대장암 수술 시 림프절 병기 정확성 향상을 위한 수술자에 의한 림프절 구득에 대한 연구

Abstract

연구 배경 및 목적 대장암 환자에 있어 검체로부터 더 많은 림프절을 구득하는 것은 암의 병기 설정 및 수술 후 환자의 예후를 예측하는데 매우 중요하다. 한국에서 수술 후 검체로 부터의 림프절 구득은 병리과에서 시행되어 왔으나, 본 연구에서는 해부학적 구조 및 수술소견을 더 잘 파악하고 있는 집도의가 림프절 구득을 할 경우 더 높은 림프절 구득률을 보일 수 있을 것이라는 가정하에 연구를 진행하였다. 연구 방법 2015년 9월부터 2016년 5월까지 이대목동병원에서 대장암으로 수술을 시행 받은 65명의 환자를 대상으로 전향적 연구를 시행하였다. 이 기간동안 수술 집도의는 수술 직후 검체를 수술방에서 신중히 촉지하여 림프절을 구득하였으며, 얻어진 림프절을 Japanese classification에 따른 림프절 그룹에 따라 분리 수거하였다. 잔여 대장 및 대장 주위 조직은 별도로 수거하여 조직검사를 시행하였다. 대조군은 본 전향적 연구를 시행하기 전 대장암 수술을 받은 65명의 환자를 무작위로 추출하여 병리과에서 구득한 림프절 상황을 분리 기록하였으며 각각의 군을 비교분석 하였다. 연구 결과 두 군 간에 종양의 위치, 수술 방법, 원격 전이 등에 있어 유의한 차이는 없었다. 구득된 평균 림프절 수는 실험군(24.95±10.986)에서 대조군(18.85±7.559)에 비하여 유의하게 높았으나(p=0.004) 전이 림프절 수(2.25±4.131 vs 3.29±5.587, p=0.105), 림프절 병기에 있어서는 뚜렷한 차이는 보이지 않았다. 총림프절 수에 대한 양성 림프절 비율은 실험군(9%)에서 대조군(16%)에 비해 현저히 낮았다(p=0.003). 결론 수술 집도의에 의한 림프절 구득이 암의 병기 자체에는 뚜렷한 영향을 보이지 않았으나 림프절 구득률을 높임으로써 병기설정의 정확성을 높이는 데 도움이 되며, 추후 장기적인 추적관찰을 통하여 림프절 구득 수가 큰 환자의 예후를 확인하는 과정을 통해 임상적인 의의를 확인할 수 있기를 기대한다.;Objectives Obtaining the more lymph nodes from the operative specimen is critical in cancer staging and predicting patient’s prognosis. Lymph node retrieval from the operative specimen has been performed by pathologists in Korea. However, we suppose that the higher lymph node yield will be achieved if lymph node retrieval is performed by operators who have better knowledge about the anatomy and the operative findings. Methods Prospective study was performed in the 65 patients who underwent operation for the colorectal cancer from September 2015 to May 2016. During that period, Operators picked the lymph nodes from the specimen right after the operation through careful palpation. Then, the obtained lymph nodes were put into separate bags marked with numbers of lymph node groups according to the Japanese classification and remnant pericolic tissues were put into bag separatelly and sent to the pathologists for pathologic evaluation. The same number of 65 patients were selected as a control group who lymph nodes retrieval was performed by the pathologists. Results There was no significant difference in tumor location, operation type, presence of distant metastasis between two groups. The mean number of harvested lymph node was significantly higher in study group(24.95±10.986) than control group(18.85±7.559, p=0.004) but there was no difference in the number of malignant lymph node(2.25±4.131 vs 3.29±5.587, p=0.105) and N-stage, lymph node positivity. The mean ratio of malignant lymph node was significantly lower in the study group (9.0%) than control group (16%, p=0.003). Conclusion Although the lymph node retrieval by the surgeon didn’t effect to the cancer stage itself, it will contribute achieving the higher accuracy of cancer staging by improving the lymph node yield in the colorectal cancer patients.
