호스피스환자의 가족보호자가 지각하는 미충족 요구와 돌봄 부담감

Abstract

호스피스환자 가족보호자들은 말기 암환자를 돌보면서 가족보호자들은 여러 방면의 미충족 요구를 경험하고 돌봄 부담감으로 고통 받고 있다. 그러나 환자 중심의 병원 환경과 돌봄 문화라는 국내 입원환경의 특수성에서 가족보호자에 대한 관리는 소홀하였다. 따라서 본 연구는 호스피스환자 가족보호자의 미충족 요구와 돌봄 부담감을 파악하고 이들 간의 관계를 확인하기 위한 서술적 상관관계 연구이다. 연구대상자는 서울, 경기 소재 의료원 산하 4개의 호스피스 완화의료 전문기관에 입원하고 있는 환자의 가족보호자 101명을 편의표집하였다. 자료수집은 2015년 9월 2일부터 2016년 3월 25일까지 시행하였다. 연구 도구는 Comprehensive needs assessment tool for cancer-caregivers (CNAT-C)와 돌봄 부담감 도구를 입원한 호스피스환자 가족보호자에 맞게 수정하여 이용하였다. 수집된 자료는 SPSS WIN 21.0을 사용하여, 기술통계, t-test, ANOVA, Tukey HSD 사후검증, Pearson’s Correlation Coefficients을 이용하여 분석하였다. 연구 결과는 다음과 같다. 1. 대상자의 미충족 요구 평균은 64.77±25.19점(가능한 점수의 범위: 0~120점)이었다. 미충족 요구의 하부영역은 의료진 관련 요구(2.00±.87), 정보∙교육적 요구(1.80±.78), 병원시설∙서비스 요구(1.68±.79), 사회적지지 요구(1.61±.72), 건강관리∙심리적 요구(1.32±.85), 가족∙사회적 관계 요구(1.23±.83), 종교∙영적 요구(1.05±.86)순이었다. 대상자의 돌봄 부담감 평균은 74.51±13.65점(가능한 점수의 범위: 25~125점)이였다. 2. 미충족 요구는 월 평균 의료비에 따라 300만원 이상인 그룹이 300만원 미만인 그룹보다 높았다. 돌봄 부담감은 다른 돌봄 제공자가 없을 경우 돌봄 부담감이 높았다. 3. 미충족 요구와 돌봄 부담감은 정적 상관관계가 있는 것으로 나타났다. 미충족 요구의 하부영역 중 건강관리∙심리적 요구, 가족∙사회적 관계 요구, 정보∙교육적 요구, 종교∙영적 요구, 사회적지지 요구는 돌봄 부담감과 통계적으로 유의한 정적 상관관계가 있었다. 본 연구결과를 통해 호스피스환자 가족보호자의 미충족 요구가 높을수록 돌봄 부담감이 높았고 미충족 요구의 일부 하부영역과 돌봄 부담감 간에 정적 상관관계가 있음을 확인하였다. 따라서 호스피스환자 가족보호자가 경험하는 미충족 요구와 돌봄 부담감을 경감시키기 위해 돌봄 부담감과 높은 정적 상관관계를 보였던 건강관리∙심리적 요구, 가족∙사회적 관계 요구, 정보∙교육적 요구를 충족시킬 수 있는 중재 프로그램 개발이 필요하다고 사료된다.;The purpose of the study was to investigate the correlation of unmet needs and caregiver burden of the family caregivers with hospice patients. The participants consisted of 101 family caregivers of terminal cancer patients residing in four hospices from 4 medical center in Seoul and Gyeonggi-do. The data was collected from September 2nd 2015 to March 25th 2016. The structured Questionnaires included Comprehensive Needs Assessment Tool for Cancer Caregivers (CNAT-C), a caregiver burden assessment tool. CNAT-C was modified to fit the research purpose to be suitable for use with family members of hospice patients. The collected data were analyzed using SPSS WIN 21.0. Specifically, descriptive statistics, t-test, ANOVA, Tukey’s post-hoc test, and Pearson’s correlation coefficients were used. The major findings of this study are as follows : 1. The study participants’ mean score for unmet needs was 64.77±25.19, the mean score for caregiver burden was 74.51±13.65. Of the mean score of the sub-categories in the unmet needs, the needs for the health care staff was the highest (2.00±.87), followed by information needs (1.80±.78), hospital facilities and services (1.68±.79), practical support (1.61±.72), health care and psychological needs (1.32±.85), family and social relationship needs (1.23±.83), and religious and spiritual needs (1.05±.86). 2. For unmet needs differed significantly for average monthly medical costs (F = 9.834, p < .001). For caregiver burden, there were statistically significant differences depending on whether there were other caregivers (t = –3.277, p < .001). 3. Unmet needs and caregiver burden were found to be positively correlated (r = .414, p < .001) Among these sub-categories of unmet needs, heath care and psychological needs (r=.515, p<.001), family and social relationship needs (r=.539, p<.001), information needs (r=.269, p=.007), religious and spiritual needs (r=.217, p=.029), and practical support needs (r=.466, p<.001) had a statistically significant static correlation with the burden of caregiver The findings indicate that there is a positive correlation between unmet needs and caregiver burden for family members of hospice patients. Therefore, the unmet needs and caregiver burden of family members functioning in the hospital environment should be assessed, and they should be provided with customized nursing intervention.
