Abstract
본 연구는 생체부분 간이식 간 공여자와 간호사의 교육경험을 알아보고, 간 공여자의 교육요구도와 간호사가 지각하는 간 공여자에게 필요한 교육에 대한 차이를 파악하여 효과적이고 실제적인 교육을 제공하기 위한 기초자료를 마련하기 위한 것이다. 자료 수집은 2015년 9월 8일부터 2015년 11월11일까지 진행되었으며 연구 대상자는 서울 소재한 3차 종합병원의 수술 전 생체부분 간이식 간 공여자 50명과 간호사 100명 이었다. 연구도구는 간 공여자 교육요구도 측정 도구로 4개의 영역, 18개 문항과 교육경험에 대한 설문문항으로 구성되었다. 수집된 자료는 SPSS window 22.0 프로그램을 이용하여 빈도, 백분율, 평균 및 표준편차, t-test, ANOVA, Scheffe test 방법으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 간 공여자는 남자 33명(66.0%), 평균 연령은 29.8세, 미혼이 33명(66.0%)이었다. 대상자의 수혜자는‘부’28명(56.0%),‘모’9명(18.0%), 형제, 자매 3명(6.0%)등 으로 나타났으며, 간 공여자가 지각하는 수혜자와의 친밀도는 10점 만점에 32명(64.0%)이었으며, 공여 결정 동기는 자발적 결정이 46명(92.0%)으로 대다수였다. 또한 간 공여 결정시 어려웠던 점으로 수술에 대한 두려움과 수술 후 일상생활을 할 수 있는지가 불명확 하다는 응답이 많았다. 2. 간 공여자들의 47명(94.0%)이 간 공여자를 위한 교육을 경험하지 못했고, 42명(84.0%)이 교육의 필요성을 나타냈다. 주 정보 취득 방법은 인터넷이 28명(56.0%)으로 가장 높았고 희망하는 교육방법으로는 1:1면담, 책 또는 인쇄물, 강의, 비디오 및 동영상의 순 이었다. 도구로 측정된 간 공여자의 교육요구도는 72점 만점에 62.10±9.93이며, 문항평균 4점 만점에 3.37점 이었다. 영역별로 가장 요구도가 높았던 영역은 간 공여 수술 관련영역(3.63점)이었으며, 간 공여 후 일상생활 관리(3.30점), 간 공여 후 정서 관리(3.27점), 간 공여 후 사회생활(3.26점)의 순이었다. 18개 문항 중 가장 교육요구도가 높았던 항목은 간 공여 수술 후 관리(3.74±.53), 간이식 수혜자의 수술방법 및 성공률(3.74±.49), 간 공여 수술 후 합병증(3.70±.61)이었다. 일반적 특성에 따른 교육요구도 차이분석 결과 결혼상태(p=.031)와 경제적 상태(p=.041)가 통계적으로 유의한 차이를 보였다. 경제적 상태는‘어려운 편이다’라고 답한 대상자보다‘보통이다’‘비교적 잘사는 편이다’의 대상자의 교육요구도가 높았으며, 결혼상태는 기혼이 미혼보다 교육요구도가 높았다. 3. 간호사는 간 공여자의 수술 전 지식정도에 대해 5명(5.0%)만‘높다’라고 지각하였으며, 간 공여자와 간호사 대상 교육의 필요성에 대해 100명(100.0%)이 응답하여 교육 필요성이 매우 높음을 알 수 있었다. 간호사는 간 공여자를 대상으로 72명(72.0%)이 교육을 시행하였으나 이 중 61명(84.7%)은 경험을 통한 구두설명으로 교육을 제공하고 있다고 하여 교육 자료가 부족함을 알 수 있었다. 간 공여자의 교육 제공자로 간호사의 75명(75.0%)이 전문간호사를 희망하였다. 간호사가 지각하는 간 공여자에게 필요한 교육요구도는 64.64±8.86으로 문항평균 3.46이었다. 영역별로 교육요구도가 높았던 영역은 간 공여 수술 관련 영역(3.56점)이었다. 문항 중 가장 교육요구도가 높았던 항목은 간 공여 수술 후 관리(3.65±.56), 간 공여 수술 후 합병증(3.62±.53), 간 공여자의 수술 방법 및 성공률(3.61±.57)순 이었다. 4. 간 공여자와 간호사의 교육요구도의 영역별 비교 분석 결과 교육요구도에서 간 공여 후 사회생활 영역(p=.045)에서 통계적으로 유의한 차이가 있어 간호사가 지각하는 간 공여자에게 필요한 교육요구도가 간 공여자의 교육요구도보다 높게 나타났다. 문항별 교육요구도의 차이는 간이식 수혜자의 수술 방법 및 성공률 (p=.023), 간이식 수혜자의 합병증(p=.030)의 문항이 통계적으로 유의하게 간 공여자의 교육요구도가 간호사보다 높았고, 간 공여 후 한약 섭취 (p=.006), 간 공여 후 성생활 시기(p=.002)의 문항에서 간호사의 교육요구도가 간 공여자 보다 통계적으로 유의하게 높은 것으로 나타났다. 이상의 결과를 통해 생체부분 간이식 간 공여자의 특성 및 경험이 자신의 질병 때문에 수술 하는 환자들과 다르다는 점을 이해해야 할 필요가 있으며 간 공여자와 가족 그리고 의료진 간의 원활한 의사소통 및 간 공여자에 대한 지지를 제공하는 프로그램 및 지원이 필요함을 알 수 있었다. 또한 간 공여자와 간호사 모두에게 교육과 자료의 불충분함을 알 수 있었다. 간 공여자들의 어려움과 혼란을 예방하기 위해 정확하고 신뢰성 있으며 쉽게 접근 가능한 구조화된 교육 프로그램의 제공과 교육 자료의 개발이 필요하며 교육을 담당할 전문 인력이 필요하다. 또한 간 공여자의 결혼, 경제 상태에 따라 교육요구도를 다르게 지각한 것을 볼 때 간 공여자의 개별적인 특성과 상황이 고려된 교육 및 1:1 면담을 통한 관리가 필요하다. 그리고 질적으로 높은 간호를 제공하기 위해 담당 간호사 대상의 전문 교육이 필요하다.;This study finds training experience of liver donors and nurses, difference between training needs of liver donors and training needs that nurse recognize in liver transplantation for living donor to understand the different and provide effective preliminary data. The data has been collected from 8th of September 2015 to 11th of November 2015 and candidate subject were 50 liver donors and 100 nurses at tertiary general hospital located in Seoul. A research tools were 18 questions in 4 areas that made to measure training needs. Collected data analyzed by SPSS window 22.0 with frequency, percentage, average, standard deviation, t-test, ANOVA and Scheffe test method. The result is as follow 1. Among liver donors, 33 of them were males (66.0%), the average age was 29.8, and 33 of them were singles (66.0%). Candidate subject’s beneficiaries were 28 fathers (56.0%), 9 mothers (18.0%), 3 brothers and sisters (6.0%) and etc. In addition, on the question of motivation, majority of 46 donors (92.0%) were voluntary made their choice. Many donors also answered that fear and uncertainty of returning back to normal life gave difficulties on their decision making for donation. 2. Among the liver donors 47(94.0%) of them were inexperienced with liver transplant training and 42(84.0%) of them expressed the need for training highest information acquisition method was internet 28 donors (56.0%). And for hoping methods of training were in order of 1:1 meeting, book or print out, lecture and video in order. Tool surveyed training needs were 62.10±9.93 out of 72 points and 3.37 points out of 4 points on Question average. Transplantation surgery related area (3.63 ) had the highest needs and manage to return back to normal life after donation (3.30), managing emotion after donation (3.27), Social life after donation (3.26) were the rest in order. Among the 18 questions, the higher the training needs items were in order of managing after surgery, surgery of liver transplantation recipient and success rate, complication after surgery. Difference analysis result of Training needs in accordance with the general characteristic shows married statute (p=.031) and economic statue (p=.041) had statically significant difference. 3. Only 5 nurses (5.0%) were recognized ‘high’ on the question of liver transplantation knowledge before surgery and 100 nurses (100.0%) answered for the needs of liver transplantation training which shows training needs is very high. 72 nurses (72.0%) commended training on liver donors but 61 nurses (84.7%) provided oral explanation base on their experience and training material was insufficient. 75 nurses (75.0%) hoped to have specialized nurse for training provider. Nurses recognize training needs for liver donor were 64.64±8.86 and 3.46 in question average. 4. The required amount of education level for the liver donors and nurses was compared and analyzed in each field. As a consequence, there were statistically visible differences among the donors in their social life(p=.045) after donation, but the average level of education that nurses recognize appeared to be higher than that of the required level. The difference showing that the required amount of education for donors was visibly higher than that of nurses in statistics. The differences in required amount of education were shown in 'the method and success rate of liver transplant operation (p=.023) and complication rate of donors (p=.030), which indicates that the required amount of education for donors were consistently higher than that of nurses. Also, in sections such as drug consumption after liver donation (p=.006) and 'frequency of sexual life after liver donation (p=.002), the required amount of education for nurses seemed significantly higher than that of liver donors. By examining those results, it has become clear that the experiences and characteristics of donors in liver transplantation were different from the patients who had surgery because of their diseases. In addition, it was discovered that more programs need to be constructed to offer aids to donors and foster better communications between medical teams and families of liver donor. And last, the amount of educations and materials provided for both donors and nurses were inadequate. More accurate, reliable, and easily accessible education programs and materials should be developed to reduce the difficulties and confusions that the donors may have, while recruiting more professionals to be in charge of education. The required amount of education was differently recognized depending on their marital and financial state of donors. Judging from that, more education and management with one-on-one talks according to their personality and circumstances are needed. Not to mention, professional training for the nurses in charge should go into effect in order to provide nursing in better quality.
