뇌종양 수술 후 환자의 암 대처에 미치는 영향 요인

Abstract

The purpose of this study was to identify the factors that impact cancer coping for patients with brain tumor. Patients diagnosed with brain tumor within the past two years were recruited from one hospital in Seoul. Data were collected from October 12th 2016 to November 18th 2016. The structured Questionnaires included the Korean Cancer Coping Questionnaire (K-CCQ), the Mishel's Uncertainty in Illness Scale-Brain Tumor (MUIS-BT), and the Tae’s healthcare professionals’ support in social support scale. The data were analyzed using SPSS version 23.0. Specifically, descriptive statistics, t-test, ANOVA, Scheffe’s test, Pearson’s correlation coefficient, and multiple regression were used. The results from this study are as follows: 1. The average age of patients was 52.14±15.09 years. Of the patients, 57.7% were female, the average postoperative period was 7.75±6.6 months and the average time since brain tumor diagnosis was 11.48±7.60 months. 2. The mean score for cancer coping was 60.08±15.61, uncertainty was 105.71±20.66, healthcare professionals’ support was 23.01±6.26. Perceived economic status (F=16.373, p<.001), Chemotherapy status (F=3.274, p<.05), postoperative period (F=4.522, p<.05), perceived health status (F=17.610, p<.001), decreased motor grade (F=15.053, p<.001) and decreased memory ability (F=4.753, p=.01) had statistically significant impacts on cancer coping. 3. Positive correlations emerged on the total individual coping scale (r=.906, p<.001), interpersonal coping scale (r=.878, p<.001), healthcare professionals’ support (r=.636, p<.001). Negative correlation emerged for uncertainty(r=-.603, p<.001). 4. The multiple regression analysis showed statistically significant results for economic status (β=.133, p=.009), decreased motor grade (β=-.125, p=.014), and healthcare professionals’ support (β=.785, p=.000). Results from this study indicated that economic status, decreased motor grade, and healthcare professionals’support influenced cancer coping in brain tumor patients. Study results can be used to improve cancer coping of brain tumor patients. To improve cancer coping for brain tumor patients, using an integrative approach regarding economical problem, neurologic deficit, and healthcare professionals’ support.;본 연구는 뇌종양 환자의 암 대처에 영향을 미치는 요인을 파악하기 위해 시행된 서술적 조사연구이다. 연구대상자는 서울시 Y구 소재 1개 상급종합병원의 신경외과 외래에서 추적관찰 중인 뇌종양 진단 2년 이내의 환자 130명을 편의 표집하였다. 자료수집은 2016년 10월 12일부터 11월 18일까지 시행되었다. 자가보고식 설문지를 사용하여 암 대처는 Moorey, Frampton, & Greer (2003)가 개발하고 김종남 등(2004)이 번안 및 수정한 도구를 이용하여 측정하였고, 불확실성은 Mishel (1988)이 개발하고 Lin et al. (2012)이 수정·보완한 도구를 번안하여 측정하였고, 의료인 지지는 태영숙(1986)이 개발한 도구를 이용하여 측정하였다. 수집된 자료는 SPSS 23.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Scheffe’s test, Pearson’s correlation coefficient, Multiple regression으로 분석하였다. 본 연구의 주요 결과는 다음과 같다. 1. 대상자의 암 대처의 평균 점수는 60.08±15.61점이었고 대상자의 불확실성의 평균 점수는 105.71±20.66점이었고 의료인 지지의 평균 점수는 23.01±6.26점이었다. 2. 암 대처는 지각된 경제수준(F=16.373, p<.001), 항암요법(F=3.274, p=.041), 수술 후 경과 기간(F=4.522, p=.005), 주관적 건강상태(F=17.610, p<.001), 근력 저하(F=15.053, p<.001), 기억력 저하(F=4.753, p=.01)에 따라 유의한 차이가 있었다. 3. 암 대처는 의료인 지지(r=.636, p<.001)과 정적 상관관계를 나타냈고, 불확실성(r=-.603, p<.001)로 부적 상관관계를 보였다. 4. 본 연구의 다중 회귀모형은 암 대처에 대해 55.6%(수정된 결정계수에 의하면 50.2%)의 설명력을 보였다(F=10.270, p<.001). 변수들 중 의료인 지지(β=.358, p<.001), 지각된 경제수준(β=.293, p=.001), 근력저하(β=.-.203, p=.013) 순으로 암 대처에 영향을 미치는 것으로 나타났다. 이상의 연구 결과를 통해 뇌종양 환자의 지각된 경제수준, 근력 저하, 의료인 지지 정도가 암 대처에 영향을 미치는 요인이며, 그 중에서도 의료인 지지가 암 대처에 주요한 영향요인임이 보고되었다. 따라서 뇌종양 환자의 암 대처를 향상시키기 위한 간호중재 개발 시 경제적 문제, 근력 저하 여부 등을 통합적으로 고려한 환자맞춤형 접근이 필요하다. 특히 의료인의 전문 간호 중재를 통한 상담, 충분한 정보 제공, 교육프로그램 등과 같은 중재 개발, 가정간호, 정서적 지지 등 의료인 지지를 강화해야 하며, 각 분야의 전문 의료인이 다 학제간 치료를 제공할 수 있도록 간호사가 중재 역할을 잘 수행하면 뇌종양 환자의 암 대처 증진에 도움이 될 것이라고 사료된다.
