고관절 수술을 받은 노인 환자를 돌보는 가족의 사회적 지지와 가족기능 및 돌봄 부담감

Abstract

본 연구의 목적은 고관절 수술 후 노인환자를 돌보는 가족의 사회적 지지와 가족기능 및 돌봄 부담감 간의 관계를 확인하는데 있다. 연구대상자는 고관절 수술 3∼9개월 후 서울 H 병원 외래를 방문하는 노인환자 주 가족 돌봄자이다. 연구 참여에 동의한 100명을 대상으로 조사하였다. 사회적 지지는 홍주우(1999)에 의해 수정 보완한 사회적 지지 측정 도구를 사용하였다. 가족기능은 Olson 등(1985)에 의해 개발된 Family Adaptability 과 Cohesion Evaluation scale-III (FACES-III)를 바탕으로 김윤희(1990)가 번안 및 수정한 도구를 사용하였다. 돌봄 부담감은 서미혜와 오가실(1993)이 개발한 도구 돌봄 부담감 도구를 사용하였다. 수집된 자료는 SPSS WIN 21.0 Program을 이용하여 각 변수의 실수와 백분율, 평균과 표준편차를 산출하였다. 대상자의 일반적 특성에 따른 사회적 지지, 가족기능, 돌봄 부담감은 t-test, one-way ANOVA로 분석하였다. 사후 검정은 Tukey HSD로 분석하였다. 연구대상자의 사회적 지지, 가족기능, 돌봄 부담감 간의 관계는 Pearson’s correlation으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 연구대상자의 평균 연령은 50.43(±14.50)세였으며, 여성이 전체의 68.0%를 차지하였다. 대상자의 46.0%가 하루에 6시간미만으로 노인을 돌보고 있었다. 2. 사회적 지지는 평균 88.30(±14.83)점(가능한 점수의 범위: 25-125점)이었다. 사회적 지지의 하위영역 중 물질적 지지가 가장 높았으며, 정서적 지지가 가장 낮게 나타났다. 가족기능의 점수는 평균 70.44(±10.31)점(가능한 점수의 범위: 20-100점)이었으며, 돌봄 부담감 점수는 평균 81.06(±14.49)점(가능한 점수의 범위: 25-125점)이었다. 돌봄 부담감의 하위영역 중 사회적 부담감이 가장 높았으며, 정서적 부담감이 가장 낮았다. 3. 사회적 지지는 대상자의 일반적 특성에 중 치료비의 경제적 부담정도(F=4.993, p=.003)와 퇴원교육 여부(F=5.782, p=.019), 가족기능은 돌봄 시간(F=4.224, p=.017)에 따라 유의한 차이를 보였다. 돌봄 부담감은 연령(F=6.504, p<.001), 최종학력(F=6.804, p<.001), 직업유무(F=-2.776, p=.007), 치료비의 경제적 부담정도(F=17.060, p<.001), 환자와의 관계(F=4.885, p=.001), 동거여부(F=3.649, p<.001) 및 돌봄 시간(F=15.515, p<.001)과 대상자가 느끼는 주관적 건강상태(F=10.246, p<.001)에 따라 유의한 차이가 있는 것으로 나타났다. 4. 사회적 지지와 가족기능은 통계적으로 유의한 정적상관관계(r=.598, p < .001)를 보였다. 이는 사회적 지지가 높을수록 가족기능이 높음을 의미한다. 돌봄 부담감 총점은 사회적 지지의 총점 및 가족기능의 총점과는 통계적으로 유의한 결과가 나오지 않았지만, 사회적 지지의 일부 하위영역과 가족기능의 일부 하위영역과는 통계적으로 유의한 역상관관계를 보였다. 본 연구결과 고관절 수술 환자를 돌보는 가족의 돌봄 부담감은 가족기능 및 사회적 지지와 유의한 상관관계가 있었다. 특히, 돌봄 부담감 중 사회적 부담감과 의존적 부담감은 사회적 지지의 모든 하위영역과 유의한 상관관계가 있었으며, 가족기능의 응집성과 유의한 상관관계가 있었다. 따라서, 고관절 수술 환자를 돌보는 가족의 돌봄 부담감을 감소시키기 위하여 정보제공, 물적, 인적 자원 제공, 관심과 격려 등 다양한 사회적 지지을 통해 돌봄자의 요구를 충족시키며, 돌봄 구성원간의 유대관계를 증진시킬 수 있는 간호중재 프로그램 개발이 필요하다. 개발된 간호 중재 프로그램을 보건소나 지역사회 복지관 및 온라인 등 다양한 환경에서 적용할 수 있도록 하여 돌봄으로 인한 부담감을 낮춰야 할 것이다.;The purpose of this descriptive research was to investigate the relationships among social support, family function and burden of family caregivers of elderly hip-surgery patients. Data were collected by conducting survey to 100 family caregivers of elderly hip-surgery patients, using the structured questionnaire, at the H hospital in seoul from November 11, 2015 to Febuary 1, 2016. Data were analyzed using SPSS 21.0. Descriptive statistics, independent samples t-test, one-way ANOVA and post-hoc analysis of Tukey HSD, Pearson correlation analysis were carried out. The results of the analysis are as follows. 1. The average age of participants was 50.43(±14.50) years old, and women comprised 68.0% of participants, 46.0 percent reported the average hours of care is less than 6 hours. 2. Average score of social support was 88.30 (±14.83) (possible range: 25-125). Among subcategories, The lowest perceived social support was emotional support. Mean scores for family function was 70.44 (±10.31) (possible range: 20-100). Mean scores of care burden was 81.06 (±14.49) (possible range: 25-125). The highest burden among the subcategories was ​​social burden followed by economic burden. 3. Social support showed meaningful difference depending on the degree of economic burden (F=4.993, p=.003) and the discharge education (F=5.782, p =.019) out of the general characteristics of the subjects, and care time (F=2.816, p= .043). For the care burden, age (F=6.504, p<. 001), educational background (F=6.804, p<. 001), occupation (F=-2.776, p=.007), relationships with patients (F=4.885, p=.001), co-residence (F=3.649, p<. 001), and care time (F=10.243, p<. 001), the economic burden of medical expenses (F=17.060, p=<. 001), subjective health condition (F=10.246, p<. 001) showed the significant difference accordingly. 4. Social support and family function showed a statistically significant positive correlation (r=.598, p<.001). This means that the higher social support increase the family functioning. The total score of social support and care burden score have no correlation statistically, but sub-factors of social support, the emotional support and total score of the burden of care (F=.- 219, p=.032) showed a meaningful negative correlation. According to the above research results, it seems to require an appropriate program to improve the well-being of patients and to reduce the care burden through identifying the relationship between social support of caregivers who takes care of the elderly hip-surgery patients and applying intervention program to improve family function and boost a program which can provide the social support.
