보건소 금연클리닉의 효율성 평가를 위한 탐색적 연구

Abstract

본 연구는 서울시 25개구 보건소 금연 클리닉의 효율성을 다각적으로 평가한 후, 그 효율성과 관련이 있는 보건소 특성을 파악하는 것을 목적으로 하는 단면연구이다. 분석방법으로는 DEA를 이용하였으며, 효율성 평가의 투입요소로는 금연상담사수, 총비용을 사용하였고 산출변수로는 금연클리닉 등록자수, 4주 금연성공자수, 6개월 금연성공자수를 사용하였다. 그리고 보건소 금연클리닉의 효율성 관련요인을 파악하기 위해서, 효율적 보건소 금연클리닉과 비효율적 금연클리닉으로 세부집단을 나누어 투입· 산출요소, 상담특성, 인적특성, 재정특성이 집단 간 차이가 있는지를 파악하였다. 효율성 평가 결과는 비효율적인 보건소 금연클리닉 모두가 규모가 커질수록 규모수익이 감소하는 규모수익체감을 나타내는 것으로 나타나, 이는 금연클리닉의 규모를 축소함으로써 편익을 얻을 수 있음을 의미하는 것으로 나타났다. 효율성과 관련있는 요인은 순기술효율성의 경우 4주 금연성공자수와 6개월 금연성공자수가 비효율적인 군에서 통계적으로 유의하게 높았으며, 규모효율성의 경우 인력, 예산, 대면상담횟수, 기타홍보비가 비효율적인 군에서 높은 것으로 나타났으며 금연상담사의 직무교육이수율만 효율적인 군에서 통계적으로 유의하게 높은 것으로 나타났다. 금연상담사라고 하는 인력에 의해 대부분의 서비스가 제공되는 보건소 금연클리닉의 특성을 고려한다면, 효율성 관련요인 중에서 인적요인과 관련이 있는 직무교육수율은 중요한 의미를 가진다고 할 수 있다. 본 연구는 보건소 금연클리닉의 효율성을 DEA 기법을 활용하여 평가하고, 더 나아가 금연상담사의 인적 특성 등을 포함하여 효율성 영향요인을 분석하였다는 점에서 의의를 가진다.;The purpose of the study is to measure the relative efficiency of the Smoking-cessation Clinic in Public Health Center and identify factors which influence an efficiency. In efficiency determinants analysis index published by Public Health Center are used. The target year of investigation is 2015 in every data. For measuring efficiency of Smoking-cessation Clinic in Public Health Center, the Data Envelopment Analysis(DEA) is used. The expense of Smoking-cessation Clinic and manpower is input variables. we select the performance records of Smoking-cessation Clinic in Public Health Center. The major findings of this study are as follows. First of all, There are significant differences in the relative efficiency among the Smoking-cessation Clinic in Public Health Center. Second, through the analysis of factors which influence an efficiency, we can get a information that would be helpful for non best-practice Smoking-cessation Clinic in Public Health Center to become more efficient institution. The five variables are significantly effecting to the Smoking-cessation Clinic in Public Health Center efficiency and the list is as follows: the number of employees, The expense of health service, the number of Counseling Activities, Advertising Spending, Job Training completion degree. All variables has negative effects except Job Training completion degree.
