(V-RQOL)를 통하여 주관적 음성 문제를 측정하였다. 합창 중재 프로그램은 대화하기, 상체와 호흡근 이완 운동, 호흡 및 발성 운동, 합창하기, 대화하기 5단계로 구성되었고 프로그램을 통해 호흡의 길이, 음성 강도, 음고의 변화를 조절하며 향상시킬 수 있도록 진행되었다. 합창 중재에 참여한 결과 실험군은 음향학적 음성의 질, 주관적 음성장애지수, 음성과 관련된 삶의 질 평가에서 모두 긍정적인 변화를 보였다. 최대발성지속시간(MPT), 기본 주파수(Fo)에서 유의한 결과를 보였으며 특히 MPT, 기본 주파수에서 유의한 향상을 보였다. 사전-사후 평가로 실시된 음성장애지수, 음성과 관련된 삶의 질 평가 점수에서도 유의한 차이를 나타냈으며 각 항목별에서 점수가 향상되었음을 알 수 있었다. 통제군의 사전-사후 평가 결과 음향학적 음성의 질, 음성장애지수, 음성과 관련된 삶의 질 측면에서 유의한 향상을 보이지 않았다. 본 연구는 파킨슨병 환자의 음성 문제를 위한 합창 중재 프로그램이 환자의 음성 기능과 함께 음성과 관련된 삶의 질에도 긍정적인 영향을 미쳤음을 알 수 있었다. 또한 파킨슨병 환자의 음성 개선을 위한 음악 중재 프로그램 개발을 위한 자료와 근거를 제공할 것에 의의가 있다.;The purpose of the study was to conduct a choral singing intervention program focused on improving voice problems of Parkinson's disease patients and examine its effects on their vocal functions. 6 participants were recruited for an experimental group and 5 for a control group saying their spontaneous intention for participation and for 6 weeks 8 interventions were conducted for 50 minutes each. In order to identify effects of the choral singing intervention program, pre-post subjective/objective voice measurements were done MPT, Intensity, Fo were measured by Praat, and both VHI and V-RQOL were examined by a questionnaire. The chorus intervention program was composed of 5 steps including a opening conversation, physical warm-up, breathing vocal exercise, choral singing and closing conversation. Research findings showed that experimental group had positive changes in MPT, Intensity, Pitch, VHI and V-RQOL in addition to significant effects in MPT and Pitch. It also showed significant difference in pre-post VHI and V-RQOL assessments. As a result of pre-post vocal measurements for the control group, it showed that there was no significant improvement in both objective and subjective evaluations. These findings reveal that chorus intervention program had positive effects on vocal improvement of PD patients. Besides, this study has significance in that it suggested data and basis for development of musical intervention programs here after.
