Abstract
Fabry disease is an X-linked lysosomal storage disorder caused by mutations in the gene encoding the lysosomal enzyme a-galactosidase A (a-Gal A), which results in glycosphingolipid storage in plasma and different cell types throughout the body. Involvement of kidney, heart, and central nervous system threatens life span, and fibrosis of these organs is considered to play a major role. I aimed to determine the gene expression profiles of epithelial and mesangial cells grown in culture and exposed to glycosphingolipid relevant to Fabry disease. I also tried to identify biomarkers of epithelial mesenchymal transition (EMT) process leading to kidney fibrosis using microarray. The identified genes were validated by reverse transcription (RT) - PCR, quantitative real-time PCR and/or western blot assay. Foxp2 (epithelial), IL4, MYH11 were upregulated in epithelial cells and Foxp2 (mesangial) and WT1 were upregulated in mesangial cells. With a few exceptions (such as IL4), upregulated genes in epithelial cells were activated by globotriaosylsphingosin (lysoGb3) and in mesangial cells, activated by globotriaosylceramide (Gb3). In addition, I observed WT1 expression in epithelial and mesangial cells and its specific contribution to renal fibrosis depending on target cells. In conclusion, I identified gene expression profiles related to EMT, leading mechanism of kidney fibrosis, and also glycolipids (Gb3 or lysoGb3) cell specific activation patterns and role of WT1 as ‘transcription activator’ inducing mesangial hyperplasia, ‘transcription repressor’ in epithelial cells leading to epithelial sclerosis. In addition, glycolipids specifically react to target organs or cells, depending on their stages of renal fibrosis where they respond, they may present the selectivity of their roles. Now, the precise mechanism of glycolipids how to contribute to progression of fibrosis and how to react differently with biomarkers of EMT depending on target cell type is not clear yet, but with this new insight into the characteristic gene expression profiles, I suggest not only for development of biomarker of Fabry nephropathy but also, for development of the specific anti-fibrotic (anti-EMT) drugs, further study is needed.;파브리병은 라이소좀 효소인a-galactosidase A (a-Gal A)를 표현하는 유전자의 돌연변이로 인해 혈장 및 신체의 각 기관 및 세포에 글리코스핑고리피드들이 축적되어 특징적인 임상 양상을 나타내는 X-linked lysosomal storage disease이다. 특히, 파브리병이 신장, 심장 및 중추 신경계를 침범할 경우 생명을 위태롭게 할 수 있으며, 이는 이 기관들이 점점 섬유화가 진행되면서 돌이킬 수 없는 기관의 손상과 기능 상실이 발생하기 때문이다. 기관이나 조직의 섬유화가 진행하는 데에는 상피세포의 간질세포로의 전이 (epithelial to mesenchymal transition, EMT)가 가장 중요한 기전으로 여겨지고 있다. 저자는 파브리병의 발병과 관련이 있는 글리코스핑고리피드 (Gb3또는 lysoGb3)들을 신장의 사구체 혈관 상피세포 및 혈관사이사세포와 함께 배양하여 파브리 신증 (Fabry nephropathy)에서의 유전자들의 발현 양상을 알아보고, Microarray 분석법을 이용하여 신장의 섬유화를 일으키는 주요 기전으로 알려진 상피세포의 간질세포로의 전이 (EMT)의 생체 지표를 확인하고자 하였다. 분석을 통해 밝혀진 유전자들은 역전사 PCR (RT- PCR)과 상대정량법을 이용한 real-time PCR및 western blot 분석을 통해 확인하였다. 상피세포에서는 Foxp2(epithelial), IL4, MYH11가 의미 있게 증가하였고 혈관사이사세포에서는 Foxp2 (mesangial)과 WT1이 의미 있게 증가하였다. 상피세포에서 의미 있게 증가되었던 유전자 (Foxp2(epithelial), IL4, MYH11들은 주로 globotriaosylsphingosin (lysoGb3)에 의해 활성화된 반면, IL4는 globotriaosylceramide (Gb3)와 반응하여 증가하는 것으로 보였다. 혈관사이사세포에서는 globotriaosylceramide (Gb3)가 Foxp2 (mesangial)과 WT1의 발현을 활성화시키는 것으로 관찰되었다. 태생기 이후의 신장에서는 다리세포 (podocyte)에서만 WT1의 발현을 관찰 할 수 있다고 알려져 있으나, 본 연구에서처럼, 파브리 신병증과 같은 대사성 신증의 경우 WT1유전자는 상피세포와 혈관사이사세포 모두에서 유전자의 발현을 관찰할 수 있었다. 더욱이 WT1의 발현 양상도 상피세포에서는 의미 있게 감소한 반면, 혈관사이사세포에서는 의미 있게 증가하는 것도 관찰하였다. 이미 알려진 바와 같이 WT1이 혈관사이사세포에서는 ‘transcription activator’로 작용하여 혈관사이사세포의 과증식 (overproliferation)을 유도하는 반면, 혈관상피세포에서는 ‘transcription repressor’로 작용하여 상피세포경화 (epithelial sclerosis)를 유도하는 기능의 선택적 특성도 확인할 수 있었다 이 사실로부터 WT1뿐만 아니라 파브리 신증에서 발현이 확인된 유전자들은 표적 기관이나 세포, 신증의 경과 및 활성화된 글리코리피드 (Gb3또는 lysoGb3)의 종류에 따라 EMT 및 신섬유화의 각 과정에 기여하는 역할과 발현 양상이 선택적으로 나타날 수 있다는 사실도 확인할 수 있었다. 이 사실로부터 WT1뿐만 아니라 파브리 신증에서 발현이 확인된 유전자들은 표적 기관이나 세포, 신증의 경과 및 활성화된 글리코리피드 (Gb3또는 lysoGb3)의 종류에 따라 EMT 및 신섬유화의 각 과정에 기여하는 역할과 발현 양상이 선택적으로 나타날 수 있다는 사실을 확인할 수 있었다. 결론적으로, 본 연구를 통해 출생 이전부터 이미 섬유화가 시작되는 것으로 알려진 파브리병에서 신섬유화의 주요 기전이 EMT와 관련되었으며, 이에 관한 유전자 발현을 분석하여 EMT 관련 생체 지표들을 발견하고 이들의 발현 양상을 확인 할 수 있었다. 이러한 생체 지표들의 발현 양상은 섬유화가 진행되는 시기와 장소 (표적 기관 또는 표적 세포) 및 활성화된 글리코리피드 (Gb3또는 lysoGb3) 의 종류에 따라 선택적으로 다르게 관여할 것이라는 가능성도 제시하였다. 현재까지 글리코리피드가 섬유화 과정에 기여하는 기전이나 표적 세포에 따라 EMT 관련 생체 지표들이 어떻게 다르게 반응하는 가에 관하여는 정확히 알려진 바가 없다. 앞으로 더 많은 연구가 필요하겠지만, 본 연구는 파브리병과 관련된 세포와 신장조직에서의 유전자의 특징적인 발현 양상을 관찰함으로써 파브리 신병증의 생체 지표를 발견하는 것과 함께 특징적인 신섬유화를 방지할 수 있는 약물을 개발하는데도 도움이 될 것으로 기대한다.
