학령전기 사시아동의 수술 전 교육프로그램 개발 및 효과

Abstract

사시는 학령전기 아동에 매우 흔하게 발생하며, 특히 외형적으로 증상이 나타남에 따라 신체적·정서적인 건강문제를 함께 지니고 있다. 이러한 사시아동의 건강문제는 아동에게만 국한된 것이 아니라 부모의 정서적 문제까지 초래하게 되어 학령전기 초기에 수술이 많이 행해지고 있다. 수술은 학령전기 아동은 물론 부모에게 많은 스트레스를 유발하여 아동과 부모의 불안을 증가시킨다. 그러나, 대부분이 수술 전 학령전기 아동과 부모를 대상으로 충분한 교육이 이루어지지 못하고 있는 실정이다. 따라서 아동과 부모가 수술에 대해 효율적으로 대처할 수 있는 교육프로그램이 필요하다. 이에 본 연구자는 사시아동과 부모를 위해 Seels & Richey(1995)의 ADDIE모형을 근거로 수술 전 필요한 교육내용에 관한 문헌고찰과 요구 조사를 실시한 후 clown nurse를 이용한 아동용·부모용 수술 전 교육프로그램을 개발하였다. 아동용 교육프로그램은 4개의 주제영역으로 나뉘어 파워포인트를 교육매체로 개발하였으며, 부모용 교육프로그램은 4개의 주제영역으로 나뉘어 팜플렛을 교육매체로 개발하였다. 이렇게 개발한 교육프로그램은 S대학병원에서 clown nurse를 이용하여 수술 전 사시아동과 그들의 부모에게 실시하였다. 개발한 교육프로그램의 효과 검증은 서울 소재 S대학병원에 사시수술을 받은 아동과 부모를 대상으로 대조군 27쌍, 실험군 23쌍으로 총 50쌍을 무작위로 선정하여 사전 ·사후 측정하였다. 아동에게는 불안과 통증을 측정하였으며, 부모에게는 불안과 만족도를 측정하였다. 연구결과 clown nurse를 이용한 교육프로그램을 받은 실험군 아동의 불안(U=52.500, p<.001)과 통증(U=116.000, p<.001)은 대조군에 비해 유의하게 낮았다. 실험군의 부모는 행동 불안(U=22.500, p<.001)·상태기질 불안(t=-3.672, p=.001)이 모두 유의하게 낮았고, 간호만족도(U=153.000, p=.001)는 유의하게 높았으며, 프로그램의 전체 만족도(22.70±2.305)는 매우 높게 나타났다. 본 연구를 통해 개발된 clown nurse를 이용한 수술 전 교육프로그램은 사시아동의 불안, 통증과 부모의 불안을 감소시키고, 부모의 만족도를 향상시킬 것으로 기대한다. 따라서 본 교육프로그램은 수술 전 교육에 있어서 임상에서 다양한 간호중재방법으로 활용될 수 있을 것으로 기대한다.;Strabismus is common in preschool children and can lead to problems in physical and emotional health. The current study aimed to develop and assess the effectiveness of a clown nurse preoperative educational program for parents and preschool children about to undergo surgical correction of strabismus. This program was developed according to the Analysis, Design, Development, Implementation, Evaluation model of Seels & Richey (1995), and was based on both the extant literature and needs assessment of parents of children with strabismus. The children's education program was divided into four subject areas, and was delivered using PowerPoint. The parent education program was also divided into four subject areas, and was delivered via pamphlets. The program was presented to both parents and children by clown nurses. Fifty child-parent dyads were randomly assigned to either an experimental (n = 23) or control (n = 27) group. Using a pretest-posttest design, the effectiveness of the program was tested with both groups at S university hospital, Seoul. Child and parent anxiety, child pain, and parental satisfaction were measured in both groups. The results showed that there was a significant decrease in anxiety (U = 52.500, p<.001) and pain(U=116.000, p<.001) in children in the experimental group, while parents in the experimental group showed a decrease in behavioral(U=22.500, p<.001) and state-trait anxiety(t=-3.672, p=.001). Parents in the experimental group also reported increased satisfaction with the nursing education(U=153.000, p=.001) and high satisfaction(22.70±2.305) with the overall education received. These results indicate that the clown nurse education program developed in this study can be expected to decrease the anxiety of parents and children, decrease the pain of children, and increase the satisfaction of parents. Therefore, we recommend that this education program be utilized for pediatric pre-operative nursing education programs in hospitals.
