Abstract
뇌전증은 특별한 유도요인 없이 발작이 재발하는 만성 신경계 질환으로써, 뇌 신경세포들의 비정상적인 과흥분 및 동기화로 인해 발생된다. 치료를 위해서는 현재 항경련제를 이용한 약물치료와 발작이 시작되는 뇌의 특정 부위, 즉 병소를 제거하는 수술적 치료가 시행되고 있으나, 전체 환자의 약 30%는 여전히 난치성 뇌전증 환자로 분류되고 있으며, 현 치료 방식의 부작용 혹은 후유증 또한 무시할 수 없는 상황이다. 최근 들어, 뇌전증 병소에 전기 자극을 가함으로써 발작을 제어하는 뇌 심부 자극술이 뇌전증 환자들을 위한 획기적인 치료법으로 떠오르며 그 임상 효과가 보고되고는 있으나, 이는 임상적 경험으로 인해 개발된 치료법으로써, 아직 그 제어 효과에 대한 정확한 메커니즘이 밝혀지지 않아, 치료 효능을 최적화하기에는 한계가 있다. 따라서, 뇌 심부 자극술의 효과를 다양한 조건 상에서 시뮬레이션을 통해 미리 예측해볼 수 있는 컴퓨터 모델링 연구가 매우 중요한 실정이다. 본 논문에서는 인비트로 실험상에서 관찰되는 전기 자극으로 인한 발작 제어 효과를 재현하고, 그 효과에 영향을 미칠 수 있는 중요 요소들을 제안하는 통합적 신경망 모델을 제안하였다. 본 모델은 측두엽 뇌전증을 모사하기 위하여, 내후각피질을 포함한 해마 네트워크의 실제 해부학적 구조와 신호 전달 경로를 반영한 흥분성 및 억제성 신경세포로 이루어진 네 개의 서브 네트워크로 구성되었으며, 생물학적 메커니즘과 인비트로 실험 결과를 기반으로 개발되었다. 쥐의 뇌 절편을 이용한 인비트로 실험은 컴퓨터 모델링에 필요한 파라미터 및 주요 메커니즘을 제공함은 물론, 실험 결과와 시뮬레이션 결과와의 비교 분석을 통해 컴퓨터 모델의 유효성을 검증하는 역할 또한 수행하였다. 개발된 컴퓨터 모델은 우선적으로 인비트로 뇌전증 모델에서 관찰되는 서로 다른 세 종류의 발작파를 재현할 수 있었다. 이들은 모두 생물학적 메커니즘에 기반하여 재현되었으며, 각 발작파 사이의 변이를 포함하여 연속적으로 서로 다른 세 종류의 발작파를 재현하였다는 데 의미가 있다. 또한 시뮬레이션 결과는 신호 고유의 시간-주파수 특성 및 전파 특성 측면에서 실제 실험 결과와 유사하였다. 둘째로, 개발된 컴퓨터 모델은 전기 자극을 통한 국소적인 발작 제어 효과를 재현할 수 있었다. 이는 본 인비트로 실험에서 주로 관찰되었던 현상으로써, 자극 인가 위치 주변으로만 발작파 억제 현상이 두드러지게 나타나는 것을 의미한다. 또한 이 현상은 신경세포들의 세포 밖 포타슘 이온 누적에 따른 탈분극 억제 메커니즘을 기반으로 재현되었으며, 이는 본 인비트로 실험 결과로도 해석할 수 있는 뇌 심부 자극술의 효과를 설명하는 가장 유력한 메커니즘이다. 마지막으로, 개발된 컴퓨터 모델은 전기 자극으로 인한 발작파 억제 현상에 영향을 미칠 수 있는 주요 인자들을 제시하였다. 특히 본 모델은 발작파의 전파 메커니즘이 국소 자극으로 인해 완전한 발작파 억제가 이뤄지는 유효 영역의 크기를 결정할 수 있음을 시뮬레이션을 통해 예측하였다. 신경세포 사이의 신호전달을 모사하는 시냅스 효과와 전계 효과의 비중을 다르게 조절하였을 때, 유도된 발작파의 신호 특성은 유사하였음에도 불구하고 전기 자극 효과를 통한 발작파 억제 현상은 다르게 나타났다. 즉, 발작파 전파에 있어서, 전계 효과의 영향이 약한 경우에는 발작파 억제 현상이 국소적으로 나타난 반면, 전계 효과의 영향이 강한 경우에는 그 억제 현상이 좀 더 넓은 영역에 걸쳐 나타날 수 있었다. 이는 전계 효과의 영향이 강한 경우, 네트워크의 국소적인 변화가 전체 네트워크의 활성도에 영향을 미칠 수 있음을 시사한다. 또한 본 모델은 또 다른 시뮬레이션을 통해, 전기 자극의 효과는 발작파가 시작되는 병소 부위에 가해질 때 더욱 효과적일 수 있다는 점 또한 제시할 수 있었다. 본 연구는 뇌전증 치료를 위한 뇌 심부 자극술의 효과를 분석할 수 있는 시뮬레이션 툴로써의 신경망 모델의 가능성을 보여주었다. 특히 실험 상으로는 파악할 수 없는 주요 인자들을 시뮬레이션을 통해 예측, 제시함으로써, 컴퓨터 모델의 강점을 보여주었다. 후속 연구를 통하여, 본 컴퓨터 모델은 특정 환자에게 적합한 최적의 치료 조건을 제시하는 실용적 모델로써 확장될 수 있다.;Epilepsy, defined as repetitive abnormal seizures, is a chronic neurological disorder. Drug treatment and surgical resection of lesions are typically used for the treatment. However, 25–30% of patients still suffer from intractable epilepsy, and the side effects and aftereffects of the traditional methods cannot be ignored. Recently, deep brain stimulation (DBS), which is to control seizure activity by electrically stimulating the epileptic zone, has received attention as a new treatment for epilepsy. However, the exact relief mechanisms are still unclear and the method is being developed by relying on clinical experience, so that there is a limit to how much the effectiveness of the treatment can be enhanced. Thus, research on computational modeling is important because a computer model can predict the effectiveness of new therapies in multiple circumstances. In this dissertation, we have proposed a comprehensive computational model that describes the DBS effect for epileptic seizure suppression and predicts the important factors that could affect the suppression effect. According to the anatomical structure of the hippocampal formation, the model was composed of four sub-networks, which consisted of excitatory and inhibitory neurons, and well-known signaling pathways between them. The model was developed based on biological mechanisms and in-vitro experimental results. The in-vitro experiment using rat brain slices not only provided the parameters and mechanisms for modeling, but also verified the proposed model by comparing simulation results with experimental data. Preferentially, the proposed model was able to reproduce epileptiform activities observed in the in-vitro epilepsy model. In particular, the model replicated the three different epileptiform activities, namely fast and slow interictal discharge and ictal discharge, and the transitions between them continuously, based on previously reported biological mechanisms. The simulation results were similar to recording data, obtained from entorhinal cortex combined hippocampal slices treated with a convulsant drug, in terms of signal features and propagation characteristics. Secondly, the proposed model was able to replicate the local suppression effect of seizure activity owing to electrical stimulation that has been observed mainly in experimental studies. The model was based on the neuronal depolarization blockade mechanism owing to accumulation of extracellular potassium ions, which is one of the most convincing theories to explain the DBS effect. Furthermore, our in-vitro experimental results also supported the mechanism. Finally, the proposed model was able to predict the important factors that can affect the seizure suppression effect by stimulation. Especially, the model proposed that the seizure propagation mechanism would determine the size of the effective region where the seizure-like events (SLE) are completely suppressed by the local stimulation. When the weight of two signal transmission methods between neurons, synaptic and electrical field transmission, was adjusted differently, the SLEs with the same characteristics were generated. However, the suppression effect due to stimulation was different. The suppression effect occurred just locally when the influence of electrical field transmission was very weak compared to synaptic transmission, while the effect occurred more globally, over the entire network, when the electrical field effect was stronger. Moreover, the model also identified that applying stimulation to the focus region, the initiating region, would give more effective suppression. The research results show the promising potential of the computational model as a simulation tool to analyze DBS effect for epilepsy. Through future works, the computer model could be advanced to a practical model that provides the optimum treatment conditions for specific patients.
