Abstract
The lateral habenula nucleus (LHb), an epithalamic nuclei playing a central role in linking the limbic and basal forebrain to the midbrain in the mammalian brain, connects reciprocally with serotonergic neurons in the raphe nucleus as well as dopaminergic neurons in the ventral tegmental area (VTA) and substantia nigra pars compacta (SNc) and negatively regulates monoamine system which influences emotional behavior. The LHb is involved in the regulation of several mental functions such as reward, stress, pain, maternal behavior and cognition. Recently the LHb has received great attention because neurons in these nuclei encoded negative reward signals in response to aversive stimuli; the LHb has been suggested to play an important role in the generation of reward and motivational behaviors and psychiatric disorders like depression. Histochemical analysis with receptor binding study and in situ hybridization revealed that LHb expresses various monoaminergic receptors including various types of 5HT and DA receptors. LHb reciprocally are connected with the raphe nuclei and VTA, implying that synaptic responses of LHb neurons may be modulated by monoamine. The LHb receives excitatory glutamatergic inputs via stria medullaris (SM) fibers from medial prefrontal cortex (mPFC), lateral hypothalamus (LH) and globus pallidus internal segment (GPi). These glutamatergic inputs have been reported to be potentiated in the learned helplessness model of depression. The reduction of LHb’s activity by means of lesion, pharmacological inhibition and DBS improves the symptoms of depression in model animals and human patients, suggesting that the activities of glutamatergic input is important in emotional state. However, electrophysiological properties of LHb are not fully understood. In addition, LHb neurons receive 5HT and DA input. Therefore, it is important to elucidate the modulatory role of the glutamatergic synapse and monoamine inputs in the neuronal activity. The first part of this study addresses how 5HT modulates glutamatergic transmission in the LHb. A whole cell voltage-clamp recording was used to examine basic properties of glutamatergic synapse onto LHb and its modulation by monoamine. Evoked EPSCs of LHb induced by stimulating SM fibers contained very little NMDAR-mediated components. CNQX-sensitive AMPAR-mediated synaptic responses showed strong inward rectification which consist of both GluR2-lacking AMPA receptor (Ca2+-permeable AMPAR) and GluR2-containing AMPA receptor (Ca2+-impermeable AMPAR). P/Q-type voltage-gated Ca2+ channels at the presynaptic terminal of SM fiber were responsible for glutamatergic transmission and presynaptic ryanodine receptors also affected the glutamatergic synaptic response. In addition, 5HT-induced long-term depression (5HT-LTD) at glutamatergic LHb synapses through 5HT1B receptor by decreasing the probability of glutamate release. The induction phase of 5HT-LTD involving the 5HT-induced acute depression (AD) of glutamate release was mediated mainly by activation of 4-aminopyridine (4-AP) and α-dendrotoxin (DTX)-sensitive voltage-gated K+ channel. Whereas the maintenance phase of 5HT-LTD required the cAMP-dependent pathway and NO-dependent Ca2+ signaling from ryanodine-sensitive intracellular Ca2+ stores. Together, these results indicate that 5HT has a role of tonic inhibition of excitatory glutamatergic input onto LHb neurons by hyperpolarizing its presynaptic terminals through the activation of 5HT1B receptors. The second part of this study searches for dopaminergic modulation of glutamate-mediated synaptic responses in the LHb. Recently, it has been reported that aversive stimuli excite LHb neurons, whereas rewarding stimuli inhibit them leading to the negative modulation of the firing of midbrain dopaminergic neurons. This suggests that LHb functions as a negative reward source. Neurons in the LHb reciprocally connect to midbrain dopamine neurons, so that DA can affect neuronal activity and function of the LHb. Indeed, the LHb receives dopaminergic input from the SNc and the VTA and also expresses DA receptors including D1-like receptor (D1R) and D2-like receptor (D2R). Although DA or dopaminergic drugs are reported to affect LHb in some studies, the effect of DA on excitatory LHb synapses and its underlying mechanism remain unexplored. Extracellular and whole-cell recordings showed that DA mainly suppressed intrinsic excitability and inhibited synaptic responses in the LHb via D2Rs. In about 65% of LHb neurons, spontaneous firing decreased following the application of DA or D2R selective agonist quinpirole, and these inhibitory responses were completely blocked by D2R antagonist sulpiride. DA and D2R agonist quinpirole also suppressed the generation of depolarizing current-evoked Na+ spikes in about 60% of LHb neurons. In addition, DA dose-dependently and reversibly decreased the eEPSCs of LHb neurons through presynaptic mechanism. The DA-induced inhibition of eEPSC was mediated by D2R. Presynaptic D2R activated 4-AP sensitive K+ channels, but presynaptic GIRK channels and AC-cAMP signaling did not produce DA-mediated synaptic inhibition. The results of the present study suggest that the presynaptic suppression of LHb by 5HT1BR and D2R is important to generate a defensive response against stress because suppression of the excitatory glutamatergic input to LHb neurons could result in attenuate depression-like behaviors in animal models. The monoamine-mediated suppression mechanism may exert a common inhibitory action through a Gi/o protein-coupled process and the subsequent recruitment of second messenger system, possibly in a synergistic way. Given that 5HT can induce the plasticity of glutamate synapse in the LHb and many antidepressants have a property of enhancing monoamine actions the abnormal adaptation of synaptic plasticity in LHb can be associated in pathogenesis of depression induced by chronic stress or drug abuse and addiction.;외측고삐핵(Lateral habenula, LHb)은 포유류의 뇌에서 변연계 (limbic), 전뇌기저부(basal forbrain) 영역과 중뇌(midbrain) 영역을 연결하는 중심적 역할을 하며, 세로토닌(serotonin) 신경세포가 존재하는 솔기핵(raphe nucleus)과 도파민(dopamine) 신경세포가 존재하는 복측 덮개(ventral tegmental area, VTA), 흑색질 치밀부(substantia nigra pars compacta, SNc) 등에 존재하는 신경세포와 신경회로를 이룸으로써 정서기능에 중요하게 작용하는 모노아민 시스템(monoamine system)을 억제적으로 조절한다. LHb는 보상, 스트레스, 통증, 모성행동, 인지 등의 전반적인 정서적 행동 조절에 기여한다. 최근 연구를 통해 LHb의 활성화가 부적 보상(negative reward)에 대한 신호로써 작용하며 스트레스 반응에 관계된다는 사실이 알려지면서 그 중요성이 증가하고 있다. 해부학적으로 LHb는 raphe와 같은 모노아민 시스템과 상호적으로 연결되어 있으며, 세로토닌에 대한 결합 부위와 여러 종류의 세로토닌 수용체 (receptor)의 mRNA가 발현되어 있다고 알려져 있다. 마찬가지로 LHb는 VTA영역의 도파민을 포함하는 신경세포와 직접적으로 연결되어 상호적으로 조절하며, LHb 신경세포는 도파민 수용체를 발현하고 있다. 이는 세로토닌과 도파민이 신경 조절 물질(neuromodulator)로써 작용하여 신경세포의 흥분성 또는 시냅스 반응의 조절, 가소성 유도 등과 같은 LHb 신경세포의 기능에 영향을 줄 수 있다는 것을 의미한다. 현재까지 보고된 바에 따르면, LHb 신경세포는 수질선조(stria medullaris, SM)를 통하여 내측전전두엽(medial prefrontal cortex, mPFC), 외측시상하부(lateral hypothalamus, LH), 창백내핵(globus pallidus internal segment, GPi) 등으로부터 대표적인 흥분성 신경전달 물질인 글루타메이트 신호(glutamatergic input)를 받으며, 지속적인 스트레스를 받은 우울증 동물 모델에서는 이 시냅스 반응이 강화(potentiation)되어 있다. 실제로 LHb를 제거하거나 약물학적 방법 또는 뇌심부자극술(deep brain stimulation) 등을 사용하여 LHb 활성을 줄이면 우울증의 증상이 향상되는 것을 동물 모델, 환자들에서 확인할 수 있다. 그러므로 LHb의 정상적인 생리학적 기능과 비정상적 조절에 의한 질병의 발병에 대해 깊이 이해하기 위하여 LHb 신경세포에 대한 전기 생리학적 특성과 시냅스 특성에 대한 연구가 필요하다. 또한 세로토닌과 도파민은 중요한 신경 조절물질이기 때문에 글루타메이트 시냅스의 특성뿐만 아니라 LHb 신경 활성과 시냅스 반응에 대한 모노아민의 영향을 조사하는 것이 매우 중요하다. 본 연구의 첫 번째 파트에서는, Whole-cell voltage-clamp recording 기법을 사용하여 LHb로 들어오는 흥분성 글루타메이트 시냅스에 주목하여 시냅스의 기본적 특성과, 모노아민 시스템에 의한 조절 기전에 대하여 연구하였다. 먼저 LHb 신경세포의 글루타메이트 시냅스의 특성에 대하여 살펴보면, 글루타메이트에 대한 시냅스 반응인 흥분성 후시냅스 전류 (excitatory postsynaptic current, EPSC)는 대부분 CNQX에 의해 억제되는 AMPA 수용체를 통한 반응이었고, NMDA 수용체를 통한 반응은 매우 적었다. 그리고 AMPA 수용체는 칼슘(Ca2+)에 대해 투과성이 있는 GluR2-lacking AMPA receptor (CP-AMPAR)와 칼슘에 대한 투과성이 없는 GluR2-containg AMPA receptor (CI-AMPAR)가 혼재되어 존재하였다. 글루타메이트의 분비과정에는 전시냅스 세포의 말단(presynaptic terminal)에 존재하는 P/Q-type 전압의존성 칼슘 통로(voltage-gated Ca2+ channel)가 주로 관여하는 것을 확인하였다. 또한 시냅스 전 세포 내의 칼슘 저장소(intra Ca2+ store) 또한 ryanodine 수용체를 통해 칼슘을 분비하여 글루타메이트에 대한 시냅스 반응에 영향을 주었다. 또한 세로토닌은 LHb 신경세포의 글루타메이트성 시냅스의 장기약화(long term depression, LTD)를 유발하는 것을 확인하였다. 세로토닌은 전시냅스 신경세포(presynaptic neuron)에 존재하는 5HT1B 수용체를 활성화하여 글루타메이트의 방출을 직접적으로 줄이는 역할을 하였다. 이러한 세로토닌에 의한 LTD는 세로토닌 존재 하에서 빠르게 나타나는 장기약화 유도 단계(induction phase, acute depression, AD)와 세로토닌의 제거 후에도 지속되는 장기약화 유지 단계(maintenance phase, long term depression, LTD)로 나눌 수 있었다. 세로토닌이 글루타메이트 방출을 억제하는 초기 단계(5HT-AD)에서는 4-aminopyridine (4-AP)과 α-dendrotoxin (α-DTX)에 의해 차단되는 전압의존성 칼륨 통로(voltage-gated K+ channel)가 관여하였다. 즉, 세로토닌은 전시냅스 말단에 존재하는 5HT1B 수용체를 활성화하며, 이는 전압의존성 칼륨채널을 열어 과분극을 유발하여 활동전압에 의해 유발되는 글루타메이트 방출을 줄이게 된다. 세로토닌에 의한 LTD가 일어나기 위해서는 전시냅스 말단에서의 칼슘의 증가가 필요하였고, 전시냅스의 세포 내 칼슘 저장소에 존재하는 ryanodine 수용체를 통한 칼슘 방출이 관여하는 것을 확인하였다. 세로토닌에 의한 LTD 유지 단계(5HT-LTD)에는 AC-cAMP 신호전달과 산화질소 의존성 신호전달과정(NO-dependent signaling pathway)이 모두 관여하는 것을 확인했다. 그러므로 세로토닌은 LHb 신경세포의 시냅스 가소성을 유발함으로써 LHb 기능을 조절하는데 중요한 역할을 할 것으로 생각된다. 본 연구의 두 번째 파트에서는, 도파민에 의하여 LHb 신경세포의 활성이 영향을 받을 수 있는지 확인하였다. LHb는 기대했던 보상을 받지 못하거나 혐오 자극을 받았을 때 활성이 증가하고 보상을 받았을 때 활성이 억제되며, VTA의 도파민성 신경세포를 부적으로 조절한다고 알려져 있다. 즉, LHb는 부적 보상에 대해 반응하여 기능하는 영역이다. LHb는 중뇌의 도파민성 신경세포와 상호적으로 연결되어 신경 회로를 이루며, 실제로 VTA로부터 도파민을 받아 mesohabenular pathway를 형성한다. 그러므로 도파민은 LHb의 신경 세포의 활성과 시냅스 반응에 영향을 줄 수 있다. 실제로 LHb영역에는 도파민에 반응하는 D1-like 수용체(D1-like receptor, D1R)와 D2-like 수용체(D2-like receptor, D2R)가 모두 발현되어 있다고 알려져 있다. 비록 도파민과 도파민성 약물에 대하여 LHb 신경세포가 반응한다는 것이 몇몇 연구들에 의해서 알려져 있지만, LHb로 들어오는 흥분성 시냅스에 대한 도파민의 영향과 그 기전에 대해서는 명확하게 밝혀지지 않았다. 본 연구에서는 extracellular recording, whole cell recording 기법을 이용하여 도파민이 LHb 신경세포의 내재적 흥분성(intrinsic excitability)과 시냅스 반응에 대해 영향을 줄 수 있는지, 그리고 어떠한 기전을 통하여 작용하는 지에 대해 살펴보았다. 도파민은 주로 D2R을 통하여 내재성 흥분성과 시냅스 반응을 억제하는 것으로 확인되었다. 약 65%의 LHb 신경세포에서 도파민을 처리하였을 때 자발적으로 발생하는 spike가 억제되었고, 이는 D2R에 특이적인 작용제(agonist)에 의해 유사한 반응이 유발되었다. 또한, D2R에 특이적인 길항제(antagonist)를 처리한 경우 도파민에 의한 spike 억제가 효과적으로 차단되었다. 자극에 의해 발생하는 Na+ spike 또한 LHb 신경세포의 약 60%에서 도파민과 D2R에 선택적인 작용제에 반응하여 억제되는 것을 관찰하였다. 한편 도파민은 글루타메이트 시냅스의 반응을 도파민 농도 의존적으로, 가역적으로 억제하였다. 이러한 도파민에 의한 시냅스 반응은 전시냅스 세포에 존재하는 수용체를 통해서 나타나는 것으로 확인되었다. D2R에 선택적인 길항제는 도파민의 반응을 막을 수 있었으며, D2R 작용제를 사용해 유사한 반응을 유도할 수 있었기 때문에, D2-like 도파민 수용체가 관여하는 것을 확인할 수 있었다. 흥미롭게도 각각의 도파민 수용체의 D1R, D2R 길항제에 대해 흥분성 후시냅스 반응이 각각 감소, 증가하였다. 이는 내재적으로 존재하는 도파민에 의해 흥분성 시냅스 반응이 지속적으로(tonically) 조절되고 있다는 것을 의미한다. 도파민에 의한 흥분성 시냅스 반응의 감소는 4-AP를 처리한 경우 억제되었다. 그러므로 도파민은 D2R를 발현하고 있는 LHb 신경세포의 활성을 억제하였고, 전시냅스의 D2R는 4-AP 민감성 전압의존성 칼륨 통로(4-AP sensitive, voltage-gated K+ channel)를 통하여 글루타메이트 방출의 확률(release probability)을 줄였다. 이러한 기전을 통하여 D2R은 LHb 영역으로부터 전달되는 전반적인 활성을 감소시키며 이는 보상 과정과 스트레스 반응과 같은 LHb의 생리적 기능에 대하여 중요한 역할을 할 것으로 생각된다. 본 연구 결과를 종합해보면, 시냅스 전 세포에 존재하는 5HT1B 수용체와 D2 수용체는 LHb 글루타메이트의 방출을 줄임으로써 신경세포의 시냅스 반응을 억제하는 역할을 하였다. 이는 LHb 시냅스가 과도하게 활성 되었을 때 모노아민이 LHb 영역으로 방출되어 그 시냅스를 억제함으로써 병리적 상황이 일어나는 것을 막을 수 있음을 가리킨다. 또한 세로토닌과 도파민은 G-protein을 공유함으로써 칼륨 통로를 통한 공통된 기전을 갖기 때문에 시냅스 반응 억제에 시너지 효과를 나타낼 수 있으리라 예상한다. 마지막으로, 세로토닌이 LHb의 글루타메이트성 시냅스의 가소성을 유발할 수 있다는 것을 고려하면, LHb가 관여하는 장기적인 스트레스에 의한 우울증 유발 또는 약물 남용, 중독 등과 같은 병리적 과정에 시냅스 가소성에 의한 시냅스 효능 조절이 중요하게 관여할 수 있을 것으로 사료된다.
