Abstract
Gastric cancer is a second leading cause of cancer death in the world. Particularly, this disease has become a top cause of cancer death in Asian countries such as Korea and Japan. According to 2015 American Cancer Society’s estimates for stomach cancer in US, about 24,590 cases of stomach cancers patients are diagnosed; 15,540 in men and 9,050 in women. Stomach cancer mostly affects older people and the average age of diagnosed people is 69. Among many pathogenetic factors of gastric cancers, overexpressed human epidermal growth factor receptor 2 (HER2) has been considered as one of the most important factors. About 1 out of 5 cases of stomach cancers is characterized to have abnormally overexpressed growth-promoting protein called HER2/neu or just HER2 on the surface of cancer cells. In detail, 27% of gastric cancer patients show HER2 gene amplification and 23% are HER2 protein overexpressed patients. Trastuzumab, a FDA-approved HER2-targeting antibody drug, has been used alone as a first-line targeted therapy or in combination with cisplatin and 5-FU to treat HER2-overexpressing gastric cancers. However, there are some challenges in the trastuzumab treatment such as severe resistance, cardiotoxicity, and no efficacy for brain metastases from gastric carcinoma because of impermeability to the blood-brain barrier due to its big size. Based on these reasons, our laboratory has studied to overcome limitations of trastuzumab on the development of small molecules which have a distinct mode of action from that of trastuzumab. I found HT38 (Dihydropyrazol Ethanone Derivative) through SEAP assay utilizing interaction of epithelial-restricted with serine box (ESX, transcription factor of HER2) with Sur2 (co-transcription factor of ESX). HT38 down-regulated HER2 amplification and HER2 expression. HT38 reduced cell proliferation and migration in NCI-N87 gastric cancer cells via induction of G1 arrest and inhibition of HER2-mediated downstream signals. HT38 induced finally apoptosis which was checked with vitality assay and caspase 3/7 assay. In order to confirm migration inhibitory activity of HT38, ESX Knock down (KD) NCI-N87 cells were prepared and compared with HT38-treated and -untreated NCI-N87 cells. HT38 inhibited cell migration activity as similar to ESX KD cells. This result tells us that ESX is an important marker of HER2-positve advanced gastric cancers that provokes metastasis to other organs. HT38 could effectively block this function of ESX. Trastuzumab-resistant NCI-N87 cells were developed by treatment of trastuzumab at low concentrations (0.05 μM until 10th passage and 0.1 μM from 11th to 20th passage) for 17 weeks and ESX, HER1, and HER2 were more expressed in resistant cells than in parent cells. It is well known that ESX can interact with HER1 and HER2 promoters in head and neck squamous cancer cells and this study firstly confirmed that in gastric cancer cells. Therefore, overexpression of HER1 and HER2 in resistant cells was apparent because of overexpressed ESX. Therefore, the strategy to target both of HER2 and ESX-Sur2 interaction would be beneficial to overcome trastuzumab, targeting overexpressed HER2 extracellular membrane domain. Co-treatment of HT38 with trastuzumab showed synergic effect in anti-proliferation of trastuzumab-resistant cells. HT38 would be a potent inhibitor of ESX-Sur2 interaction and a good combination partner of trastuzumab to overcome its resistance. ;위암은 전세계 암으로 인한 사망률 2위를 차지하는 질병이다. 특히 한국, 일본과 같은 아시아 국가에서는 암으로 인한 사망률 1위를 차지하는 질병이다. 2015 American Cancer Society에 따르면 미국에서 약 25,000명의 위암환자가 발생하고 그 가운데 약 15,000명은 남성, 약 9,000명은 여성일 것이라고 예측하였다. 위암은 평균 진단 나이가 69세이고, 대부분 노인이 위암에 높게 노출되어 있다. 위암을 일으키는 다양한 병리학적 요인 가운데 HER2 과발현이 중요한 요인 가운데 하나로 꼽히고 있다. 위암환자 5명 중 1명이 암세포 표면에 HER2라고 불리는 단백질을 비정상적으로 과발현하고 있다. 구체적으로, 27%의 위암환자가 HER2 유전자 과발현을 보이고 23%의 환자가 HER2 단백질 과발현을 보인다. 트라스트주맙은 HER2를 타깃으로 하며, FDA의 승인을 받은 항체제이다. 트라스트주맙은 타깃치료제로써 위암치료에 있어 단독으로 처방되기도 하고 cisplatin 혹은 5-FU와 병용처방 되기도 한다. 하지만 트라스트주맙은 내성, 심장독성, 분자량이 큰 항체제제의 특수성 때문에 뇌-혈관 장벽을 통과하지 못하므로 뇌로의 전이에 있어서는 효과가 없는 등 몇 가지 단점을 가지고 있다. 이러한 이유에 기초하여 이번 연구에서는 트라스트주맙과는 다른 기전을 가진 저분자약물을 이용하여 트라스트주맙의 제한을 극복하기 위한 연구를 진행하였다. 이번 연구에서는 HER2의 전사인자인 ESX와 ESX의 보조전사인자인 Sur2를 이용한 SEAP assay를 통해 HT38 (Dihydropyrazol Ethanone Derivative)을 선택하였다. HT38은 HER2 유전자 증폭과 HER2 단백질 발현을 저해하였다. 또한 HT38은 위암세포인 NCI-N87 세포에서 G1 arrest와 HER2-연관 하위신호전달계를 저해함으로써 세포증식과 세포이동을 저해하였다. HT38이 세포사멸을 어떤 방식으로 유도하는 지 확인하기 위해 vitality assay와 caspase3/7 assay를 실시하였다. HT38이 효과적으로 세포 이동성을 저해하는지 확인하기 위해 parent 세포에 HT38을 처리한 뒤 ESX knock down 세포와 비교하였다. ESX의 발현을 저해했을 때와 HT38을 처리하였을 때 모두 동일한 양상을 보였다. 이 결과는 ESX가 세포 이동성에서 중요한 의미를 가지며, HER2 과발현 위암세포에서의 전이에도 ESX가 중요한 역할을 할 것이라고 기대할 수 있었다. 최종적으로 HT38이 트라스트주맙-내성세포에서도 효과적인지 확인하기 위해, 17주에 걸쳐 트라스트주맙-내성세포를 만들었고, 웨스턴블랏을 통해 HER2-관련 하위신호전달계에 관여하는 단백질들의 변화를 확인하였다. 내성이 생기면서 ESX, HER1과 HER2가 증가하는 것을 확인하였다. ESX가 HER1과 HER2의 프로모터에 결합한다는 사실이 알려져 있으므로 HER1과 HER2의 과발현은 ESX의 증가 때문일 것이라 예상하였다. 트라스트주맙은 과발현 된 HER2를, HT38은 ESX-Sur2 상호작용을 저해하므로 트라스트주맙과 HT38을 함께 처리하는 것은 암세포 증식을 저해하는 데 효과적이었다. 따라서 이번 연구를 통해 HT38이 ESX-Su2 상호작용을 효과적으로 저해하고 트라스트주맙 내성을 극복하는 데에 있어 트라스트주맙의 병용 처리 약물로서 개발 가능함을 확인하였다.
