포괄간호서비스를 제공하는 간호사의 간호업무와 직무스트레스, 직무만족에 관한 연구

Abstract

본 연구는 포괄간호서비스를 제공하는 간호사의 간호업무 수행도, 중요도, 지식정도를 확인하여 간호업무의 실태를 파악하고 간호사의 직무스트레스와 직무만족의 관계를 확인하기 위해 시행한 서술적 상관관계 연구이다. 연구대상자는 포괄간호서비스병원 1차 시범사업에 참여중인 5개 의료기관의 보호자 없는 병실에 근무하는 간호사 119명이었다. 일반적 특성, 간호업무별 수행도, 중요도, 지식정도, 직무스트레스 및 직무만족으로 구성된 총 486문항의 구조화된 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 SPSS WIN 21.0 프로그램을 이용하여 분석하였으며, 결과는 다음과 같다. 1. 포괄간호서비스를 제공하는 간호사의 12개 간호업무영역(대분류)의 수행도는 ‘환자간호관리 및 정보관리’(4.23±0.65), 중요도는 ‘투약 및 수혈’(4.59±0.47), 지식정도는 ‘투약 및 수혈’(4.41±0.59)이 가장 높았다. 2. 포괄간호서비스를 제공하는 간호사의 간호업무영역별 149개 간호업무(소분류)의 수행도는 ‘경구약 투약’(4.86±0.54), 중요도는 ‘의식상태 관찰’(4.86±0.42), 지식정도는 ‘활력징후 측정’(4.71±0.63)이 가장 높았다. 3. 포괄간호서비스를 제공하는 간호사의 직무스트레스는 평균 49.42±8.67점(점수범위 0∼100점)이다. 하위영역별 평균 점수는 ‘보상부적절’ 52.75±15.73점, ‘조직체계’ 51.4±13.3점, ‘직무자율성결여는 49.58±12.97점, ‘직무불안정’ 43.14±17점, ‘직무요구’ 43.14±17점, ‘관계갈등’ 29.51±14.64점이었다. 4. 포괄간호서비스를 제공하는 간호사의 직무만족은 문항 평균 점수 2.78±0.793점(점수범위 1-5점)이다. 5. 포괄간호서비스를 제공하는 간호사의 직무스트레스와 직무만족은 서로 유의한 부적상관관계를 보였다(r=-.604, p<.001). 본 연구의 결과, 포괄간호서비스를 제공하는 간호사의 간호업무 중 일상생활 보조업무의 수행이 증가하였으며 포괄간호서비스를 제공하는 간호사의 직무스트레스와 직무만족 정도는 모두 낮았고 서로 유의한 상관관계가 있었다. 향후 포괄간호서비스병원의 간호업무 기준을 마련할 때 간호업무 실태를 파악한 본 연구의 결과가 활용될 수 있을 것으로 기대하며 포괄간호서비스병원의 효과적인 간호사 인력 관리를 위해 간호사의 직무스트레스를 낮추고 직무만족을 높이는 전략을 마련할 필요가 있다.;The purpose of this study was to investigate nurses’ work performance, importance, knowledge, the degree of job stress and job satisfaction of nurses working in the hospital with no guardian. The sample of the study were 119 nurses, who work in the hospital with no guardian. The questionnaire used in this study composed of 488 items: background characteristics, performance, importance, knowledge of nursing tasks, job stress and job satisfaction. The collected data were analyzed using the SPSS WIN 21.0 to calculate frequencies, percentage, mean, standard deviation and Pearson correlation coefficient. The major findings of this study were as follows: 1. The most frequently performed nursing task was patient nursing management and information management(4.23±0.65), the most importantly perceived nursing task was medication and transfusion(4.59±0.47) and the most knew nursing task was medication and transfusion(4.41±0.59) by nurse with no guardian. 2. The most frequently performed nursing activity was oral medication(4.86±0.54), the most importantly perceived nursing task was observating mental status(4.86±0.42) and the most knew nursing task was checking vital sign(4.71±0.63) by nurses working in the hospital with no guardian. 3. The mean score of nurses’ job stress was 49.42±8.67(possible range of score 0∼100). 4. The mean score of nurses’ job satisfaction was 2.78±0.793(possible range of score 1∼5). 5. Nurses’ job stress was significantly associated with job satisfaction(r=-.604, p<.001). The data indicated that nurses working in hospital with no guardian perform daily living assistance services more than before. The mean scores of nurses’ job stress and job satisfaction were low. This study will be useful to establish for nursing standard of hospital with no guardian. And also, effective managements of nurses’ job stress and satisfaction need to be established.
