Abstract
The World Health Organization (WHO) defines health as "A state of complete physical, mental and social well-being and not merely the absence of disease or infirmity," pursuing whole-person health across all aspects of physical, mental and social well-being. With health issues being perceived as a social concern, healthcare has become an obligation that need be redeemed at a national or social level. Given the fact that adults' physical and mental health is under threat, the Ministry of Health and Welfare set up the National Health Plan from 2002 to 2010 in compliance with the Article 4-1 under the National Health Promotion Act, and opted for smoking cessation, temperance, physical activities and nutrition as four core healthy living practices, with local public healthcare centers taking macroscopic approaches involving education, promotion and legislation. As a result, smoking rates have reportedly decreased, whereas the prevalence of smoking in females and drinking rates are expected to keep growing. Also, nutrition and exercise projects are carried out via public healthcare centers nationwide, whereas national obesity rates are tipped to increase, which suggests the National Health Plan has not been quite successful. In this respect, previous studies pointed to a paucity of multidimensional understanding of targets, insufficient network systems and implementing agencies, issues attributable to priorities among projects and two-dimensional approaches centering on publicity and education. As reported previously, to illuminate why the healthy living practices have failed to permeate to the wider society, multi-dimensional understanding of subjects is needed. The present study presents the need to understand the mental health status of subjects focusing on depression which draws unprecedented attention in the field of mental health, sheds light on the effects of healthy living practices (smoking, drinking, physical activities and regular diet) on depression in adults aged 19 to 64, and elucidates some implications for viable healthy living practices. The present findings delineated below are based on the complex sample logistic regression analysis of the raw data from the third round of the National Health and Nutrition Examination Survey for the 5th Period First, according to the analysis of the relevance between healthy living practices and the prevalence of depression, all the variables of healthy living practices ('smoking status', 'frequency of drinking over the past year', 'medium-level physical activities over the past week' and 'the total number of regular meals') proved statistically significant. Second, as for the effects of healthy living practices on the prevalence of depression, those who are females, economically inactive, subjectively unwell and smokers are highly likely to feel depressed. Among the variables of healthy living practices, only smoking proved to be statistically significant, which can be explained by the predominant effects of actual subjective health status, leading to the other variables being less influential or insignificant. In terms of gender, the odds of depression prevalence in women are 4.101 times higher than in men. The odds of depression prevalence in economically inactive subjects proved to increase by 1.568 times in comparison to economically active counterparts. As for subjective health status, the prevalence of depression grows as one feels unwell, where the odds proved to increase by 2.291 times. Among the healthy living practices, smoking was found to be significantly associated with the prevalence of depression. more smoking leads to higher risks for depression, where the odds increased by 1.168 times. From these findings, the following implications are elicited. First, given the finding that more smoking leads to a higher risk of depression, a coordinated collaborative system need be formulated between the smoking counseling centers in charge of helping people quit smoking and the local mental health promotion centers in charge of those who suffer from depression. Second, in light of the effects of subjective perception of health status on the prevalence of depression, those who feels unwell have a higher risk of depression. This may be accounted for by distorted thinking among other symptoms of depression. Thus, early diagnoses, intervention and follow-up care are needed for those who are at a high risk of depression. Third, significant effects of gender and economically inactive on the prevalence of depression are found. This finding suggests the need for multidimensional understanding of subjects. In other words, any provision of service for subjects in practice calls for multidimensional and multifaceted understanding of difficulties surrounding subjects. Furthermore, to apply such understanding consistently and effectively, it is necessary to take into account some standardized criteria mirroring the multidimensional understanding. Fourth, as suggested in previous and present studies, multidimensional factors surrounding subjects need be considered so as to encourage the healthy living practices. Multidimensional and multifaceted understanding of factors means responding to diverse needs of subjects, specifically by taking personalized approaches to those having difficulties independently coping with personal concerns on account of complex mental health issues like depression, and further by taking closely coordinated network approaches to whole-person health in tandem with multi-disciplinary specialists. Fifth, the present study shed no light on nicotine withdrawal symptoms as the factors causing depression associated with smoking, which warrants further studies that could present practical implications for coping as well as facilitate the multifaceted understanding of target selection and intervention from the perspective of social welfare in communities.;세계보건기구 WHO에서는 건강에 대해 “완전한 신체적, 정신적, 사회적 안녕의 상태와 단순히 질병이나 장애가 없는 상태”라고 정의하며, 신체적, 정신적, 사회적인 모든 부분에서의 전인적인 건강을 추구하고 있다. 오늘날 건강문제가 사회문제로 인식되면서, 국가나 사회차원에서 이를 보장할 의무를 지니게 되었다. 성인들의 신체적, 정신적 건강이 위협받고 있는 오늘날 보건복지부에서는 국민건강증진법 제4조 제1항에 따라 2002년에서 2010년까지의 국민건강종합계획을 수립하면서 금연, 절주, 신체활동, 영양을 건강생활실천사업의 핵심 분야로 선정하였고, 지역보건소 중심으로 교육, 홍보, 법제도화 등의 거시적 접근 위주로 이뤄지고 있다. 이에 따른 결과로 흡연의 증가가 둔화되었다는 평가가 있었지만, 여성 흡연율은 증가하는 추세가 전망되며, 음주율은 지속적으로 증가하고 있다고 분석되고 있다. 또한 영양사업과 운동증진사업의 경우 전국 보건소를 통해 보급되고 있으나 우리나라 국민의 성인 비만율은 증가하는 추세로 사실상 국민건강증진종합계획이 성공했다고 볼 수 없다. 이러한 결과에 대해 선행연구들은 대상자에 대한 다각적인 이해 부족, 사업수행기관 및 네트워크 체계의 미비, 사업의 우선순위에 대한 문제점, 홍보와 교육 중심의 평면적인 접근에 대한 평가를 하고 있다. 선행연구에서도 언급된 것처럼 건강생활실천 확산의 결과가 저조한 이유에 대해 이해하기 위해서는 대상자를 다각적으로 이해해야할 필요가 있다. 이에 본 연구에서는 대상자의 정신건강에 대한 이해의 필요성을 제시하며, 최근 정신건강 영역에서 관심이 급부상되고 있는 우울을 중심으로 이해하고자 하였다. 이에 우리나라 만19세 이상 만64세 이하의 성인을 대상으로 이들의 건강생활실천 행위(흡연, 음주, 신체활동, 규칙적 식생활)가 우울여부에 어떠한 영향을 미치는 가를 밝히고, 그 결과에 따라 건강생활실천행위에 대한 실천적인 제언을 하고자 한다. 본 연구는 국민건강영양조사 제5기 3차 원시자료를 이용하였고, 복합표본 로지스틱 회귀분석을 활용하여 분석하였다. 분석결과는 다음과 같다. 첫째, 건강생활실천 행위에 따른 우울증 유병여부와의 연관성을 분석한 결과, 건강생활실천 변수인 ‘현재 흡연여부’, ‘1년간 음주빈도’, ‘1주일간 중등도 신체활동’, ‘규칙적 식사의 합’ 모두 통계적으로 유의했다. 둘째, 건강생활실천행위에 따른 우울증 유병여부와의 영향요인을 분석한 결과, 여성이면서, 경제활동이 없는 사람, 주관적 건강상태가 안 좋은 사람, 흡연하는 사람이 우울할 가능성이 높은 것으로 나왔다. 건강생활실천의 변수들 가운데에서는 흡연만 통계적으로 유의미한 결과를 나타내었는데, 실제 주관적 건강상태의 영향력이 매우 커서 다른 변수들의 영향이 줄어들거나 유의미하지 않은 것으로 볼 수 있다. 성별에 있어서는 여자의 우울유병이 남자에 비해 odds가 4.101배 높은 것으로 나타났고, 경제활동이 없는 대상자의 우울유병은 경제활동이 있는 대상자에 비해 odds가 1.568배 증가하는 것으로 나타났다. 주관적 건강상태는 자신의 건강상태가 좋지 않다고 느낄수록 우울유병은 증가하였고, odds는 2.291배씩 증가하는 것으로 조사되었다. 건강생활실천 행위 중에서는 흡연이 우울유병에 유의미한 연관성이 있다고 조사되었다. 흡연의 경우 흡연을 늘릴수록 우울 유병의 위험성은 증가한다는 결과를 나타냈고, odds는 1.168배 씩 증가하는 것으로 나타났다. 이러한 결과를 바탕으로 본 연구에 대한 실천적인 함의를 제시하자면 다음과 같다. 첫째, 흡연을 늘릴수록 우울유병의 위험도를 증가시킨다는 본 연구 결과에 따라, 건강생활실천행위 중 금연을 담당하는 부서인 보건소 금연상담소와 우울증 유병 대상자들을 주로 담당하고 있는 지역정신건강증진센터와의 사업적인 협조체계 구축을 통해 유기적인 관계를 유지할 필요가 있다. 둘째, 주관적 건강인지가 우울유병에 미치는 영향관계를 통해 자신이 건강하지 못하다고 느낄수록 우울증의 위험도가 높아짐을 확인했다. 이는 우울증상 중 왜곡된 사고 등의 증상으로 이해할 수 있어, 우울증 고위험 대상자들을 조기발견 및 개입, 사후관리에 관심을 기울일 필요가 있다. 셋째, 본 연구에서는 성별과 경제활동상태에 따른 우울유병에 유의미한 영향관계가 나타났다. 이는 대상자에 대한 복합적인 이해의 필요성을 제시하는 것으로, 실천현장에서 대상자에게 적절한 서비스를 주기 위해서는 대상자의 어려움을 둘러싼 복합적이고 다각적인 이해가 필요하며, 이러한 이해가 일관적이고 효과적으로 적용되기 위해서는 서비스 및 기관의 우선순위를 결정할 수 있고, 다각적인 이해가 반영된 표준화된 기준에 대한 고려도 필요할 것이라 사료된다. 넷째, 선행연구 및 본 연구에서도 제시되었듯이, 건강생활실천확산을 위해서는 대상자를 둘러싼 다각적인 요인들에 대한 이해가 필요하다. 다각적인 요인들을 이해한다는 것은 대상자의 다양한 욕구들에 대응할 수 있어야 한다는 것으로, 우울과 같은 정신건강적인 문제를 복합적으로 가지고 있어, 자신의 문제를 혼자 해결하기 힘든 대상자들에게 개별적인 접근이 필요하며, 이들에 전인적인 건강에 대한 복합적 접근을 위해서는 다학제적인 전문가들과의 긴밀한 네트워크가 필요할 것이다. 다섯째, 본 연구 결과에서는 흡연의 영향으로 우울을 유발할 수 있는 요인 중 금단증상에 대한 결과는 나타내지 못하였다. 이에 추후 연구에 있어서는 지역 사회 내에서 사회복지적 관점을 가지고, 대상자 발굴 및 개입을 하는데 있어서 다각적인 이해 및 대처를 돕기 위한 실천적인 제언을 할 수 있는 연구가 수행되어야 할 필요가 있다고 사료된다.
