간호·간병통합서비스를 제공하는 간호사의 비판적 사고성향과 임상의사결정능력과의 관계 연구

Abstract

본 연구는 간호·간병통합서비스를 제공하는 간호사의 비판적 사고성향과 임상의사결정능력과의 관계를 파악하기 위한 서술적 상관관계 연구이다. 연구 대상자는 2013-2014년 시행된 정부의 간호·간병통합서비스 1차 또는 2차 시범사업에 참여한 후, 현재 시행 중인 지방 6개 의료기관의 간호·간병통합서비스 제공 병동에 근무하는 일반간호사로 본 연구에 동의한 112명이었다. 연구 도구는 구조화된 설문지로 비판적 사고성향은 윤진(2004)이 개발한 비판적 사고성향 측정도구를 사용하였으며, 임상의사결정능력은 Jenkins(1985)가 개발한 임상의사결정능력 척도(Clinical Decision Making in Nursing Scale, CDMNS)를 백미경(2004)이 번안한 도구를 사용하였다. 자료 수집은 2016년 4월 11일부터 5월 2일까지 실시하였다. 수집된 자료는 SPSS 23.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, One-way ANOVA, Pearson's correlation coefficients, 사후검정은 Scheffé test로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 연구대상자의 비판적 사고성향은 총점 평균 95.34±9.38점이었으며, 임상의사결정능력은 총점 평균 126.74±10.01점이었다. 2. 연구대상자의 일반적 특성에 따른 비판적 사고성향은 연령(F=7.962, p=.001)과 최종학력(t=-2.142, p=.034)에 따라 통계적으로 유의한 차이를 나타냈다. 사후검정 결과 40세 이상, 4년제 대학 졸업 이상인 대상자 군에서 비판적 사고성향이 더 높은 것으로 나타났다. 임상의사결정능력은 일반적 특성에 따른 유의한 차이를 나타내지 않았다. 3. 연구대상자의 비판적 사고성향과 임상의사결정능력은 유의한 정적 상관관계를 나타냈다(r=.452, p＜.001). 본 연구의 결과 간호·간병통합서비스를 제공하는 간호사의 비판적 사고성향과 임상의사결정능력은 유의한 정적 상관관계를 나타냈으므로 간호·간병통합서비스를 제공하는 간호사를 대상으로 교육프로그램을 적용할 때 이러한 변인을 고려해야 한다. ;The purpose of this study was to identify the relationship between critical thinking disposition and clinical decision-making ability of nurses who provide comprehensive nursing care service in korea. The sample of the study were 112 nurses, who work in the local hospital with providing comprehensive nursing care service. A structured questionnaire used in this study composed of 73 items: background characteristics, critical thinking disposition, clinical decision-making ability. The collected data were analyzed using the SPSS WIN 23.0 to calculate frequencies, percentage, mean, standard deviation and t-test, One-way ANOVA, Pearson's correlation coefficients. The Major findings of this study were as follows: 1. The critical thinking disposition showed the mean of 95.34±9.38, of the clinical decision-making showed the mean 126.74±10.01. 2. There were statistically significant differences in critical thinking disposition by age (F=7.962, p=.001) and education(t=-2.142, p=.034). In case of the clinical decision-making ability, there were not statistically significant differences in clinical decision-making ability by participant's background. 3. A significant positive correlation(r=.452, p＜.001) was found between critical thinking disposition and clinical decision-making abilities of nurses to provide comprehensive nursing care service. As a result of this study, it was reported that the positive relationship between the critical thinking disposition and clinical decision-making ability. To improve the critical thinking disposition and clinical decision-making ability of nurses to provide comprehensive nursing care service, training program is requested.
