Abstract
제주조릿대(Sasa quelpaertensis Nakai)는 우리 나라 중부 이남지역에서 자라는 대나무과 식물로 오래 전부터 약용 및 식품원료로 사용되어 왔다. 제주조릿대 추출물은 최근 시행된 시험관, 동물 연구에서 항비만 효과가 보고되어 왔다. 이에 본 연구에서는 성인 비만환자에서의 제주조릿대 추출물의 체지방 조절기능을 확인하고자 하였다. 본 인체적용시험은 2012년 9월부터 2013년 12월까지 1개 대학병원에서 20세 이상 65세 미만의 성인으로 체질량 지수가 25kg/m^(2)이상 35kg/m^(2)이하인 90명을 대상으로 시행된 평행비교, 무작위배정, 이중맹검, 대조식품 비교 시험이다. 인체적용시험에 참가한 피험자들은 대조군 또는 저용량 시험군(1g/일), 고용량 시험군(2g/일) 중 한군으로 무작위 배정되어 각각 위약 혹은 제주조릿대 추출물로 제조된 정제를 12주간 복용하였다. 인체적용시험 종료시점에서(12주째) 대조군, 저용량군, 고용량 군에서의 신체계측치수는 통계적으로 유의한 군간 차이를 보이지는 않았으며, 허리 둘레는 통계적으로 유의하지는 않으나 수치적으로 군간의 차이를 보였다(p=0.068). 12주째 복부지방 면적은 통계적으로 유의한 군간 차이를 보이지는 않았으며, 총 복부지방 면적(total fat mass)은 수치적으로 시험 군간의 차이를 보였다(p=0.052). 12주째 혈중 총 콜레스테롤 농도는 각각 212.61±34.226 g/dL, 196.88±34.95 g/dL, 201.32±33.5 4g/dL로 시험군 간에 한계 수준의 차이를 보였다(p=0.05)으나, 혈중 중성지방 농도, 저밀도 콜레스테롤 농도, 고밀도 콜레스테롤 농도는 시험군간의 유의한 차이를 보이지 않았다. 대조군과 저용량 시험군(1g/일)을 비교하였을 때, 12주째 신체계측치수는 대조군과 1g/일 투여군에서 통계적으로 유의한 군간 차이를 보이지는 않았으나, 시험 종료 시점의 허리 둘레는 각각 각각 91.1± 6.68 cm(대조군)과 90.00±7.52 cm(1g/일 투여군)으로 조릿대 1g/일 투여군에서 다소 낮은 경향을 보였다(p=0.061). 12주째 복부 지방 면적 역시 각각 415.63±89.667cm^(2)과 392.93±86.254cm^(2)로, 조릿대 추출물 투여군에서 대조군에 비하여 지방 면적이 다소 감소하는 경향을 보였다(p=0.062). 제주조릿대 추출물은 한국인 비만 성인에서 12주간의 인체적용시험 결과 대조군에 비하여 허리 둘레 및 총 복부 지방 면적, 중성 지방의 감소 경향을 보였으며, 안전성 측면에서는 유의한 차이를 보이지 않았다. 본 임상시험은 최초의 인체 적용 대상 시험으로, 동물 시험 등 전 임상 시험에서의 체지방 감소 효과와 유사한 경향을 보였으며 대조군과 유사한 정도의 내약성을 보였다. 향후 보다 대규모의 임상시험을 통하여 체지방 감소효과를 확인할 수 있을 것으로 기대된다.;Recently, interest in dietary phytochemicals with good potential for obesity treatment and prevention has been increasing. Sasa quelpaertensis Nakai is an edible dwarf bamboo that belongs to the family Poaceae. Its leaf extract shows anti-obesity properties from preclinical studies. This study is the first human clinical trial aiming at the assessment of antiobesity property of Sasa quelpaertensis leaf extract. This study is 12 week parallel, randomized, double-blind, placebocontrolled clinical trial designed to evaluate the anti-obesity property and safety of Sasa quelpaertensis leaf extract in 90 Korean obese adults whose BMI were within 25kg/m^(2) to 30kg/m^(2). This study has been conducted at single center in Korea from Sep 2012 to Oct 2014. Ninety subjects have been randomized in a 1:1:1 fashion to placebo, low dose (1g/day) SQE treatment and high dose (2g/day) SQE treatment group. The primary endpoint was the fat mass change measured by fat CT after 12week treatment. After 12 week treatment, mean waist circumferences were numerically different among treatment groups (p=0.068). Total abdominal fat mass measured by fat CT were marginally different among the treatment groups(p=0.052). Total cholesterol level difference among the arms were also marginally significant (p=0.05). I also compared 1g/day SQE treated group to placebo group. After 12 weeks treatment, waist circumference were numerically lower in 1g/day SQE treatment group compared to placebo group (p=0.061). Total abdominal fat mass measured by fat CT were also numerically lower in 1g/day SQE treatment group compared to the placebo group (p=0.061). Sasa quealpaertensis extract (SQE) has shown the tendencies to reduce waist circumferences, total abdominal fat mass and triglyceride level in Korean adult obese people after 12-week treatment. SQE showed similar safety profile with placebo. This study was the first human clinical trial. It showed the consistent anti-obesity properties with preclinical data. I expect to confirm the significant anti-obesity property of SQE with long term clinical trials with bigger number of subjects based on these results in the future.
