Abstract
연구 배경과 목적: Multidrug resistance 3 (MDR3)은 ATP-binding cassette, subfamily B, member 4 gene(ABCB4)에 의해 발현되는 약물 수송체로 주로 간세포의 담관막에 발현되어 있으면서 간세포 안에서 밖으로 내인성 지질인 포스파티딜콜린을 수송할 뿐만 아니라 약물과 생체이물의 수송에도 관여한다. 진행성 가족성 간내 담즙정체증 3형과 낮은 인지질 관련 담석증은 ABCB4의 돌연변이에 의한 희귀 간 질환이다. 본 연구에서는 진행성 가족성 간내 담즙정체증 3형과 낮은 인지질 관련 담석증 환자들에게서 발견된 ABCB4 돌연변이들의 기능적 특징을 알아보고자 하였다. 연구 방법: 진행성 가족성 간내 담즙정체증 3형에서 발견된 13개의 ABCB4 과오 돌연변이와 낮은 인지질 관련 담석증에서 발견된 3개의 ABCB4 과오 돌연변이를 대상으로 하였다. 각 돌연변이의 수송능은 인사이드 아웃 원형질 막 소포를 이용한 ATP-의존성 파클리탁셀 수송을 통하여 측정하였다. ABCB4 돌연변이가 수송능을 변화시키는 기전을 알아보기 위해 세포 표면 바이오티닐레이션, 면역블로팅, 면역형광염색을 실시하였다. 연구 결과: 진행성 가족성 간내 담즙정체증 3형 환자들에서 발견된 ABCB4 돌연변이 중 8개(G126E, A364V, A511T, T593A, M630V, P726T, A737V, A1193T)가 야생형과 비교하였을 때 유의하게 감소된 수송능을 보였다. 이중 2개(G126E, P726T)의 돌연변이는 심각한 단백질 절단을 초래하였다. 뉴클레오티드 결합 도메인에 위치한 3개(A511T, T593A, M630V)의 돌연변이는 MDR3의 발현을 아주 심하게 감소시킨 반면, 세포내 도메인에 위치한 1개(A364V)의 돌연변이는 약한 단백 발현 감소를 일으켰다. 나머지 2개(A737V, A1193T)의 돌연변이는 세포막으로의 이동에 결함이 있는 돌연변이로, 이들 돌연변이의 세포막 내 MDR3 발현과 수송능은 약리학적 샤페론인 사이클로스포린 에이에 의해 복구되었다. 낮은 인지질 관련 담석증 환자들에게서 발견된 돌연변이의 경우, 2개(F165I, S320F)의 돌연변이가 야생형과 비교하였을 때 유의하게 감소된 수송능을 보였다. 또한 두 돌연변이 모두 유의하게 세포 전체 및 세포막 MDR3의 발현 감소를 나타냈다. 연구 결론: 본 연구에서는 ABCB4 돌연변이의 분자적 기전을 밝혔다. 이는 진행성 가족성 간내 담즙 정체증 3형과 낮은 인지질 관련 담석증, 혹은 MDR3의 결핍과 관련된 다른 질환에 대한 돌연변이-특이 치료법 및 진단 키트 개발에 도움을 줄 수 있을 것이라고 기대된다.;BACKGROUND: Multidrug resistance 3 (MDR3) coded by the ATP-binding cassette, subfamily B, member 4 (ABCB4) gene is hepatocellular canalicular transporter associated with not only biliary secretion of phosphatidylcholine but also efflux of drugs and xenobiotics. Progressive familial intrahepatic cholestasis type 3 (PFIC3) and low phospholipid-associated cholelithiasis (LPAC) are rare hepatic diseases which are caused by mutation of the ABCB4 gene. This study was conducted to investigate the functional characterization of several mutations of ABCB4 gene found in PFIC3 and LPAC patients. METHODS: Thirteen missense ABCB4 gene mutations found in PFIC3 and 3 missense ABCB4 gene mutations found in LPAC patients were investigated. Transport activity of each mutant was measured through ATP-dependent uptake of paclitaxel using plasma inside-out membrane vesicles. To determine the mechanism how ABCB4 mutants change transport activity, cell surface biotinylation, immunoblotting and immunofluorescence staining were performed. RESULTS: Eight mutations (G126E, A364V, A511T, T593A, M630V, P726T, A737V, and A1193T) of ABCB4 gene found in PFIC3 patients showed a significant reduction in the transport activity compared to the reference. Among these 8 ABCB4 mutations, 2 mutations, G126E and P726T resulted in severe protein truncation; 3 mutations, A511T, T593A and M630V located in the nucleotide binding domain markedly decreased the MDR3 expression, while 1 mutation, A364V in the intracellular domains resulted in a less pronounced decrease in protein expression; the others, A737V and A1193T are trafficking-defective mutants, and the expression of MDR3 on the plasma membrane and transport activity could be rescued by a pharmacological chaperone, cyclosporin A. In the case of mutations found in LPAC patients, 2 mutations, F165I and S320F resulted in significantly reduced transport activity compared to the reference. Both mutants showed significantly decreased total and cell surface MDR3 expression. CONCLUSION: This study provides information about the molecular mechanisms of ABCB4 mutations that may contribute to development of new mutation-specific targeted treatments or diagnostic kits for PFIC3, LPAC or other MDR3-deficiency related diseases.
